- A. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. B. The Great Plains Area Indian Health Service (IHS) intends to award one single award Firm-Fixed Price, Non-Personal Service, Commercial Item, Purchase Order in response to Request for Quote (RFQ) RFQ-20-136. Partial awards will not be made. The Government reserves the right to not make an award, if necessary. This is a RFQ only and in no way obligates the Government to award a Purchase Order or to reimburse offeror(s) for any costs incurred in preparing their quote. C. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-07, effective July 2, 2020. D. The RFQ is issued as a 100% Small Business Set Aside and the associated NACIS code is 562211, with a small business size standard of $41.5 million. E. The quoted unit pricing must be all inclusive, to include but not be limited to shipping, travel, lodging, per diem, fringe benefits, federal, state and local taxes (contact the local Tribal TERO Office to see if taxes are applicable), plus all other costs pertinent to the performance of this purchase order. Utilize your most competitive and reasonable rates. Please complete the attached Medical Waste Removal Services Price Schedule F. Medical Waste Removal Services for the Indian Health Service (IHS), Woodrow Wilson Keeble Memorial Health Care Center, 100 Lake Traverse Drive, Sisseton, SD 57262 (See attached Statement of Work). G. Services will be provided to the Indian Health Service (IHS), Woodrow Wilson Keeble Memorial Health Care Center, 100 Lake Traverse Drive, Sisseton, SD 57262. Period of Performance: September 1, 2020 to August 31, 2021 (Base Year), with four (4), one (1) year option periods. H. FAR 52.212-1 Instructions to Offerors- Commercials Items (June 2020) � Applies to this acquisition and is incorporated by reference. Quotes shall be submitted on company letterhead stationery, signed and dated. Quotes shall include the following information to be considered technically acceptable: 1. Solicitation Number: RFQ-20-136 2. Closing Date: July 17, 2020 at 3:00 PM CST 3. Name, address, and telephone number of offeror and email address of contact person. 4. Technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary. 5. Terms of any express warranty. 6. Price and any discount terms. 7. ""Remit to"" address, if different than mailing address. 8. Acknowledgement of solicitation amendments (if any issued). 9. A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically). 10. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offerors that fail to furnish required representations or information, or reject the terms and conditions of the solicitation, may be excluded from consideration. 11. Completed Medical Waste Removal Services Price Schedule as attached to this RFQ. 12. Business Permit for Transporting Medical Waste. Basis for Award: Lowest Price Technically Acceptable (LPTA). LPTA factors in determining the successful contractor: - Meet the Requirements in the Medical Waste Removal Services Statement of Work as attached to this RFQ. - Provide a Business Permit for Transporting Medical Waste. - Meet the Delivery Schedule. I. FAR 52.212-3 - Offeror Representations and Certifications - Commercial Items (June 2020) - See attachment for full text. The offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via the System for Award Management (SAM) website accessed through https://www.sam.gov. If the offeror has not completed the annual representations and certifications electronically, the offeror shall complete only paragraphs (c) through (o) of this provision. J. FAR 52.212-4 - Contract Terms and Conditions - Commercial Items (October 2018) � Applies to this acquisition and is incorporated by reference. K. FAR 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (July 2020) � Applies to this acquisition. See attachment for full text. L. Attached are the Federal Acquisition Regulation (FAR) and the Health & Human Services Acquisition Regulation (HHSAR) provisions and clauses that are applicable. M. Quotes will be accepted electronically. Please be advised that the Government reserves the right to transmit those questions and answers of a common interest to all prospective vendors through a solicitation amendment. ACCEPTANCE PERIOD: Your quote must stipulate that it is predicated upon all the terms and conditions of this RFQ. In addition, it must contain a statement to the effect that it is firm for a period of at least 60 days from the date of receipt by the Government.
