- This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 621511 and size standard is $32.5M. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry www.vetbiz.gov. Project requirements: This requirement is for interferon gamma release assay TB TESTING at the VAMC. The Contractor shall provide personnel, materials, facilities, supplies, and all other services and items necessary to perform special testing for interferon gamma release assay TB Testing, as well as interpretation of the testing results. The contractor shall comply with the Joint Commission Accreditation Healthcare Organization (JCAHO), and all Federal, State, and local laws applicable to the performance of work including the Food and Drug Administration (FDA). SUPPLIES: Provide all necessary supplies, not limited to the following: Requisition forms Specimen containers Dry ice and appropriate container Special instructions Current list of tests with reference ranges and specimen requirements Specimen carriers All forms, including Chain of Custody forms Special media or tubes for viral samples All supplies for special tests The Contractor will be responsible for transportation of tests via Courier Service or overnight FedEx service with tracking receipts. Tests pick-up must occur within twenty-four (24) hours of notification to the contractor. The tests will be available for pick-up during the hours of 8:00 am-4:30 pm CST, Monday through Friday, excluding holidays. The contractor shall provide a 48-72 hour turnaround time. The turnaround time begins at the time the test is received at the contractor's laboratory. The Contractor shall provide On-Line reporting of test results at no additional charge to, as well as a fax copy of the results as soon as they are available and a paper copy of the results via Federal Express with tracking for documentation purposes. Interferon gamma release assay TB tests shall be performed Monday through Friday 8 AM. to 4:30 P.M.; excluding federal holidays. The Contractor shall provide a POC client service representative that can be reached. New medical testing shall not be introduced without prior recommendation and approval of the Contracting Officer and Contracting Officer's Representative (COR). The Contractor shall provide advanced notification of test changes, additions and deletion one month in advance. The Contractor shall provide the scheduled days of testing for the different test procedures with turnaround times. The Contractor shall provide single-site (one lab) specimen testing for easy specimen tracking. The Contractor must fax or mail the tests reports to the VAMC Memphis, TN. NOTE: All records generated during the performance of the contract are considered Government records and shall be turned over to the Government upon termination of the Contract. SPECIMEN TESTING SERVICES: Send out Interferon gamma release assay TB tests, Interpretation and reports. The Contractor shall provide a report that includes the following: Identify patient, referring facility, and name of physician collecting specimen. Date and time specimen collected. Date and time specimen received at Contractor's lab. Date the test was performed and specimen type. Results of specimen testing including any interpretation and/or analysis of the test results. Name of the individual performing the test. Outline of testing protocols, reference ranges and abnormal results shall be flagged. Note of any comments related to the specimen testing as deemed important to be validity of and/or interpretation of the test results. Anticipated period of performance: 01 October 2020 through 30 September 2021 and 4 option periods. Information sought: Response to this announcement shall not exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc.,) pursuant to the applicable NAICS code; (3) Information should be provided electronically in a Microsoft Word or Adobe PDF format.
