Description
VETERANS HEALTH ADMINISTRATION Mobile Deployable Pharmacy Shelter STATEMENT OF NEED 2 TITLE OF CONTRACT: Veterans Health Administration Mobile Deployable Pharmacy Shelters. SCOPE OF WORK: Veterans Health Administration (VHA) currently has a requirement to procure containerized, expandable, self-contained standalone mobile fold out shelter that will serve the Veteran and civilian populations as medical clinics. The shelters would be used to create an efficient clinic system which is a model of traditional primary care. BACKGROUND The VHA Office of Emergency Management purchased a Mobile Intensive Care Unit that can provide care for up to twenty-three (23) Intensive Care Beds. During the proof of concept, operational concept it was determined that a pharmacy unit was needed to support the unit. This unit will provide the platform and security/safety requirements to allow pharmacy staff to provide the service to the Veterans as needed. OBJECTIVES: The contractor shall perform the mandatory tasks and provide specific deliverables described below within the performance period stated in this Statement of Need (SON). MOBILE DEPLOYABLE PHARMACY SHELTER- CONTAINERIZED Provide Fold Out Pharmacy shelter - The contractor will provide one (1) Fold Out Medical shelter that contain at a minimum 310-320 ft2. Fold Out Pharmacy Shelter Construction The Fold Out Pharmacy shelter will be constructed so that it is reinforced to be able to withstand movement over the road. This reinforcement must protect the Fold Out Pharmacy Shelter to maintain the exterior integrity allowing it to be successful in rapid deployment. Exterior of Fold Out Pharmacy Shelter - Fold Out Pharmacy shelter will consist of hard side walls, roof and floor. The walls, roof and floor should fold and unfold in a manner that allows the unit travels within the legal dimension over the road. The Fold Out Pharmacy Unit once expanded will consist of three (3) rooms; General Pharmacy, Hazardous IV Room (Containment-Segregated Compounding Area (C-SCA)) and a Segregated Compounding Area IV room (SCA). All rooms shall be compliant with USP 797 and 800 requirements for these activities. Leveling of Fold Out Pharmacy Shelter - The Fold Out Pharmacy Unit must have a mechanism in place to allow it to be leveled on surfaces that consist of hard stand (blacktop, concrete or steel road plates) as well as grass, dirt areas. The Fold Out Pharmacy Unit should be constructed that a maximum of three (3) unskilled laborers can deploy/redeploy within 45 minutes. Water, sewer and electrical connections shall be preinstalled to a single point and quick disconnect if possible. Entry Ways - The Fold Out Pharmacy Shelter will have a door for entry into the unit that enters the General Pharmacy area. The Hazardous IV room and Segregated Compounding Area IV room will each have a door for entry/exit from inside of the Fold Out Pharmacy Shelter. The door into the structure and doorway into the general pharmacy area can maneuver over them without difficulty and requires (1) a deadbolt (with controlled key access), (2) electronic keypad access, (3) audible and monitored alarm system. The door openings to the IV rooms will be automated or another hands-free option. Windows The Fold Out Pharmacy Shelter must not have windows to the outdoors. Awning - The Fold Out Pharmacy Shelter will have a deployable awning unit outside of the front door that will provide shade and shelter during rain. A connected environmentally controlled hallway between the ICU Pod and the Pharmacy Pod is preferred to ensure drugs can be transported safely and without concern for inclement weather. HVAC - The Fold Out Pharmacy Shelter will be equipped with Heating, Ventilation and Air Conditioning (HVAC) that is attached to the unit and has the duct work installed inside of the unit that allows sufficient air flow to cool and heat the Fold Out Pharmacy Shelter must be able to maintain a constant interior temperature of 68o-70o Fahrenheit, with outdoor temperatures ranging from -25o to 120o Fahrenheit; and humidity between 30-60% All air to the unit must be filtered to a minimum of MERV 14. The pressure in the C-SCA must be maintained between negative .01 and neg .03 in wg. the airflow to the room must be 12 ach. A continuous [recorded] monitoring system with alarm is required. Power - The Fold Out Pharmacy Shelter must be wired for 120/240 volt electric, 125-amp single phase convertible load center. There must be at least a 100-amp break for permanent power connections for shore power/generator. Power Connections The Fold Out Pharmacy Shelter will have external electrical connections that will allow the unit to be connected to shore power and generator power Generator - An appropriate size generator that can power the Fold Out Pharmacy Shelter to the above specifications will be provided. This generator will operate on diesel fuel. The generator will be outfitted in a manner that allowed it to be maneuvered by hand or other means that do not require material handling equipment (forklift etc.) Tie Down System - The Fold Out Pharmacy Shelter will be constructed so that it can be secured during heavy winds. The building will be designed to sustain wind speeds up to 125 mph. The tie downs straps will be provided. Outside Appearance - The outside of the Fold Out Pharmacy Shelter will be white in color and branded with the Veterans Affairs Logo on all four sides of the structure. Interior Rooms There will be three (3) interior rooms; General Pharmacy, Hazardous IV Room (Containment-Segregated Compounding Area (C-SCA), and Non-Hazardous IV Room (Segregated Compounding Area (SCA). Interior Walls - The interior walls will be made of a smooth, solid material that will allow them to be cleaned and disinfected without causing any damage to the finish. Flooring - The flooring inside of the Fold Out Pharmacy Shelter will be of a membrane material that will allow the floors to be damp mopped without causing water damage. The floor should consist of a texture that prevents slippage under dry and wet conditions in the general pharmacy; for ease of cleaning, smooth flooring in the IV room is preferred Data Ports - The General Pharmacy Area will be equipped with a minimum of two (2) data port connections. The CAT 6 cables will be routed through the walls of the Fold Out Medical Shelter and all terminate at one