- Statement of Work Holmium Yag (Holap) Laser Rental Service, Surgical Service Washington DC VA Medical Center The following Statement of Work outlines contractor requirements in regard to supplying Holmium Yag (Holap) Laser, Cryoblation Rental and ultrasound Service to the DC VA Medical Center (DC VA), for Surgical Surgery. SCOPE The contractor shall work collaboratively with Surgical Surgery to provide a Holmium Yag (Holap) Laser Rental Service, the Holmium Yag (Holap) Laser Fiber is used in performing various surgical procedures, which allow providers to work at high levels of precision by focusing on a ����������� area, damaging less of the surrounding tissue. Task Deliveries: On each scheduled day (or alternative day arranged in accordance with procedures below) Contractor shall deliver the laser to Client's site not less than 30 minutes prior to the first laser procedure, for usage by Client up to the conclusion of the day or part day as agreed to. Client shall afford Contractor not less than 48 hours' telephone notice of the time of the first procedure (and preferably shall notify contactor at the conclusion of the preceding Client Day). Contractor may remove the laser at the conclusion of the last scheduled surgical procedure. Each of Client and Contractor may, in the event of an emergency, cancel a scheduled day by 24 hours' advance telephone notice to the other. As an alternative the party giving notice may request that Client and Contractor adopt an alternative time or site. Because of Contractor's itinerary and because of Client's patient commitments, the rights to cancel and to request change shall be exercised sparingly. Services Include: A minimum of two (2) ""LASER DANGER"" signs, eight (8) pairs' of wavelength specific laser safety eyewear, a trained, fully insured laser operator, installation and set up, and current preventive maintenance verification. Laser Service Breakdown: Holmium Yag (Holap), Evolve 980 Diode, and HPS - for treating BPH, Ablation of Prostate, Strictures, and Bladder Tumors. Omniguide C02 w/ adapter - used in a full range of surgical procedures by Ear Nose Throat, Otolaryngologist, I-lead & Neck Surgeons, Neurosurgeons, Spinal Surgeons, Laryngology and Airway Surgeons, Otology and Neurotology with a disposable fiber of choice (Oto-M, Oto-S, ENT-L, Neuro-L). Argon - used as an ophthalmic laser ABC (Argon Beam Coagulator) allows physicians to achieve non-contact hemostasis with minimal thermal injury of adjacent or underlying tissue. Effective on high impedance tissues, and eliminates muscle chattering. C02 Lasers-for treatments in Surgical Service. Service and Repair: Each of Contractor's lasers receive a Preventive Maintenance (PM) check to maintain compliance with our PM Schedule with manufacture recommendation of two (2) per year.
