Attachment 1: Combined Synopsis-Solicitation for Commercial Items VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-06 05/06/20, 2008-22 05/08/2020. This solicitation is set-aside for: Service-Disabled Veteran-Owned Small Business (SDVOSB) The associated North American Industrial Classification System (NAICS) code for this procurement is 339113, with a small business size standard of 750 Employees. The FSC/PSC is 6515. The VA Ann Arbor Healthcare System, Network Contracting Office (NCO) 10, 2215 Fuller Rd. Building 1, Ann Arbor, MI 48105-2303 is seeking to purchase Inspektor Digital Boroscope Systems. All interested companies shall provide quotations for the following: Supplies/Services Line Item Description Quantity Unit of Measure Unit Price Total Price INS7004 INS8007 INS8008 INS6005 INSPEKTOR CT DUO 10"" Monitor with built in light source INSPEKTOR CT Scope, 2.1 mm, 1.8 meter INSPEKTOR CT Mini Scope, 1.2 mm x 1.8 meter Wall Mount Articulating Arm Monitor Bracket 4 4 4 4 EA EA EA EA The Inspektor CT borescope and Inspektor CT mini borescope are used to look through the channel of lumened instruments, such as suctions and lumened endoscopes. This is to help identify quality issues such as damaged internal mechanisms, as well as possible contamination such as blood or tissue that was retained inside the lumen after decontamination. It is a stand-alone system with a built in monitor, therefore no computer system is required. It is always in focus and illuminated due to automatic adjustment. The borescope itself can be thermally disinfected and is compatible with Sterrad NX and Sterrad 100NX sterilization to ensure that repeated use of the borescope does not result in cross contamination when transitioning from inspection to inspection of lumened items. These borescopes are small enough to get into small channels and long enough to inspect GI endoscopes. Specifically, they are 2.1mm diameter and 1.8 meters long for the CT, and 1.2mm x 1.8m for the CT mini. The borescopes are waterproof and can be used on both the dirty and clean side of sterile processing. Delivery shall be provided [insert delivery date or ""no later than 90-days after receipt of order (ARO). FAR 52.212-3, Offerors Representations and Certifications Commercial Items a completed copy of this provision shall be submitted with the offer unless SAM registered which must be noted in the quotation. Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. The following contract clauses apply to this acquisition: FAR 52.212-4, Contract Terms and Conditions Commercial Items. The following additional clauses apply: 52.232-40, 852.203-70, 852.212-71, 852.219-10, 852.232-72, 852.246-71. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders. The following subparagraphs of FAR 52.212-5 are applicable to include clauses: 52.204-10. 52.209-6, 52.219-27, 52.219-28, 52.222-3, 52.222-19, 52.222-21; 52.222-26, 52.222-36, 52.222-50, 52.223-18, 52.225-3; 52.225-13, 52.232-34. All quoters shall submit the following: NOTE: This equipment will need to be delivered to the SPS department at the VAAAHS and requires professional installation by an authorized technician. All quotes shall be emailed only to john.mccallum3@va.gov for consideration in this acquisition. The following are the decision factors: Lowest Price Technically Acceptable offer (LPTA). The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional clauses apply: 52.204-7; 52.204-16. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" Submission of your response shall be received not later than Monday, July 13, 2020, by 4:30 p.m. EDT.
