Combined Synopsis-Solicitation for Commercial Items VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2020-07. This solicitation is set-aside for 100% Small Business Full and Open competition. This will be Brand Name only. The associated North American Industrial Classification System (NAICS) code for this procurement is 339114, with a small business size standard of 750. The FSC/PSC is 6515. The NMVAHCS located at 1501 San Pedro Drive SE, Albuquerque NM 87108 is seeking to purchase Extraoral Suction System. The Albuquerque VA Hospital requests 13 Beyes DuraMax X1 Extraoral Suction units and associated supplies for the Dental Clinic in response to COVID-19 to remove contaminants produced during dental procedures including droplets, aerosols, blood, dust and virus. Scope. Albuquerque VA requests an Beyes DuraMax X1 Extraoral Suction system that removes contaminants produced during dental procedures including droplets, aerosols, blood, dust and viruses and uses the most advanced Quattro layer filtration system to kill viruses and bacteria absorbed with over 99.9% efficiency. The system will effectively remove these droplets and aerosols from the immediate treatment site thereby reducing the potential danger from dental aerosols and keep the air in the office safe and clean for patients and staff alike. Specific Tasks. Requested Multi-purpose industrial dehumidifier high level disinfection system must have/be the following: Effectively remove droplets and aerosols from the immediate treatment site. A layered filtration system to include HEPA and germicidal UV-C. Up to 265 CFM capacity. Portability. Remote control 110v Product warranty. 5.1 Task 1 - Enterprise Management Controls. 5.1.1 Subtask 1 - Integration Management Control Planning. Provide the technical and functional activities at the required level for integration of all tasks specified within this SOW. Include productivity and management methods such as quality assurance, progress/status reporting and program reviews. The FMS Vendor File Update form needs to be submitted in all cases to the Contracting Officer as part of the offer or proposal. Prior information sent to the VA Payment Center in Austin does not preclude submittal of the FMS Vendor File Update form. Failure to include this information could preclude the offeror from consideration for award or considerably delay award. ·         Please See Attachment B Vendor FMS paperwork for applicable documents.   All quotes shall be sent to the Contract Specialist Jasmon Cornick, jasmon.cornick@va.gov Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The following are the decision factors: Award shall be made to the quoter whose quotation is the lowest priced responsive and responsible quotation on an All or None basis, meaning that the quoter must be able to supply items that meet the customer s specifications, in the total quantity requested, within the delivery time required, and retains positive past acquisition performance as outlined in this solicitation documentation to be considered for award. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Quoters shall list exception(s) and rationale for the exception(s), if any.
