Kingsbridge Road Rm. 9b16 Bronx NY 10468 Synopsis: Please note that this is not a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, Brooklyn VA Medical Center, is conducting a market survey for qualified firms capable of providing the following services for Brooklyn VAMC. Scope of Work The Government is in need of providing all necessary labor, equipment and material for cleaning, honing, restoring and Crystalizing an approximate 7,352 sq. ft. of Terrazzo surfaces that are located in the main entrance lobby areas, ER Hallway, Ground Floor Elevator Landing Area and First Floor of the Brooklyn VAMC. Through this effort, we will be making an important step towards our modernization goals by contributing to creating a clean, hygienic and pleasant environment for our outpatient Veterans who visit the clinics and for those who are admitted in the facility. SUMMARY OF WORK 1a. Summary of Major Tasks Contractor shall initially provide with a sample of work on a 3 ft. X 3 ft. area, which shall be evaluated by NYHHCS Management, COR, and EMS to appraise the overall job outcome. Contractor shall protect entire surrounding areas to prevent damage. Contractor shall Deep Clean all areas to be Crystalized Contractor shall utilize diamond grinding for entire stone surface to remove scratches and etching, using up to 3000 grit Diamond Disks. Contractor shall apply stone sealant where needed and Polish on all Terrazzo surfaces. Contractor shall crystallize entire stone surface to protect stone. Contractor shall polish entire stone surface to a high gloss finish utilizing appropriate polishing compound. Estimated Areas 1st Floor - 416 sq. ft. G Floor Main Entrance Lobby 3732 Sq. ft G Floor ER Hallway 1168 Sq. ft G Floor Elevator Landing area 2036 1b. Specific Requirements Contractor shall perform a site visit at NYHHCS Brooklyn VAMC to assess the job requirements and obtain confirming measurements. Contractor shall work with NYHHCS Engineering to assess any and all electric needs, upgrades, voltage, extensions, etc. for performance of required work. Contractor shall perform necessary consultations with NYHHCS Management, COR EMS and Engineering Service regarding scheduling of all work that needs to be performed. Contractor shall provide all management, tools, equipment and labor necessary to ensure that the terrazzo cleaning and restoration services are performed in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance. The contractor shall accomplish all tasks to meet all necessary requirements of this SOW. It is the responsibility of the contractor to verify all quantities, field conditions and measurements prior to starting work. The contractor shall notify the COR of any differing site conditions prior to starting. Contractor shall provide an on-site supervisor at all times when work is being performed and for the duration of this project. Contractor may coordinate with Site for storage for materials. The VA is not responsible for loss of tools, materials, equipment, etc. Contractor shall post visible construction signs and barriers prior to beginning any work. In areas where dust will be created, the contractor shall install life safety measures (ILSM) as follows in each area of work: Proper signage. Screen/barrier fencing or work area. Floor protection to prevent tracking dust into adjacent areas. Floor protection to prevent slippage. Contractor is responsible for any damage caused by his employees. All damages shall be repaired and/or replaced at no additional cost to the government. Contractor shall submit schedule to VA for approval. The Contractor shall not perform any work on utility services without prior coordination with COR (Contract Officer Representative). The contractor shall not interfere in any way with Hospital operations. Contractor shall complete all work within the specified number of calendar days. Contractor shall provide construction signage at location of work. Signage shall comply with OSHA, Life Safety and NFPA requirements. Contractor shall perform cleanup at any time when work area becomes cluttered and/or restricts traffic. Nevertheless, work area must be cleaned at the end of each work shift. After completion, re-install all previously removed furniture and/or equipment. Exceptional care must be taken to assure tidiness of work. Suitable temporary covers, masking, drop sheets, drapes and/or barriers shall be provided and maintained effectively where necessary to protect carpeting and furniture or other finishes that are to be painted or not to ensure what they are not exposed to paint and put at risk. These are to be removed when the protection is no longer required. Provide ""Caution Wet Surface"" signs as required to protect newly restored surfaces. Upon completion of the work all supplies and equipment, other materials, containers, debris and protective coverings shall be cleaned up and removed from site and the Contractor shall leave the area in a clean, neat, and orderly condition satisfactory to the BK EMS Assistant Chief, COR and Contracting Officer. In the performance of all work, the workers must be incompliance with safety regulations governing worker safety (OSHA: 29 CFR 1926 Safety and Health Regulations for Construction; NFPA, Life Safety Codes, etc.). All design items must meet TIL - Special Requirements as published on, http://www.cfm.va.gov/TIL/spclRqmts.asp. Work under this contract shall be carried out strictly in accordance with specifications attached and will meet US and Local codes. 1c. General Requirements Contractor shall be required to have VA issued identification badges for all of his/her personnel working on VA property. Contractor shall ensure that his/her personnel conduct themselves in a professional manner In the performance of all work, the workers must be in compliance with safety regulations governing worker safety (OSHA: 29 CFR 1926 Safety and Health Regulations for Construction; NFPA, Life Safety Codes, etc.). BASE SERVICES This is a single job option; no option years nor extensions. EQUIPMENT Contractor Is Responsible for utilizing equipment that has at its disposal to perform the installation of items contained herein. WARRANTY Warranty Period is necessary after completing the restoration process and will be determined according to stone restoration industry standards, which is usually a 5-year period of warrantee/service coverage which shall be at no cost to the VA. Upon completion, NYHHCS Brooklyn VAMC Management, COR, EMS and Engineering Service shall evaluate the Contractor s work, provide with punch list items and request repair and/or adjustments as required. SAFETY Contractor's Vehicles, Equipment, Operators, Mechanics, Technicians Must Meet All State, Federal and Local Safety/Maintenance Requirements. DELIVERY OF SUPPLIES/EQUIPMENT/PLACE OF SERVICE NYHHCS Brooklyn VA Medical Center 800 Poly Place, Brooklyn, NY 11209 ACCIDENT REPORTS Damage to VA Buildings of Other Property by contracted employees must be repaired or replaced immediately by contractor. TIMELY PERFORMANCE The period from award of contract to job completion should be no more than 60 days. WORK HOURS Normal work hours will be between 4:00pm and midnight, 7 days a week. As stated above, Contractor shall communicate with COR and Environmental Management Services and obtain approval regarding scheduling of work that needs to be performed. Due to the nature of the job and the room needed to perform the task, weekday and weekend hours may be employed to minimize any inconveniences to the normal day to day operations of the facility. No overtime payments will be authorized. Courtesy hours of early arrival or late departure will be allowed. EMERGENCY SERVICES Emergency calls during and after the installation must responded to eight (8) Hours of Notification. Only three (3) Occasions of Failure To Meet Deadline will be tolerated. Unacceptable response will be penalized. PRIOR TO CONTRACT STARTUP As stated above under Specific Requirements contractor shall complete site visit(s) to NYHHCS Brooklyn VA Medical Center - to obtain measurements, confirm job requirements and assess the logistics and placement of equipment necessary to perform the job. AFTER CONTRACT STARTUP As stated above under Warranty , upon completion, NYHHCS Brooklyn VA Medical Center Management, COR, and EMS shall evaluate the Contractor s work, provide with punch list items and request repair and/or adjustments as required to ensure all aspects of the job are complete. The North American Industry Classification Systems (NAICS) is 561720. Responses to this notice must be submitting in writing via email and must be received no later than August 14, 2020. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources for this procurement relative to NAICS 561720. Qualified offerors shall be experienced in addition to providers (sellers) capable of supporting this facility. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, and examples of similar facilities which you have provided services to DUNS number, address, and point of contact.
