- DEPARTMENT OF VETERANS AFFAIRS MEDICAL CENTERS BOILER PLANTS STATEMENT OF WORK Safety Device Testing SERVICE: Contractor to provide: The contractor shall provide equipment, labor, tools and materials to perform Safety Device testing on four (4) Boiler Systems, DA and surge tank at the St. Cloud VA. A. GENERAL: The contractor shall perform safety device testing on the boilers as outlined in the VHA Boiler Plant Safety Device Testing Manual, Fourth edition. Contractor shall report the results using the prescribed forms and methods. Contractor is responsible for all costs for transportation, lodging, meals and other personal necessities for its employees and or subcontractors (as applicable). Contractor is responsible for the shipping costs and logistics for all tools and equipment necessary for the performance of this contract. Work must be performed between the hours of 8:00 am and 4:00 pm. If a boiler plant, site or national emergency develops, Contractor will cease testing at the request of site personnel. Boiler plant must maintain operational readiness per VHA Directive 1810, Page 5, para 8, section c. Appendix A (d) (N+1 Minimum Requirements). No more than one boiler may be out of service at any given time for test or maintenance. At the conclusion of the site work, the Contractor will hold an exit meeting with appropriate VA representatives for the site to highlight preliminary findings. The Contractor shall identify deficiencies and steps that can be taken by the VA to address the identified deficiencies. A VA ST Cloud Contractor badge shall be worn by all Contractor employees while on premises. Contractor badges are issues in B-3, second floor, Facilities Maintenance and Operations Office. The C&A requirements do not apply, a Security Accreditation Package is not required. SPECIAL NOTE: Any anticipated sub-contracting work shall be presented in a sub-contracting plan, which is to include the identity of the sub-contractor who will be performing the work. The CO and the COR must approve, in writing, any sub-contract work prior to its commencement. B. SPECIFICS: Safety Device Testing; Contractor shall perform each test as detailed in the VHA Boiler Plant Safety Device Testing Manual, Fifth edition. 1. The Contractor is responsible for the identification and inspection of all appropriate areas and items related to this scope of work required to complete a comprehensive inspection. The on-site Inspection shall be in compliance with ""VHA Boiler Plant Safety Device Testing Manual -Fourth Edition"" and contain information observed through first-hand inspection and testing. In no case shall an inspection test render any boiler un-useable by the test alone. The plant consists of the following boilers; a. (3) Cleaver Brooks Fire Tube Boilers, Model 200-600-200, Built in 2008. b. (1) Cleaver Brooks Fire Tube Boiler, Model 200-400-200, Built 2008. c. Surge tank. Stamp# 13138. Serial# 08407. Max allowable working pressure 25 psi at 267 deg. Min allowable working pressure -20 deg. at 25 psi. Year built 2008. d. DA tank. Stamp# 13194. Serial# 08434. Max allowable working pressure 50 psi at 275 deg. Min allowable working pressure -20 deg. at 50 psi. Year built 2008. 2. Substitution of test procedures shall be rare. Whenever practicable, the exact test shall be performed. If a substitute test method is required to meet the intent of the test outlined in the manual, the alternate method shall be reviewed by the Boiler Plant Supervisor and the Section Chief/Assistant Chief. The results of the test shall be reported on the prescribed form and a notation of: a. Why the prescribed test could not be performed. b. The details of the alternate method used.
