- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will NOT be issued. The solicitation number is 75H70720R00015 and is issued as a Request for Proposals (RFP), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-01. The associated North American Industrial Classification System (NAICS) code for this procurement is 238220 with a small business size standard of $16.5. This requirement is a 100% Set-Aside for Small Business Concerns and only quotes from qualified offerors will be considered. FOB Destination shall be described herein.� The Jicarilla Service Unit (JSU), in Dulce, New Mexico, Rio Arriba County, is seeking proposals from Small Business (SB) vendors to provide requires the services of authorized Ultraviolet C(UVC) disinfection in three (3) air-handler units(AHU) installers and repairs who can sell, install, existing system currently located at the Jicarilla Service Unit located at 500 North Mundo Drive, Dulce, New Mexico 87528. This solicitation is set-aside for 100% Small Business (SB). If IHS does not receive at least two offers that are reasonably priced offers from SB, IHS will then evaluate quotes and consider award to other small business concerns under the following 2-tier cascading set-aside (in order of first consideration to last consideration):� � 1st Tier - Small Disadvantage Businesses � 2nd Tier-All Other Small Business Concerns. *** SITE VISIT *** If potential offerors are interested in viewing the building prior to submitting their quote, this is schedule for July 9th, 2020 at 10:30 am with Don Smith and Thomas Dodge, telephone number (505) 759-7206, located at Jicarilla Service Unit(JSU), 500 North Mundo Drive, Dulce, New Mexico 87528 PLACE OF PERFORMANCE. Services are to be provided on site at the Jicarilla Service Unit located at 500 North Mundo Drive, Dulce, New Mexico 87528. DESCRIPTION OF REQUIREMENT.� Services will require that the Contractor be an certificated installer and seller of Steril-Aire Ultraviolet C(UVC) for the installation into three(3) air-handler units(AHU), either through their GSA Contract or Open Market. UVC Lighting in AHU's 1.�� �Perform a site visit to Jicarilla Service Unit�in Dulce, NM to determine size, location and power needs for installing Steril-Aire UVC in the three (3) AHU's located in the clinic Mechanical Room. 2.�� �Install Steril-Aire UVC lighting for three AHU's at the Jicarilla Health Clinic. 3. �Install all electrical service needed for the Steril-Aire UVC lighting. �Include any other electric service deemed to be necessary for the new Steril-Aire UVC lighting such as controllers, notification and switches. 4. �Provide and install all mounting boxes, framing, brackets, clips and all other fittings and materials necessary for installing the new Steril-Aire UVC lighting. 5. �Install Warning Signage on each AHU to prevent UVC exposure if AHU is opened. 6. �Provide Technical Manuals of product information, specifications, preventative maintenance, etc. 7. �Work to start 14 days after acceptance of bid and award, and completed within 14 days of start. The Government will periodically evaluate the Contractor's performance. The Contracting Officer's Representative will evaluate the Contractor's performance through intermittent inspections of the Contractor's work. Offerors interested in submitting proposals for this requirement MUST submit the following: �� 1. Itemized list of all new furniture and panels requested. �Pricing proposed shall be an all-inclusive rate that includes all state, federal, and local taxes (ex. NM Gross Receipts"" taxes) and any miscellaneous costs associated with the above services. Pricing proposals should be itemized to detail a total amount for installation with a charge for taxes only for installation services. Pricing in proposals MUST be good for 30 calendar days after close of solicitation. Failure to provide any required items will result in the offer being determined as non-responsive. BASIS FOR AWARD: FAR 52.212-2 Evaluation - Commercial Items (OCT 2014) (a) The Government will award one contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Technical Qualifications and (ii) Past Performance. Technical Qualification and past performance, when combined, are significantly more important than price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. PRICE EVALUATION. The price proposals shall be evaluated to determine the Offeror's ability to carry out its proposed technical approach and indicate that the Offeror understands the nature and extent of the work to be performed. The price evaluation will be point scored. PAST PERFORMANCE EVALUATION. The Offeror will be evaluated to assess the demonstrated quality of performance on similar work, to include the ability to control the quality and cost of work, timeliness of performance, and effectiveness at accomplishing the goals of previous work.
