Attachment 4 - Floor Plan is being added to the solicitation. Chaz D. Bowling Contracting Officer Is the VA providing cleaning supplies?  No Is the VA providing the equipment? No Is the VA assigning storage for cleaning materials?  There should be designated space within floor for storage Who is the current contractor  Don t know - hired through lessor; this contract replaces lessor provided housekeeping services What is the current monthly or annual purchase expense for the services  Don t know; lessor would have it if willing to share Can you please clarify when waxing and stripping the floors are required? On page 3 section 1.4 states stripping and waxing floors will be required after clinic hours from 5pm to 8pm. On page 7 within the task/frequency chart item 18 states Initially and then twice/year (totaling 3 times a year). Page 7 item 6 Floor maintenance Sub items a) and b) states as requested per on site POC and staff. Can you please clarify so we can price accordingly.   Initially and every 6 months thereafter Can you please clarify what will and will not be provided by the Government/Client. Page 6 section 3 states no government furnished property or supplies will be provided. Page 9 item 17 states the dispensers and supplies will be provided by the government.  Dispensers and supplies not provided however existing dispensers may be used if successful offeror so chooses Is an onsite supervisor required for this contract  Not on-site but available to address problems related to sanitation services when required. How many restrooms and examination rooms are there? See attached floor plan Will the number of fixtures and dispensers be made available at the restroom/exam room?  Each sink shall be equipped with soap dispenser; each exam room and waiting area  equipped with hand sanitizer dispenser.  24 sinks (including 2 labs, 2 offices, staff lounge) 2 sinks in hallway (1 primary care side of hall, 1 specialty end of hall) 8 bathrooms with sinks Does the facility provide hand sanitizer and hand soap supplies? No Is there a requirement for the number of cleaning staff to be on site? No What is the average FTE and occupancy rate?  Average staff occupancy:       26 Average daily patients:          102 Is this contract only for 1 year term without any Option years.  Yes, extension of contract may be warranted at the discretion of the contracting officer should there be a delay with opening of the new clinic location. I couldn't locate the price schedule, will it be provided for bid submission.  Contracting officer can provide guidance regarding mandatory wage rates if applicable. Would there be any background investigation process?  Yes, staff working at the VA facility will be subject to background checks.
