- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. �This announcement constitutes the only solicitation. Solicitation number 12024B20Q9019 is being issued as a Request for Quote (RFQ) by the U.S. Forest Service Incident Support Branch AQM located at 3833 S Development Ave Boise ID, 83705. �The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-06 September 10, 2019. �This acquisition is a Total Small Business set aside under the NAICS code 492210 with a small business size standard of $30 million. �All offerors must ensure that they complete the required submission of FAR 52.212-3 Offeror Representations and Certifications � Commercial Items (MAR 2020) indicating size status in accordance with the applicable set aside of this acquisition. The resultant award will be a Blanket Purchase Order for the required service. �Multiple awards may be made to contractors whose quote provides the best value to the Government in accordance with the FAR 12.602 Streamlined Evaluation of Offers. �Evaluation of offers will be in accordance with the factors and procedures listed in this solicitation only. Quotes will be priced only on the identified Line Items structure. Any quote that does not provide pricing according to the outlined structure will be considered non-responsive. Please see Attachment �Solicitation RFQ 12024B20Q9019 Shelf Stable Food Boxes�. Orders for Line Item 001 are reserved for use by the WO-FAM team and the intent is to order samples of Shelf Stable Food Boxes for inspection. Line Items 100, 101, 102: These line items are for Incident and Cache ordering purposes.� The intent is to obtain quantity price discounts for increasingly larger orders. Line Item 200: Shipping will be paid for based on the requested deliver speed in the order.� Vendors will attach receipts to show actual shipping costs to backup payment requests. The intent of this Blanket Purchase Agreement (BPA) is to acquire Shelf Stable Food Boxes to supply two (2) people with meals for three (3) days.� The Government may award multiple BPA�s and intends to award a sufficient number of BPA�s to meet the anticipated incident resource needs for Shelf Stable Food Boxes.� Since the needs of the Government and availability of Contractor�s resources during an emergency cannot be determined in advance, it is mutually agreed that, upon request of the Government, the Contractor shall furnish Shelf Stable Food Boxes to the extent the Contractor is willing and able at the time of order.� Due to the sporadic occurrence of incident activity and the various feeding strategies available to the Government the placement of any orders against this BPA is not guaranteed. The proposed pricing found in the Schedule of Items shall include all labor, equipment, operating supplies, materials, State and Federal taxes, insurance coverage, overhead, and profit required to produce and delivery Shelf Stable Food Boxes. The technical specifications and content requirements for the food boxes is detailed in two attachments: Attachment 1 � Specifications for Contractor or Agency Built Shelf-Stable Food Box Attachment 2 � Nonperishable Food Box � Sample Menu, or Equivalent Required Production Timeframes: �All orders issued against this agreement will be produced and ready for shipping within 72 hours of receipt of the order unless otherwise negotiating with the ordering entity. Expiration Date and Shelf Life: �Shelf Stable Food Boxes provided under this agreement shall have a shelf life/minimum expiration date of 18 months from the date of shipping out to the ordering entity.� The expiration date for the entire Shelf Stable Food Box is based on the product contained in that Shelf Stable Food Box with the earliest expiring date. Packing: �The entire contents of the Shelf Stable Food Box shall fit into one cardboard box (carton).� The carton should be a single wall with the dimensions of 16 X 10 X 12. Contractor can pack carton in the most efficient manner possible.� To the maximum extent practical, small items (i.e. can opener, accessories) should be contained inside a resealable plastic bag.� The entire contents shall be documented on a packing list and included inside the carton.� The Shelf Stable Food Box carton shall be securely taped closed allowing easy open access as shown in Figure 1 below.� The intent of this easy open taping is to avoid users cutting open the box and accidentally cutting the contents inside. Marking: The outside of the carton shall be marked with �Food-Shelf Stable� and shall clearly notate the expiration date (MM/YYYY) of the Food Box.� Palletization: Contractor shall palletize cartons no more than five layers high for all orders under Line Items 100, 101, and 102.� Pallets shall be secure for shipping and for transporting using forklifts and other warehousing equipment. First Article Sample and Testing The Government anticipates ordering First Article Sample for Testing (FAST) prior authorizing open ordering under Line Items 100, 101, and 102.� The FAST will be used for inspection and testing to provide assurance that the products will meet the requirements and technical specifications of the Government.� Results of the FAST inspection will be provided to the contractor and the Contracting Officer if there are failures or performance issues that need to be addressed.� Failure to pass a FAST inspection may result in delay in bulk ordering and may result in cancellation of the agreement. The delivery location for FAST orders will be detailed in the individual BPA Call. FAST inspections will minimally cover the following points: Adherence to attachment �Specifications for Contractor or Agency Built Shelf-Stable Food Box� Packaging � is the packaging appropriate for long term shelf storage and does it meet the requirements of the Government Timeliness of Order Completion � was the order processed in a timely manner in accordance with the timelines outlined in the BPA Call Photo Samples The photographs provided offer samples of how the finished product may look.� Actual product will vary based on contents and the vendors specific processes.