central location within the office area of the Fold Out Medical Shelter. At the terminated end the CAT 6 cables will have the appropriate male connectors installed. Each data jack and CAT 6 cable will be labeled identifying the destination. Each data jack connection will be tested to CAT 6A standards. General Pharmacy Area Furnishings Countertops will be a solid, smooth, and impervious surface. Shelving will be positioned above and be the length of the counter tops and be -solid, smooth, and impervious surface. Cabinets There will be cabinets with doors located underneath of the countertops with option to lock. Allow for an opening that can store the stainless-steel cart (solid-no wire shelves) with locking doors on storage beneath, with wheels (referenced below). Workstations two (2) workstations are needed in this area. Data ports will be located near the stations. The workstation will be large enough to support a laptop, keyboard and a printer. Chairs two (2) adjustable chairs are required. The chairs allow a person to work at the workstation as well as the countertop as needed. Sink one (1) stainless steel no touch sink deep enough to wash to elbows will be provided. The faucet will be equipment with an eye wash station mounted. Mirror one (1) mirror approximately 1 x2' will be mounted on a wall allowing personnel to ensure the personal protective gear is being worn properly. PPE Dispenser a shoe cover dispenser will be provided. Additionally, a dispenser for gowns, gloves, face masks, beard covers, and hair covers will be mounted on the wall. Refrigerator one (1) commercial medical grade refrigerator will be provided to store nonhazardous drugs. The refrigerator will be equipped with an audible alarm and continuously recorded temperature monitor. Steel cabinet- Anchored in place and requires an electronically accessible locking mechanism. The cabinet will be certified to house controlled dangerous substances. Storage A stainless-steel cart (solid-no wire shelves) with locking doors on storage beneath, with wheels Tote with the ability to be locked for transport of drugs outside of the ICU Space Plastic pharmacy bins Twenty (20) approximately 8 x10 and ten (10) 10 x 12 Segregated Compounding Area (SCA) IV Room Furnishings Doors Aluminum framed doors with glass window inset. Doors will have a handsfree option to open A Horizonal Laminar Airflow Hood 37 in width, 34 in depth and 49 ½ in height is required. Recommendation is Germfree Model BZ-3SSRX. One (1) Waste Bin Sharps Container will be portable and NOT attached to the wall. Cart - Stainless-steel cart (solid-no wire shelves) with a flat surface and 3 solid shelves (not wire) with wheels. Stool Stainless-steel (adjustable). Provide two (2) stainless steel hooks on the wall that will allow gowns to hang for reuse. Hazardous IV room (C-SCA) Doors Aluminum framed doors with glass window inset. Doors will have a handsfree option to open. Biological Safety Cabinet (BSC) Germfree Stainless Steel BBF Series Benchtop Bioflow Chamber (BBF-3) Class II, Type A Biological Safety Cabinets with a CFM of 116-146 Must be ducted/vented to outside of the building including an exhaust fan which must be located on the roof of the unit. Airflow requirements Between 0.01 and 0.03 inches of water column relative to adjacent areas 12 air changes per hour (ACPH) Continuous Digital Pressure monitor on wall to monitor negative pressure. Exhaust Vent (behind fridge) must be placed low on the wall in this area Refrigerator one (1) a commercial medical grade refrigerator will be provided. The refrigerator must be placed in front of exhaust point to the outside; will be equipped with an audible alarm and continuously recorded temperature monitor. Waste Bins one (1)Hazardous Waste RCRA Bin, one (1) Yellow Trace Waste Bin, one (1) regular waste. Sharps Container will be portable and NOT attached to the wall. Cart - Stainless-steel cart (solid-no wire shelves) with a flat surface and 3 solid shelves (not wire) with wheels. Stool Stainless-steel (adjustable). Provide two (2) stainless steel hooks on the wall that will allow gowns to hang for reuse Shelving Stainless-steel shelving unit (solid shelving no wired)(4 to 6 shelves) Trailer or Method of Movement - The Containerized Fold Out Rigid Tactical Shelter will be moved on a trailer that can be towed with a standard ball hitch or pintle hook. This trailer must be road worthy and have all appropriate lights, brake lights and turn signals. The trailer will be equipped with electric brakes and have a 7- pin connector providing electric and control to the brakes and lights. Upload/Offload of Shelter- The unit will be constructed to upload and off load from a trailer using only integrated mechanisms and without utilizing any additional material handling equipment such as a forklift or crane. Storage Compartments on Trailer - The trailer will have storage compartments permanently mounted to allow storage of additional equipment as deemed necessary by Veterans Health Administration Office of Emergency Management (VHA OEM). All compartments will be keyed alike. Fire Extinguishers - Fire extinguishers mounted inside according to the applicable laws and statues will be provided for each of the Fold Out Medical shelters. Entry Ramps - Must conform to the ADA guidelines will be provided for each of the Fold Out Medical Shelters. These ramps should have handrails that are easily mounted on both sides of the ramp and are continuous. DELIVERY - The contractor will be required to deliver the Mobile Deployable Medical Shelter to the locations listed in the SON upon completion. The units will be delivered to the destinations NLT November 30, 2020. The contractor will provide the warranty information for equipment, manufactured by the contractor, so that the manufacturer warranty can be put in place. TRAINING The contractor will provide the appropriate training to VHA OEM staff on the proper way to load/unload the Mobile Deployable Medical Shelter- Tandem, setting up and taking down, the operational equipment inside the unit. Additionally, the care and maintenance of the shelter, trailer and associated equipment. The location and time of the training will be coordinated with the PM/COR. WRITTEN DOCUMENTATION The contractor will provide a comprehensive owner s manual and laminated quick operational guide for each unit. END OF STATEMENT OF NEED
