- This is a Sources Sought Notice (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although ""proposal,"" ""offeror,"" contractor, and ""offeror"" may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as ""proprietary"" or ""confidential,"" intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by VISN 2 Contracting Office in New York, NY for the purpose of collecting information for Sharps & Regulated Medical Waste Disposal for NY Harbor Healthcare System 3 campuses and 2 associated Outpatient Clinics (Manhattan Campus 423 East 23rd Street, New York, NY 10010; Brooklyn Campus 800 Poly Place, Brooklyn, NY 11209; St. Albans Community Living Center 179-00 Linden Blvd, Jamaica, NY 11425; Harlem Community Based Outpatient Clinic- 55 West 125th Street, New York, NY 10027 and Staten Island Community Based Outpatient Clinic 1150 South Avenue, 3rd Floor, Staten Island, NY 10314). The VA is seeking to provide for this requirement as of October 1st, 2020. The NAICS code identified for this requirement is 562112 Hazardous Waste Collection. The requirement is detailed in the Statement of Work section of this document. (e) Cursory market research has identified several small businesses that may be able to provide for this requirement. Contractors that are able to meet the requirements in the attachment are encouraged to email a capability statement and full information to Monique Paltan at Monique.Paltan@va.gov by Thursday, July 30, 2020 at 3 PM EST. - Part of the purpose of this sources sought is to determine the viability of set aside to a specific socio-economic category. Contractors shall identify the NAICS code for the services being offered as well as their size status under said NAICS. If contractor is a Service-Disabled Veteran Owned Small Business (SDVOSB), a Veteran Owned Small Business (VOSB), or any other special category of contractors, then contractor is responsible for providing documentation in support of this. This information will be used to determine the viability of a set aside for this requirement. - Contractors shall also identify any Federal Supply Schedules that may carry the desired services. - Contractors shall identify pertinent point of contact for company, contractor DUNS number for size standard and socioeconomic verification in SAM and VIP. - Contractors shall include any relevant comments about the Attachment(s) if applicable. Statement of Work Regulated Medical Waste (RMW), Pharmaceutical Removal and Reusable Sharps Exchange Services BACKGROUND: Department of Veterans Affairs, New York Harbor Health Care System (NYHHS) is seeking removal services of Regulated Medical Waste (RMW), Pharmaceuticals and Disposable and Reusable Sharps Exchange Services for 12 months for each of 3 campuses and associated Outpatient Clinics. SCOPE: The contractor shall provide all labor and materials necessary for the collection and removal of Regulated Medical Waste (RMW) containers, Trace-Chemotherapeutic Waste, Pathological Waste, Pharmaceutical Gen-Ex Cartridge Waste and Reusable Sharps Container Exchange services for the Department of Veterans Affairs NY Harbor Healthcare System; Regulated Medical Waste, Trace-Chemotherapeutic Waste, Pathological Waste and Reusable Sharps Waste shall be processed and disposed of off-site by contractor at a facility in compliance with all Federal, State and Local requirements. Management and processing of regulated wastes and completion of all services shall be in accordance with all Federal, State, and Local regulations. REGULATED MEDICAL WASTE REMOVAL SERVICES: Regulated Medical Waste (RMW) includes but is not limited to items contaminated with blood and body fluids/waste, trace-chemotherapeutic wastes, pathological and microbiological waste, or other potentially infectious materials. Contractor shall: Provide the appropriate containers, shipping papers, pick-up, transportation, processing and tracking of regulated medical waste, trace-chemotherapeutic waste, pathological waste, and reusable sharps containers. Ensure provided containers are leak-proof, stackable, lidded, shall display the biohazard symbol and all other US DOT and state required marking to ensure compliance with all transportation requirements. All containers shall always be damage free and in good working order. Containers selected should be intend so that government employees will not be required to lift more than 35 lbs. Ensure provided containers are clearly identifiable for the regulated waste streams. Contractor shall provide labels for incinerate only waste streams (trace-chemotherapeutic waste & pathological waste. Remove full containers and replace them with an equal number of empty sanitized containers. Sanitizing shall be completed off grounds when the waste is processed. Ensure prior to departing any facility, contractor has obtained the COTR or designee s signature for confirmation of manifest including but not limited to: Pickup/Delivery Location, Pickup/Delivery Date, Proper Description of the Waste, Volume of Waste (number of containers picked up and/or pounds), US DOT Compliant Manifest with Unique Number (Shipping Papers). Transport the waste to the treatment facility. Treatment shall be by autoclaving, incineration, or other means in accordance with all applicable Federal, State and Local requirements. DISPOSABLE AND REUSABLE SHARPS EXCHANGE SERVICES: Sharps are included in the medical waste and must be in reusable sharps containers approved for shipping. Contractor shall ensure: If in disposable sharps containers, they