Jared Zinsmeister Page 2 of 2 A.1 ADDITIONAL ATTACHMENTS Answer s to Vendor Questions: Can you provide the anticipated volume of supplies to be delivered from CBOC in Mt. Vernon to the VA Hospital in Seattle? Also, if items are to be picked up at the VA Hospital in Seattle, the anticipated volume of these items that will be taken back to CBOC in Mt. Vernon? The amount/volume of supplies being delivered to the VA Hospital in Seattle consist of outgoing mail, dirty/used RME (Reusable Medical Equipment), used for Dental and Tele-Derm procedures. Also included are lab bags from our Lab sent for processing by VA Hospital in Seattle. Likewise, items picked up at the VA Hospital in Seattle includes new RME for Dental and Tele-Derm and returning lab bags back to our lab department. If there are supplies that are picked up at the VA Hospital in Seattle for delivery to CBOC in Mt. Vernon it sounds like it will be a next day pick up? If it is a next day pick up from Seattle VA going back to CBOC in Mt. Vernon is there a fixed schedule for this pick up that will allow the driver to be back at Mt. Vernon CBOC for his 4:00 PM departure to Seattle? I am trying to determine the size of vehicle required and if one driver and one vehicle will be sufficient for our proposal. Almost most all supplies can wait the next day for delivery at CBOC Mt. Vernon. Current Courier is here between 1545-1600 M-F. Courier drives a small vehicle, only 1 person is required to accomplish this task. What was the cost of the final full option year of the previous contract? $44,476.50 How many drivers/vehicles are currently used to service the current contract? 1 driver, 1 vehicle. What is the vehicle size currently servicing this location? Small passenger vehicle. Will dry ice be needed for specimen transport? Is it the responsibility of the contractor to provide dry ice? No, dry ice is not needed. Can you confirm if there will be any STAT runs required for this contract, what is the required turnaround time, time of day, location(s), and expected trip volume per year and per location for additional pickup requests? No STAT requests, expected volume is 250 trips per year. What is the daily item load volume? The amount/volume of supplies being delivered to the VA Hospital in Seattle consist of outgoing mail, dirty/used RME (Reusable Medical Equipment), used for Dental and Tele-Derm procedures. Regarding specific employees information: It is common practice for small businesses that the drivers, cars and other equipment used for a contract are not sourced until after an award has been made, for cost effectiveness and to reduce loss. However, we will be forwarding our standard Training (HIPAA) Certificates-Procedures for your reference? That is left up to the individual vendor. Regarding evaluation factors for the price Volume: we usually like to provide a Pricing realism cost break-down, to include SCA minimum per year, the number of drivers, hours and miles that are estimated to be required for performance, and other costs. Is it acceptable that we forward this Pricing Realism chart with our Price response? It is not required information. Under Specific Tasks of SOW on pages 3 to 4, the solicitation states that ""Mount Vernon CBOC PICKUP: 307 South 13th St, Mount Vernon WA, 98274 to Veteran Affairs PugetSound Health Care System, Seattle Division, 1660 S. Columbian Way, Seattle, WA 98108. Pick-up will be at 4:00 p.m. with delivery by 7:00 p.m. Monday through Friday excluding Federal Holidays. Any pick-up items from the Seattle Division will be returned to MT Vernon CBOC on the following day during normal scheduled pick-up time. (GENERAL MEDICINE SERVICES)."" If the pick up schedule is from Monday to Friday by 4pm, then does the VA expect the delivery from Tuesday to Saturday by 7pm? No Saturday delivery is requested. Schedule is Monday Friday. Under Description of Services/Scope on page 3, the solicitation says that ""The Contractor shall provide courier services to transport laboratory specimens, medical and other supplies and pharmaceuticals from the Mount Vernon Community Based Outpatient Clinic (CBOC) to the Seattle Veteran Affairs Hospital."" Can you confirm the dimension (width and height) and weight of the load that the contractor will pick up and deliver to both locations? Loads can fit in a small passenger vehicle. Is There a current vendor performing this route? Yes. What is the contract number that is of the previous contract? VA260-16-F-0685. What was the total award for the final year of the current contract? $44,476.50 Is there a SF1449 that should have been attached to the RFP? No, this is a combined solicitation/synopsis, and does not require a SF1449.
