- Transaction number 523-20-3-9176-0676 Date 06-22-20 A. GENERAL GUIDANCE 1. Title of Project: Delivery of Dry Ice to the VA Jamaica Plains and West Roxbury Research groups. 2. Scope of Work: The Contractor shall provide all labor, supervision and all other resources required to deliver the dry ice. 3. Background: The Jamaica Plain and West Roxbury groups have been receiving dry ice from for the last year and everyone has been very pleased with the product due to the lack of powder in the boxes they receive. That means that the product is all in usable form. JP will need a standing order while WR thinks ahead, so that next day ordering can be workable situation for them. It would be best to have an in-state Company. We all seem to prefer to order by phone or by email. 4. Performance Period: Twelve months from date of award. 5. Type of Contract: Firm Fixed Price B. CONTRACT AWARD MEETING The contractor shall commence work only when the Contracting Officer (CO) has conducted a kick-off meeting or has elected to waive the meeting. This meeting is very beneficial to the success of the project. Various aspects of work details can be discussed so there is a consistent understanding among all parties. Roles and responsibilities of the Contracting Officer s Representative (COR) and the Project Manager/Task Manager (PM/TM) are discussed and presented in the delegation of authority letters issued by the CO at the meeting. The nature of some small projects may not require a kick-off meeting with the CO. This will be determined on a case-by-case basis by the CO. C. GENERAL REQUIREMENTS The Contractor shall Dry Ice to VA West Roxbury and Jamaica Plains campuses on an as needed basis. The Contractor shall deliver on Tuesday if Monday is a Federal Holiday. Orders called in on Friday shall be delivered on Monday or Tuesday (Friday in WR) if Monday is a Federal Holiday. Federal Holidays: Jan - New Year's Day Jan - M.L. King's Birthday Feb - President's Day May - Memorial Day July - Independence Day Sept - Labor Day Oct - Columbus Day Nov - Veterans Day, Nov - Thanksgiving Day Dec - Christmas Day D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES As described above The Contractor shall dry ice to JP VA as a predetermined standing order until such time as the needs of the researchers need to change the amount of the order. The Contractor shall deliver dry ice to the WR campus on an as needed basis. Unless there is an emergency need for the dry ice and then they should try to accommodate the emergency need. E. EVALUATED OPTIONAL FEATURES N/A F. SCHEDULE FOR DELIVERABLES As stated above in D. G. CHANGES TO THE STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer (CO). A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the action so parties other than the CO shall be borne by the Contractor. H. REPORTING REQUIREMENTS The Contractor shall report its number of deliveries once a month to the manager of the dry ice deliveries. Xiaowei Dou at the VA West Roxbury facility and Alix Weaver at the Jamaica Plains facility should receive a copy of the reports. I. TRAVEL No travel is anticipated under this contract. J. GOVERNMENT RESPONSIBILITIES The VA West Roxbury facility and the Jamaica Plains facility will make every reasonable effort to provide access for delivery. K. CONTRACTOR EXPERIENCE REQUIREMENTS Qualifications of Key Personnel All Contractor personnel shall be trained in the safe handling and delivery of dry ice and know all of the rules and regulations associated with the shipping and delivery of dry ice as there are hazards involved in these actions. L. CONFIDENTIALITY AND NONDISCLOSURE The Contractor shall comply with all rules and regulations regarding the delivery of dry ice.. M. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS In accordance with the VA Handbook 6500.6, Appendix A, no additional VA Contractor personnel security requirements are needed for this project