will be sealed and packaged with the regulated medical waste. To furnish containers (to be determined by site requirements) at various locations throughout the facilities of the VA NY Harbor Healthcare System (Manhattan VAMC, Brooklyn VAMC, St. Albans Community Living Center, Harlem CBOC and Staten Island CBOC) accordingly as stated in this SOW. Containers include regulated medical waste containers, reusable sharps containers and associated hardware, disposable sharps containers, and trace-chemotherapeutic waste containers. Sharps include both reusable and disposable sharps containers (rigid, puncture proof, and leak proof containers, generally heavy-duty plastic with closable cover) for syringes, needles, scalpel blades, glassware used in the diagnosis of patients. SPECIFICATIONS: VA Medical Center will provide the red bags for the contractor owned red regulated medical waste roll off containers. Liners shall meet all shipping requirements (ASTM-D 1709 & ASTM-D 1922) for impact and tear resistance. Containers shall be sealed to prevent leakage and handled in a manner that will prevent bursting or tearing during transport by the Department of Veterans Affairs employees. Disposable sharps containers shall also be placed in the regulated medical waste containers. Reusable sharps containers will be shipped for decanting, processing, and washing. Liquid regulated medical waste will either be solidified or placed in liquid tight containers for shipping. No leaking containers will be shipped. Reusable and disposable sharps containers utilized should be lockable/sealable eliminate human intervention while container is in service or being transported. DISPOSAL OF NON-HAZARDOUS PHARMACEUTICAL WASTE BY RMW INCINERATION: Contractor shall dispose of nonhazardous waste pharmaceuticals by Regulated Medical Waste (RMW) incineration. The pharmaceuticals shall not include any Environmental Protection Agency (EPA) regulated hazardous wastes. Contractor shall obtain and maintain all necessary licenses and/or permits required to perform this work. The Contractor shall insure strict compliance with all Environmental Protection Agency (EPA) federal waste disposal regulations, New York State Department of Environmental Conservation and New York State Department of Health Laws, New York City laws and all other applicable regulatory requirements. The waste is generated on medical wards and therefore is presumed to be potentially infectious. The waste shall be handled, transported and disposed of as Regulated Medical Waste. The ultimate disposal shall be in a properly permitted RMW incinerator. The waste may not be sent for sterilization and landfilling, only incineration is acceptable. All containers are to be provided and resupplied by the contractor as needed. Containers shall be nominal 10-gallon capacity, leakproof and tamper proof and shall be designed such that medications may not be readily retrieved once they are placed in the container. Contractor shall submit a photograph and specification sheet of the proposed containers to the COR for approval prior to deployment. All containers shall be clearly labeled with a biohazard symbol and state that the waste is designated for destruction by RMW incineration only. The contractor is responsible for furnishing and replacement of all labels, inner bags, liners, locking devices and other appurtenances necessary to ensure that the waste containers are continuously maintained in a clean and secure manner. Responsibilities of the contractor include but are not limited to: provision of all needed waste containers, placement of waste containers in specified locations, direct pickup of waste containers at scheduled intervals, replacement with new empty containers, waste transportation and ultimate disposal by RMW incineration. Prior to departing the facility, Contractor shall obtain the COR or designee s signature for confirmation of manifest including but not limited to: pickup/delivery Location, pickup/delivery date, amount of waste (number of containers picked up and/or pounds) and destruction certification document/manifest number. The Contractor shall transport and incinerate the waste in accordance with all applicable Environmental Protection Agency (EPA), U.S. Department of Transportation (DOT), New York State Department of Environmental Conservation, Occupational Safety and Health Administration (OSHA), and any other state and federal guidelines. The Contractor is required to return the completed certification of destruction to the Contracting Officer s Representative (COR) within 15 calendar days from the date of removal from VA grounds. SCHEDULE OF PICKUPS: NONHAZARDOUS PHARMACEUTICAL WASTE DISPOSAL FOR RMW INCINERATION CAMPUS LOCATION PICKUP FREQUENCY Brooklyn VAMC Emergency Department (Ground Floor), 8W, 11E (ICU), 11W (four locations total) Twice Monthly Manhattan VAMC Emergency Department (Ground Floor), 4th Floor Clinical Addition (PACU), 11W(CCU/MICU), 13N (four locations total) Twice Monthly St. Albans CLC Building 85 (C-Building) 1st, 2nd and 3rd Floors (three locations total) Once Monthly NOTE: One waste container shall be provided in each room (specific room numbers to be designated). Each waste container shall have a nominal volume of ten (10) gallons. REUSABLE SHARPS CONTAINER EXCHANGE SERVICES: Contractor will conduct business and contact directly with the Contracting Officer's Representative (COR) assigned to oversee the items described in this section. The COR is the point of contact for all initial inquiries