- THE VAMC MILWAUKEE REQUIRES AN EYE ULTRASOUND UNIT THAT IS THE ELLEX EYE CUBED SYSTEM OR AN EQUIVALENT SYSTEM. THE REQUIREMENTS ARE LISTED BELOW. Statement of Work (SOW) for Ellex Company Eye Cube Background The Clement J. Zablocki VA Medical center is looking to replace the eye ultrasound system that is at end of life. The new system should be able to integrate with our EMR system and provide a detailed view of the inner eye. This new system will help ophthalmologist and ultrasound technicians diagnose eye disorders and diseases. This system should come with four different configurations to scan the eye. 2.0 Applicable Documents The contractor shall fill out the following documents to be considered for this procurement: 3.0 Requirements One (1) Ellex Eye Cubed Brand Name or Equal 3.1 Physical Requirements The unit shall weight no more than 15 kg The width shall be no more than 400 mm The depth shall be no more the 450 mm The height shall be no more than 200 mm 3.2 Electrical Requirements The system shall operate at 115 VAC, 60Hz, 20 amps 3.3 Software Requirements System shall be compatible with windows 10 3.4 System Components and Accessories The contractor shall provide the eye cubed system, accessories, related training, warranty and maintenance services identified below. Contractor shall deliver the eye cubed system, system related documentation and any accessories, training or maintenance services to the delivery points specified in the below section. EyeCubed V4 Console Wide screen high resolution monitor minimum of 1920 x 1200 A-Biometry A-Scan Probe Axial length biometry with multiple IOL formulae Holladay-I TM, Hoffer-Q TM, and SRK-T TM and Haigis 10MHz probe with internal fixation light for immersion and contact techniques Includes ability to print A-scan echo patterns and IOL calculations B- 10MHz Posterior Segment probe Diagnostic B-scan + cross-vector A-scan with ruler in mm 10MHz sealed B-Probe for true real-time scans at 25 frames/sec Electronic cross-hair measurements with variable velocities D-Diagnostic A-Mode Probe Standardized diagnostic A-scan for tissue characterization 8MHz parallel beam A-scan probe Electronic measurements with variable velocities Automatic tissue sensitivity gain reset function U-40MHz UBM Wide Field Anterior Segment Diagnostic B-scan with ruler in mm 40MHz UBM B-probe for true real-time scans Electronic measurements with variable velocities Provides wide-field imaging of anterior segment structures EyeCubed Cart SI DICOM Able to query/retrieve modality work list from the EMR and be able to store DICOM objects to the EMR and PACS. Quantity (7) Spare 10MHz B-Probe Quantity (7) Space Biometry A-Probe Quantity (10) Immersion Shell Adult Quantity (4) ClearScan Immersion Bags for 40MHz Probe (10/box) ClearScan® is an innovative single-use ultrasound probe cover. Consisting of an extremely thin film that is acoustically invisible, ClearScan® provides distortion free ultrasound imaging with the added benefit of patient comfort. In addition, the ClearScan® conical shape enables the safe and effective examination of alley quadrants without causing corneal abrasion. UPS- Uninterruptible Power Supply Quantity (2) Service Manuals In-Service Training Provide in-service to a minimum of 9 staff In-service shall take place during normal business hours Shall provide a day of follow-up training to staff Standard Manufacturer s Warranty Each Eye Cubed System carries a full 2-YEAR WARRANTY on all components including peripheral accessories included in the EyeCubed System purchase. Peripheral accessories typically included in the original purchase and thereby included in the 2-year warranty include: A-scan probes, B-scan probes, printer, monitor, equipment cart, keyboard, mouse, power transformer, scleral shells, and footswitch. If warranty service is required, Ellex Inc. will pay all shipping charges to repair or replace defective equipment. One (1) Preventive Maintenance visit per year (to be scheduled by customer). There are no fees for loaner components required while under warranty Trade-In Credit 4.0 Delivery and Acceptance 4.1 Delivery The contractor shall deliver and install the product. After contacting COR (or COR designee) at the VA, a specific time will be scheduled for the delivery of the product and the installation within 30 days of award. All quantities are listed above that should be delivered. They shall be delivered to the following address: Attn: Eye Clinic Clement J. Zablocki VAMC Building 111, warehouse 5000 West National Avenue Milwaukee, WI 53295 4.2 Testing and Certification Upon completing delivery and installation the contractor shall pre-test the entire system to verify that the system meets the manufacturers technical specifications and performance capabilities as set forth in the operator manuals, passes all internal diagnostics test, and VA requirements as set forth in this SOW. After pre-testing has been successfully complete the contractor shall contact the COR/Biomedical representative at the delivery site in writing that the system has met all requirements and is ready for Government testing and final acceptance by VA. 4.3 Government Acceptance Testing The contractor shall provide acceptance letter to the COR for approval. This will indicate government acceptance of the system and the beginning of the 12-month warranty period for that system. If the system does not pass the government acceptance test, the contractor shall return the system delivered and deliver a new system at no cost to the government. 5.0 Response Time Contractor shall respond with two (2) hours of notification to the customer service department If the problem cannot be solved by phone the contractor shall commence work with on-site physical response within 24 hours of receipt of first notification or as approved by COR 6.0 Information Security Contractor must have an approved national Business Associates Agreement (BAA). 7.0 Place of Performance The place of performance will be at Clement J Zablocki VA Medical Center, Milwaukee, WI. 8.0 Period of Performance All hardware and software shall have a full warranty period of 12 months. VA shall be entitled to all software updates to fix bugs and safety issues if that version of software is being supported by contractor.
