This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Please advise if you carry this product or a suitable or equal . It is anticipated that a Request for Quotes will be issued in July 2020. Award of a firm fixed price contract is contemplated by August 2020. This notice is published to conduct market research to determine if there is a sufficient number of certified Service-Disabled Veteran Owned Small Businesses (SDVOSB) and/or Veteran Owned Small Businesses (VOSB), capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. Registration may be accomplished at http://www.vip.vetbiz.gov/ and www.sba.gov/, respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance (3) intentions of subcontracting program to include set-asides if any; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) If you are a distributor a statement from the manufacturer that your company is an authorized distributor; (6) a statement to inform the government if you are a manufacturer or distributor; (7) any information regarding FSS Schedule availability; (8) SAM Registration (9) any other pertinent company documentation. The response date to this Sources Sought notice is July 21, 2020 at 4:30 pm AZ time. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full & open competition, or a sole-source procurement is appropriate. The Government will not pay any costs for responses submitted. Electronic submissions are acceptable via william.cornick@va.gov, and are highly preferred. NO ADDITIONAL INFORMATION IS AVAILABLE OTHER THAN WHAT IS NOTED HEREIN. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION. This procurement is for Preventative and Repair Maintenance on Biomedical Engineering equipment. Vendor shall be an authorized equipment repair and maintenance provider such that OEM warranty integrity and services are provided and maintained. All warranty and service associated with any equipment shall be in accordance with the OEM terms and conditions. Statement of Work Biomedical Technician Staffing Contract Phoenix VA Health Care System Purpose: The Phoenix VA Health Care System (PVAHCS) is requesting a contract to provide the Biomedical Engineering department with supplemental biomedical technician services until the vacant positions are filled. Background: The PVAHCS Biomedical Engineering Department had (4) Biomedical Engineering Support Specialists leave the beginning of 2020 and have yet to backfill the positions. The PVAHCS Biomedical Engineering Department has (7) vacancies: (4) BESS, (2) Network BESS, and (1) Biomedical Engineer. The department is struggling to keep up with regular maintenance activities, COVID-19 preparation, and new projects. Therefore, an emergency staffing contract is required to maintain standard level of service from the Biomedical Engineering department. Required Services The Contractor shall provide an indefinite delivery, indefinite quantity (IDIQ) contract for the period of performance described in paragraph 11. The Contractor shall provide a minimum of (2) dedicated, locally-based, biomedical engineering technicians. The technicians shall be certified and trained in complex equipment described in paragraph 12. The Contractor shall provide up to (4) dedicated, locally-based, biomedical engineering technicians. The Contractor shall report to PVAHCS Biomedical Engineering department. The Contractor shall follow the local policies in place at PVAHCS. The Contractor shall provide preventive maintenance services. Services include checking for proper operation, detection and correction of failures either before they occur or before they develop into major defects; and includes, but is not limited to, actions about periodic technical inspections, lubrication, adjustments, calibration, interior cleaning, and replacement of worn or deteriorated parts. The Contractor shall provide corrective maintenance services. Unscheduled routine maintenance includes the inspection, testing, troubleshooting, servicing, recalibration (if incidental to repair), and all other actions necessary to return the item to a fully serviceable state. Services may also include relocation, installation, and de-installation of equipment. The Contractor shall verify that the equipment that was serviced is fully functional before closing the service ticket. The Contractor shall provide on-call support 24 hour per day, 7 days per week, 365 days per year. The Contractor shall be responsible for parts. The Contractor shall be responsible for travel. The Contractor may need to travel to the (9) community based outpatient clinics (CBOC) at least once a month. The Contractor shall not remove any hard drives off station. All hard drives shall be turned into the COR. IAW 15 USC 657s and 13 CFR 125.6 - Limitations on Subcontracting, the Contractor shall perform over 50% of the work. FAR 52.217-8, Option to Extend Services: The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days. Noncontract Charges: The contractor shall not perform any services that shall result in additional charges without prior approval from the Contracting Officer. Payment: Payment shall be made in arrears, and upon receipt of a properly prepared invoice which references the period for which payment is due, the contract number and/or assigned purchase order number. The invoice shall include the correct payment address. Failure to carry out the above procedures shall be considered as a failure to document the service visit and shall constitute a failure to perform. Security: In reference to VHA Handbook 6500.6 Appendix A, Block 6, the service included in this contract does not involve connection of IT devices to a VA network. Therefore, C&A, SAP, & the following from App C do not apply. In reference to VHA Handbook 6500.6 Appendix A, Block 7, service does not involve storing, generating, transmitting, or exchanging VA sensitive information. The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information protection, patient privacy, and information system security as delineated in this contract. During contract performance, contractor will not require remote access to VA facility, equipment, information systems, or sensitive data; to provide services, install, train, maintain or repair equipment. During contract performance, contractor will require physical access to VA facility, equipment, information systems, or sensitive data; to provide services, install, train, maintain or repair equipment. VA Course No. VA10203 is privacy training required for VA contractors requiring access to VA facilities for more than 2 consecutive days or any access to VA information systems. Training will be completed prior to being granted access to facilities, equipment, or to the data therein. Training is available on VA TMS website at: https://www.tms.va.gov. Contractor will provide evidence of training to the COR, if requested. A local Business Associate Agreement, which governs VA information sharing with contractor, will be in place between the Contractor and PVAHCS. An ISA/MOU, which documents terms and conditions for sharing data and information resources through network, wireless, remote connections, in a secure manner, does not exist between the Contractor and PVAHCS. All hard drives or other data storage devices will be removed, and if done on-site will be purged of data, prior to contractor access. The Contractor is required to obtain the Contracting Officer s approval prior to engaging in any contractual relationship in support of this contract requiring the disclosure of information, documentary material, and/or records generated under or relating to this contract. The contractor is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. Upon arrival at the facility, all contractor personnel shall check-in with a valid Driver License or Company ID to the Biomedical Engineering department. Prior to conducting services, the contractor shall be issued a temporary PIV badge, which shall be worn above the waist during the contractor s visit. The contractor shall return to the same location to sign out and turn-in issued visitors badge along with the documented service report for the service performed. Signing in/out shall be during normal business hours: Monday Friday 7:00 am to 4:00 pm local time, excluding Holidays, reference federal holiday link in paragraph 9. No photography of VA premises is allowed without written permission of the Contracting Officer. Risk Control: The Contractor shall be escorted by the Agency. The Contractor shall need passes from the on-station Police Department. Performance Monitoring: The Agency shall oversee work done by the contractor and accept the quality of work as appropriate. Guarantee The contractor shall guarantee workmanship and all parts furnished by the contractor for a period of not less than 90 days Place of Performance: Phoenix VA Health Care System 650 E. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. PVAHCS will not invalidate or make reimbursement for parking violations of the Contractor under any conditions. Smoking is prohibited inside any buildings at PVAHCS. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. Period of Performance: Base Year: 7/1/2020-1/31/2021 Month to month services after base year up to 6 months per FAR 52.217-8, Option to Extend Services.