regarding this section of the contract. Contractor must provide at a minimum two (2) days per week full sharps container exchange services at both Manhattan VA Medical Center and Brooklyn VA Medical Center. The St. Albans VA Medical Center will require once per week service. Technician(s) must exchange Sharps Containers ½ to ¾ fullness and replace the container with clean Sharps Containers from all Sharps Container locations. Technician will perform the necessary collection/packing/storing of used Sharps Containers for collection/transport by a licensed regulated medical waste transporter. All personnel entering into any/all facility locations and performing any duties or services stated within this section must be current employees of the Contractor or its subcontractor, and must obtain VA-issued identification and shall be trained in the collection, packaging and transporting of Regulated Medical Waste and Sharps in accordance with all federal, state and local laws. Training records to be provided as part of technical proposal. Contractor-assigned personnel must report to Environmental Management Service (EMS) office prior to commencement of any and all work assignments. A log is provided by EMS office to record service visits as well as list concerns and problem calls. Concerns and/or problem calls must be addressed immediately at the commencement of the service visit and recorded in the log as ""completed. REUSABLE SHARPS CONTAINER TYPES: Contractor shall provide rigid, heavy-duty, puncture-resistant, leak-proof sharps disposal containers reusable and/or disposable, generally heavy-duty plastic with closable cover for syringes, needles, scalpel blades, and glassware used in the diagnosis of patients of the following sizes. All reusable sharps must include hardware for either wall mount (brackets or cabinets) and floor use (dollies or carts). See attached blueprints for container locations. Since the facilities increase and decrease their need throughout the life of the contact, contractors to include additional containers at no additional charge as needed. The containers allotment for extra containers beyond the blueprint will be for as follows: 1). Manhattan: Up to 50 small container locations and up to 25 large container locations annually 2). Brooklyn: Up to 50 small container locations and up to 25 large container locations annually 3). St Albans: Up to 25 small container locations and up to 10 large container locations annually 4) Harlem CBOC Up to 10 small container locations and up to 3 large container locations annually. 5) Staten Island CBOC Up to 10 small container locations and up to 3 large container locations annually. DOCUMENTATION FROM THE CONTRACTOR: The contractor shall prepare and submit to the COR an original signed Certificate of Destruction (manifest) for each pick up for each site. The contractor shall maintain proper records concerning each Certificate of Destruction issued. The manifest shall at a minimum contain the following: Pickup/Delivery Location, Pickup/Delivery Date, Amount of waste (number of containers picked up and/or pounds), Destruction Certification Document/Manifest Number, Destruction Certification Date, any other information as required by the COTR. Completed manifests confirming delivery and processing of the regulated medical waste shall be submitted monthly for the entire month. Must be within 60 days of the end of the month. Must be separated by facility then sequenced based on the date of pick-up. This will be in the form of a PDF and submitted to the COTR. Contractor shall provide monthly reporting that shows the pounds by waste stream shipped and processed to the COTR. This shall be tracked each month and provided in excel spreadsheet. TRAINING FOR CUSTOMER: Contractor shall provide annual training to government staff as required by the COTR. Training shall be for US DOT HAZMAT employees. Training shall cover General Awareness/Familiarization, Function Specific (including bulk outer packaging), Safety, Security Awareness. Shall also include bloodborne pathogens, universal precautions, and personal protective equipment as it relates to the safe handling of regulated medical waste. Training shall be ON SITE at the VA facilities and cover all shifts. Training shall occur over three days annually as follows: 1). Manhattan VA Medical Center 6 AM, 10 AM, 2 PM, 6 PM 2). Brooklyn VA Medical Center 6 AM, 10 AM, 2 PM, 6 PM 3). St Albans VA Medical Center 6 AM, 2 PM TRAINING FOR CONTRACTOR: All contractor on-site employees and on-site subcontractors under this contract or order are required to complete the VA's on-line Combined Privacy and Information Security Awareness Training Course and the additional Privacy Awareness Training Course annually. Contractors must provide signed certifications of completion to the COTR every 12 months during each year of the contract. This requirement is in addition to any other training that may be required of the contractor and subcontractor(s). BRACKETS & CABINETS: Where applicable and required, Contractor shall provide lockable uniform brackets for the reusable containers and or outer cabinets for the disposable cabinets. Cabinet must be capable of accepting the suitable container. All containers and cabinets shall be clearly labeled with a biohazard symbol and have a visual display window or means to check the level of the container or inner container. DOLLIES/CARTS: Department of Veterans Affairs requires all large sharps containers (disposable or reusable) to be on