Rev_02 Statement of Work Repair to Chiller Number Three Chiller Alexandria VA Health Care System, Pineville, LA Scope The Alexandria VA Health Care System, Pineville, LA is requiring repair to existing Chiller Number 3 located on the Alexandria VA Mediccal Center campus. Chiller Number 3 was repaired under previous contract in 2019 and ran a few weeks until wiring system and /or motor caused extension damage to SCR modules and starter boards. These boards are in need of replacement and motor examined ( and repaired if needed ) to insure motor is in acceptable working condition. Objective To insure all repair requirements are adhered to and completed by the completion date provided in contract documents The existing chiller unit make manufactured by YORK bearing the model number of YKGLP9-CTF and is a 134a Refrigerant chiller capable of 650 tons of cooling . This Chiller unit and other Chiller Units located in Chiller Plant provide necessary cold air through out the Alexandria VA Health Care complex located in Pineville, Louisiana. To ensure that all repair activities are performed in such a manner as to ensure safe and correct operation and in compliance with applicable local, state and federal regulations and codes. Tasks Furnish all labor, tools, materials, equipment and supervision necessary for the work to repair existing Chiller Number Three as stated above. Recommended skill trades for this project includes, but is not limited to: Mechanical Trades (pipe fitters). Prior to mobilization and commencement of any repair work, the contractor shall conduct a thorough survey and investigation of the complete Chiller Unit and site. Delivery All personnel in the contractors services shall have certified or proper training on the tasks they perform. The Project Engineer or COR shall be informed by the contractor of the following situations/events in writing: The anticipated start date of any contract work. As soon as it is known that the work will take longer than expected. As soon as it is known that a possible change order will have to happen. To provide a written plan for corrective action for any work that cannot be completely within contract specified time. At completion of work prior to leaving the facility. Contractor Payment Application shall be provided at the begining of each month for the previous month work that has been completed. All required contract works completed shall be approved and accepted by Project Engineer or COR prior to accepting and approving contractor application for payment. Security and General Requirements All on-site work should be completed between the hours of 8am and 4:30pm CST, Monday through Friday excluding holidays unless otherwise listed in the tasks section or other working hours are approved by the COR. Contractor shall obtain an access badge from the VA Police Department or from Facility Engineering Department while performing work on-site for their self, their employees and all subcontractors under their authority. Contractor shall contact Project Engineer or COR whenever work is being performed in any VA facility to determine if an escort is needed. Project Engineer or COR will arrange for an escort if needed. Security and privacy requirements vary depending on the equipment to be serviced and the service being performed. Contractor personnel may be required to complete any or all of the following but not limited to: Online training. Read, sign, and comply with Contractor Rules of Behavior . Submit to finger printing and background investigation. Provide network security information as requested for any equipment used in the VA facilities. The extent of these requirements shall be determined by Project Engineer or COR using current facility Information Technology and Privacy rules in cooperation with experts in those departments. Parking shall be limited to established concrete or improved rock parking lots, Project Engineer or COR will advise which parking lots to use. Contractors and personnel are not allowed on the grass area unless specifically authorized by the Project Engineer. U.S. District Court Violation Notices shall be issued for parking violations. The contractor and all employees shall comply with all applicable Alexandria VA Health Care System fire, safety, and traffic regulations. The contractor is to field verify existing conditions. All work is to be performed within all ASME, OSHA, EPA, VA, and Federal, State and local codes and regulations. Proper Lock-out/Tag-out procedures shall be used when appropriate. No roadway, drainage ditch, pedestrian walkway, or any other service, utility, or access to any of the above is to be blocked or rendered unusable for any length of time during this project unless previously approved by the Project Engineer. All work, materials, storage of materials and equipment, repair procedures and unforeseen problems or circumstances are to be coordinated with the Project Engineer. Work on this project shall not begin until authorized by the Contracting Officer. The title Project Engineer or COR shall refer to the Contracting Officer s Representative. Work to be conducted on a Fully Operational VA Hospital Campus, in and around Mental Health and Long Term Care facilities. Tools and equipment shall be kept under close watch to prevent patient injury. Place of Performance Work shall be performed, with issuance of Notice to Proceed, at the Alexandria VA Health Care System, Pineville, LA, or in the immediate vicinity as required by this Scope of Work. Equipment or Government property may be removed from the facilities with prior permission from Project Engineer or COR and the Department Director. Period of Performance All contractually required work and tests must be successfully completed within 30 to 60 calendar days of issuance of Notice to Proceed. ISO Requirements: The C&A requirements do not apply, therefore a Security Accreditation Package is not required.
