- DEPARTMENT OF VETERANS AFFAIRS NETWORK CONTRACTING OFFICE (NCO) 16 MICHAEL E. DEBAKEY VA MEDICAL CENTER (VAMC), HOUSTON, TEXAS REQUEST FOR INFORMATION 36C25620Q0868 BACKGROUND: The Houston VAMC has a requirement for the following brand name supplies: Fuel Delivery GENERAL: Contractor shall furnish all transportation to deliver 52,000 gal of #2 low sulfur red dye Diesel Fuel to the Michael E. DeBakey VAMC. Fuel request will be submitted by the Contracting Officer Representative and the contractor shall delivery the requested quantity of supplies ten days after receipt of order. Delivery shall be FOB destination. DEFINITONS/ACCRONYMS: CO Contracting Officer COR - Contracting Officer Representative SOW- Scope of Work DELIVERY LOCATION: Michael E. DeBakey Medical Center Building 105 Boiler Plant Att: Will Ray COR 2002 Holcombe Blvd Houston, Texas 77030 HOURS OF DELIVERY: Normal hours of delivery are Monday through Friday from 8:00 a.m. to 4:30 p.m., excluding holidays. All delivery shall be performed during normal hours unless requested or approved by COR or his designee. Federal Holidays observed by the VAMC are: New Years Day, Martin Luther King, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. WARRANTY: Contractor shall warrant the supplies for one year from the date the Government makes the final acceptance of supplies. INVOICES AND PAYMENTS: Invoices shall be submitted thru OB10 System after delivery and acceptance of supplies. Payments shall be made via EFT (electronic Fund Transfer) from the VA Financial Services Center upon receipt of a properly prepared invoice. Invoice shall be submitted electronically in accordance with VAAR Clause 852.232.72. PERIOD OF PERFORMANCE: 5 day REQUESTED RESPONSES: The intent of this Request for Information is to establish sources to define the procurement strategy (e.g set-aside, sole source, unrestricted) for a solicitation that VA intends to post in the near future. Interested contractors are requested to respond in accordance with the following: Please respond to this RFI if you can provide the exact brand name products listed in the table in the background section above. In the response please cite your business size status. If you have an existing GSA or VA, Federal Supply Schedule contract, please include the contract details in your response. Please respond to this RFI if you can provide supplies that can be determined EQUIVALENT to the products listed in the table in the background section above. Please provide details on the proposed EQUIVALENT products such as Manufacturer Name, Part Number, and Description. If you have an existing GSA or VA, Federal Supply Schedule contract, please include the contract details in your response. Please note that VA is particularly interested in determining the availability of Small Business Manufacturers.