either dollies or carts and not sitting directly on the floor. BLUEPRINTS: Contractor is responsible to provide an initial blueprint for Manhattan, Brooklyn & St. Albans. This shall be updated quarterly to ensure it is always as accurate as possible. QUALITY CONTROL: Contractor must replace all containers that are more than approximately half to two-thirds full, to ensure no overfills between service days. Contractor must replace any/all containers which have not been filled and/or serviced in over 30 calendar days, regardless of its content or volume. Contractor should have service infrastructure to handle all applicable issues related to sharps waste disposal and service. Contractor should be able to complete all terms and conditions of the contract immediately following the award of the contract. There are to be no delays in delivering the service and/or products. ISOLATION ROOMS (INFECTION CONTROL): Contractor shall not enter isolation rooms for reusable sharps exchange services. Contractor will provide disposable sharps containers for the staff to take into the rooms for usage while the room is isolated. The containers will be disinfected and placed in the soiled utility room. Contractor will remove from the soiled utility room and pack for the shipping with the regulated medical waste. MANIFESTING (SHIPPING PAPERS): Contactor is responsible for completing the regulated medical waste manifest prior to leaving the medical center. No regulated medical waste is permitted to leave the medical centers until a manifest is properly completed and signed by an authorized VA employee. Completed manifests confirming delivery and processing of the regulated medical waste shall be submitted monthly for the entire month. Must be within 60 days of the end of the month. Must be separated by facility then sequenced based on the date of pick-up. This will be in the form of a PDF and submitted to the COTR. Contractor shall provide clean replacement containers as needed on or before scheduled service days. Contractor shall provide access to spare containers at each facility in the case that a container is full in between services and EMS needs and empty container. Contractor shall provide 10 keys for the sharp s brackets to Manhattan VA Medical Center and Brooklyn VA Medical Center. Contractor shall provide 5 keys for the sharp s brackets to the St. Albans VA Medical Center. Contractor shall provide sharps exchange services to all medical center areas to include medical wards, clinics, exam rooms, restrooms, crash/medication carts and various areas throughout the medical center. All locations as identified on the blueprints for each facility. The only areas that the technicians shall not exchange sharps containers will be isolation rooms and areas designated by the COTR. Sharps service can be performed on weekdays (Monday through Friday) 8 AM to 4 PM. All disposal and transportation of sharps (reusable and disposable) shall be as regulated medical waste and in accordance with all Federal, State, and local regulations. MONTHLY SHARPS REPORTING: Contractor is required to provide a monthly sharps report with collection information that includes but is not limited to outlining the quantity of containers; sharps disposal weight; total plastic diverted. Note: Pickup days and hours indicated above is tentative and subject to change to insure adequate service. However, excluding the unforeseen conditions, Contractor may be required to make unscheduled pickups as well. CONTRACTOR S RESPONSIBILITY: Contractor will only transport the waste to facilities that are permitted for the processing of regulated medical waste. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to their employees, or others, as well as for any damages to personal or public property that occurs during performance of this contract that is caused by their employees' fault or negligence. GEN-EX CARTRIDGE WASTE DISPOSAL: VAMC requires the technical manpower, containers, manifesting and the disposal of the cartridges from the diagnostic machine. 2-55 gallons drums per month. Rate includes 2-replacement drums, monthly pick-up, disposal, manifesting, energy, and insurance. Allowance of 200 pounds per drum and one hour for loading. Contractor shall provide the appropriate containers, shipping papers, pick-up, transportation, processing, and tracking. It must be incinerated SERVICE LOCATIONS: Contractor shall ensure services are performed as specified for each location. Pickups will be made from medical waste room located in the main building basement as well as the shed located in the compactor area at the rear of the main building between the hours of 7 AM to 5 PM. Once usage stats are established RBX pickups can be placed on a schedule. Furnish appropriate containers, pick-up, and disposal of infectious/regulated medical waste. Pick-ups will be scheduled at a minimum of once per week or pickups as needed per week. If quantity increases, pick-ups shall be adjusted accordingly. Pick-ups will be made from designated locations throughout the area as well as the medical waste room located in clinic between the hours of 8 AM to 4 PM. Additional sites may be added with agreement between the Government and the Contractor. 22. QUALITY ASSURANCE: The Government will investigate complaints from any facility. All information received will be provided either verbally and/or in writing to the contractor for appropriate action. 23. WORK HOURS: Normal Work Hours are site specific, see each site for specific work hours. 24.
