Description
This amendment is regarding more Q&A and notice for deadline for Q&A. During the walk through of the eight ORs, we saw that four (4) of the ORs have an exam light in each room. What should be done with these lights? Replace them. See amendment #5 for specific rooms. We also noticed that there are currently two sets of dual surgical lights in OR 8. Please confirm that you would like our proposal to include two sets of dual surgical lights for OR 8. Still need the 2 sets replaced, we want 2 sets in that room. On page 6 of the solicitation, under Paragraph A: General Information and Scope of Work, you indicate that Equipment disposition from the staging point will be dependent upon Customer selection of disposal option. Can you provide direction on whether these will be disposed by the Government or Contractor? Pricing could differ if we are required to dispose of the lights. Scope of Work: The contractor shall deinstall the existing Exam Lights and make all necessary utility disconnections, remove and transport equipment to a predetermined location within same customer facility for pickup or disposal. Customer is responsible for decontamination and release of the equipment as non-hazardous. Equipment disposition from the staging point will be dependent upon Customer selection of disposal option. If removed equipment is to become contractor property, it will be held on-site at the designated location for pickup by the contractor representative within a reasonable time, not to exceed five (5) working days. Can you clarify what infection control measures are required during the de-installation and installation of the lights? Will need isolation barriers to contain any contaminates during the replacement process. Can you please provide interstitial drawings or any details of the above ceiling structure? See amendment #5 (Blue print ceiling mount) RM# C408 (OR 8) states: 2 dual arm surgical lights separate suspension. Can you please clarify this requirement does this mean four lights (two sets of dual lights) or two lights (one dual arm split into two single lights)?  It is 2 lights from 1 mount/suspension.  So, in the room with 2 duals, that is 4 lights. Note: All the OR rooms with 1 dual arm and 1 single arm (OR #2,3,4,5) and 2 dual arm (OR#8) will have same configuration/specification as stated on SOW with the same bulb replacements and operating unit.  The deadline to submit questions regarding this RFQ is July 16, 2020 @1200 CST.
