- Combined Synopsis/Solicitation � 123A9420Q0027 In support of the Office of Public Health and Safety (OPHS), the U.S. Department of Agriculture, Food Safety and Inspection Service (FSIS) intends to award a firm- fixed price purchase order, with four option years, for maintenance on its media preparation systems, located in FSIS�s Western and Eastern Laboratories. � Offerors responding to the RFQ constituted by this document must submit pricing information and a service plan commensurate with the specifications in the attached Statement of Work. �The total number of pages for the technical plan is not expected to exceed ten (10) pages in length. Performance of the required services will take place at 950 College Station Road, Athens, Georgia 30605, and 800 Buchanan Street, Albany, CA 94710.� Please see the attached Statement of Work for more details on the requirement, including model and serial number of the units to be maintained. This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued. This solicitation, 123A9420Q0027, is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-06. This acquisition is not set aside for small businesses. The contract specialist projects that the following line items will appear in the award document: Maintenance plan for media prep units in the Eastern Laboratory Maintenance plan for media prep units in the Western Laboratory See attached Statement of Work (SOW) for a description of requirements for the products to be acquired. The required services will occur at 950 College Station Road, Athens, Georgia 30605, and 800 Buchanan Street, Albany, CA 94710 from 8/1/2020 through 7/31/2021 for the base period of the purchase order resulting from this solicitation, and for corresponding one-year periods thereafter in each of option years 1, 2, 3, and 4. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Evaluation of offers will be conducted on a Lowest Price Technically Acceptable (LPTA) basis. Offerors should include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with their offers.
