Page 4 of 4 Battle Creek MI VAMC 515-20-4-702-0564 Mobile PACT Team Partitions Lisa Elliott 269 223-5272 STATEMENT OF WORK 1.0 GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for healthcare furniture for Battle Creek MI Facilities Mobile PACT teams. Teams see patients in 6 satellite locations requiring mobile partitions to screen patients and provide privacy. Screens must be able to withstand the new COVID cleaning protocols and meet the VHA IDIQ specifications of high-quality threshold while integrating excellent environmental stewardship. Each clinic is open 1-2 days per week. Screens will be folded and stored when not in use. All mechanism must be easily operable by a single employee without furniture knowledge. This requirement includes healthcare furniture and maintenance/warranty services. Service required is detailed in SV1 Healthcare Furniture technical requirements per VHA Healthcare Furniture multiple award IDIQ The Contracting Officer Representative (COR) is Lisa Elliott. Healthcare furnishings include Patient Procedure Area Modular Privacy Dividers, PP1. PP1 Patient Procedure Area Modular Case goods   Salient Characteristics: To include minimum of the following specifications:   a.       A line of storage, benches, TV cabinets, adjustable privacy dividers, and nurse cabinets used to divide space between patient procedure areas such as Oncology and provide storage for amenities. Varying heights from 29 up to 72 are desired.   b.      Surfaces and interiors of case goods shall be durable, easy to clean, free of crevices, and consist of replaceable parts. Softened edges are desired to prevent injury.   c.       Materials may be HPL, thermoformed veneers, and in limited cases, wood veneer. All internal surfaces shall be metal, high impact plastic or melamine surface over substrate. Backs of freestanding units shall be fully finished. All edges shall be finished with matching edge banding or thermoformed.   d.      Construction shall be seamless as much as possible. Drawer inserts are allowable to achieve this.   e.      All connections shall be metal-to metal joinery and be modular to allow for moving and repair.   f.        Adjustable leveling glides are required with optional toe kick base to allow owner application of wall base.   g.       Privacy dividers that can be open or closed can be made of acrylic, fabric that may be disinfected, or other solid nonporous decorative material. Please see the other categories listed below for ancillary products: Tables, Storage, Patient and Sleeper Seating Accordion style portable partitions with powder coated metal frames. Panel to be of non-porous material able to withstand the intense disinfecting protocol required for COVID cleaning. Hinges shall allow for up to 180 degrees of movement for various configurations. Locking swivel casters to be rubber to protect all types of floors. Panels to arrive assembled and ready for use. Assembly to be easy to move and use by a single individual. Individual panel to be approximately 39 ½ x approximately 72 high. Panels required at each location: Ludington- (3) 11 panel units Newago-(4) 11 panel and (1) five panel Hart- (2) 11 Panel units Baldwin- (4) 11 Panel units, Morley- (3) 11 Panel Big Rapids- (1) 3 panel The contractor shall provide all product, transportation, and supervision necessary to satisfy the needs of each ordering activity. 2.0 PERFORMANCE REQUIREMENTS The contractor shall provide and be responsible for the technical assistance, development and generation for final specifications. Furniture delivery to Battle Creek MI facility will be required in Early August. Duty hours will be 8:00am to 4:30pm. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. The contractor shall: Coordinate and provide all products and services to order, ship, deliver and install new furniture from the manufacturer until final acceptance by Lisa Elliott. Only include products specified within the IDIQ. Provide maintenance manuals for all products specified. 4.0 SUBMISSION OF QUESTIONS Questions: All questions must be received by 8/24/2020. All questions submitted for this solicitation must be electronically sent to the following email address: matthew.durden@va.gov. Questions received after 8/24/2020, may be addressed at the discretion of the Contracting Officer. 5.0 SUBMISSION OF QUOTATION Quote Submission: The contractors shall submit complete emailed electronic copy portable document format (.pdf) or Microsoft Excel (.xls) format of: Volume 1: (technical) drawings, (floorplans, isometrics, shop drawings, typicals etc.) complete manufacturer s product specifications Bill of Materials tagged per CLIN/ floorplan department and room etc. quantity of man hours for labor/installation product literature physical finish samples sustainability certification documents complete testing results / certificates warranty. Self-certifying statement confirming ability to meet project deadlines Volume 2: (pricing) price quote with separate line for labor/installation Bill of Materials with subtotals and tagged per CLIN/ floorplan Submissions can be sent in multiple emails to avoid computer system email size limitation, to Matthew Durden at matthew.durden@va.gov by 8/24/2020. Quotes submitted by any other method will not be considered. All proprietary information shall be clearly marked. The use of hyperlinks in quotes is prohibited. Late quotes will not be accepted for evaluation. The contractor is responsible for confirming the Government s receipt of the contractor s quote. Pricing must be within the established IDIQ Pricing. 6.0 EVALUATION CRITERIA Task order will be evaluated on: Bill of Materials with subtotals and tagged per CLIN /location. Cut sheets with product detail for each line item. Labor/Services appropriate to the scope of work. (no pricing included) Sustainability Certification 7.0 PRODUCT General All finished surfaces shall be free of scratches, mars, dents, or blemishes, and withstand staining, and exhibit to flaking, cracking, or loss or adhesion. Furnishings shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing. See attachment for technical requirement of products. 8.0 SERVICES Service required is detailed in SV1. Design Contract Tasks/Requirements The contractor shall allow for three (3) revisions per line item included in design services including updating Auto-Cad Drawings and PDFs as request by VA Interior Designer. The contractor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by VA Interior Designer. The product must not be placed into production without clearance from the VA COR. The contractor shall track manufacturing schedule and notify VA Interior Designer with updates of estimated completion date by email. The contractor shall attend a post-installation meeting to assess, address, and document any punch-list items and shall submit to the VA Interior Designer a remediation plan within 5 business days. The contractor shall provide 30 days free storage. The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission. Work Required by Others All work required by other trades shall be identified under this section by the quoter.
