Description: The Contractor shall provide all labor, supplies, travel, transportation, oversight on phone calls, lodging and miscellaneous services necessary to meet the purpose of the procurement. The Contractor shall perform the following tasks: Provide the list of all clinics performing ART in the U.S. and its territories in operation, closed or reorganized during each calendar year from 2020 through 2025, and for each year provide an Excel spreadsheet listing all clinics that performed ART and that were required under the Fertility Clinic Success Rates and Certification Act of 1992 (FCSRCA) to report data to CDC.
