- STATEMENT OF WORK (SOW) BOTTLED WATER SERVICES The following is a Sources Sought/Request for Information notice. This posted notice is not a Pre-Solicitation or Solicitation document. The government is not currently soliciting bids, quotes or proposals at this time; however, reserves the right to do so at a later date. BACKGROUND The Lebanon VA Medical Center, 1700 S. Lincoln Avenue, Lebanon, PA 17042, has a requirement for a Firm Fixed Price (FFP) five (5) year Indefinite Delivery Indefinite Quantity (ID-IQ) Contract. The purpose of this ID-IQ contract is to provide potable bottled drinking water for the Lebanon VA Medical Center. PERIOD OF PERFORMANCE 10/01/2020 through 09/30/2025. Base Period: 1 Oct 2020 thru 30 Sep 2021 Option Period 1: 1 Oct 2021 thru 30 Sep 2022 Option Period 2: 1 Oct 2022 thru 30 Sep 2023 Option Period 3: 1 Oct 2023 thru 30 Sep 2024 Option Period 4: 1 Oct 2024 thru 30 Sep 2025 SCOPE OF WORK The contractor shall provide all management, labor, materials, and transportation necessary to provide filtrated bottled water free standing dispensers/coolers and 5-gallon bottled water refills to the Lebanon VA Medical Center. Unless specified as Government Furnished, the contractor shall provide all personnel, equipment, materials, transportation, quality control and supervision to perform this service. The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The contractor shall provide the quantities of water bottle dispensers and full water bottles listed below. The Government does not guarantee that it will place any orders under this contract in excess of the guaranteed minimum award amount. SPECIFIC WATER REQUIREMENTS Dispensers: The contractor shall provide cooler type bottled water, 5-Gallon dispensers, that dispense cold water and provide cold and hot water. Repair/replacement of contractor provided dispensers is at no cost to government. Bottled Water: The contractor shall deliver full bottles of water to designated delivery points without exceeding par levels in accordance with Water Delivery Sheet. After water delivery is completed, the contractor shall sign and provide a completed Water Delivery Sheet to warehouse personnel. Water Cooler Cups (cone-shaped): The contractor shall provide disposable drinking cups or cones to be replenished with water delivery, along with the appropriate dispenser attached to the cooler. Emergency Water Agreement: Under this agreement, the contractor shall supply available bottled water needs in the event of a local or national declared emergency or natural disaster at the same price awarded upon demand and outside delivery terms agreed upon limited to Lebanon VA Medical Center. DELIVERY The contractor shall deliver bottled water refills and pick up empty water bottles every other week, on Thursdays, for a total of 26 regular deliveries per year, to the following: Lebanon VA Medical Center 1700 S. Lincoln Avenue Lebanon, PA 17042 Deliveries will be made to various locations throughout the Medical Center. Services shall be performed during the hours of 7:30 a.m. through 12:00 p.m. on Thursdays. Delivery may include, but not exceed: 5-Gallon Bottles of Spring Water: minimum of 45 and maximum of 150 bottles per delivery Water Cooler Cups (cone-shaped): minimum of 1 and maximum of 2 cases per delivery On Call Delivery Service: Upon notification by the Technical Point of Contact (POC) or designee, the contractor shall deliver refilled bottles within one calendar day after notification in any area covered under this contract. The price for pickup and delivery services, to include On Call services, shall be included in the unit price for the bottled water refills. OTHER CONSIDERATIONS Security Requirements: All personnel operating a vehicle shall be a licensed driver. All vehicles shall be licensed and insured. The contractor s employees shall be restricted to those areas necessary to perform work under this contract as well as direct routes to and from work site. The government shall provide, without cost to the contractor, electricity. The contractor shall use electricity from existing outlets.
