Sources Sought - BRAND NAME OR EQUAL Stryker Transport Stretchers NAICS Code: 339112 Sources Sought Number: 36C26320Q0538 Notice Type: Sources Sought This is a Request for Information (RFI) only; it is not a request for quotations or offers. 52.215-3 -- Request for Information or Solicitation for Planning Purposes (Oct 1997) (a) The Government does not intend to award a contract on the basis of this solicitation or to otherwise pay for the information solicited except as an allowable cost under other contracts as provided in subsection 31.205-18, Bid and proposal costs, of the Federal Acquisition Regulation. (b) Although proposal and offeror are used in this Request for Information, your response will be treated as information only. It shall not be used as a proposal. (c) This solicitation is issued for the purpose of: conducting market research to identify potential sources for a potential procurement of BRAND NAME OR EQUAL Stryker Transport Stretchers as indicated below at the Lincoln VA CBOC located at 600 South 70th Street, Lincoln, NE 68510. See attached item specifications This is a request for information and sources only, which may or may not lead to a future solicitation. This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this sources sought notice. Information should be forwarded to the Contract Specialist. Any questions should be addressed to the Contract Specialist, in writing, at the email address provided. Because this is a request for information only, answers to questions will not be posted. If your organization has the capability to provide these supplies/services and is interested in this opportunity, please respond to Traci Johannes, Contract Specialist, Network 23 Contracting Office (NCO23), 4801 Veterans Drive St. Cloud, MN 56303 or via e-mail at traci.johannes@va.gov. Socioeconomic status of your company/entity. If applicable - GSA/FSS/ECAT/NASA SEWP contract number that these products or services are available under. Product information/specifications of the systems that you can provide that meet the requirements of the DRAFT Specification-Statement of Work. Proof of Authorized Distributorship for any product that is not manufactured by your company/entity. **Please note that the government will not accept any gray market items. Manufacturer Information of the product that your company/entity can provide to include: Manufacturer Name, Address, and DUNS Number. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. The deadline for this information is 4:30 PM Central Time on, Friday, June 26, 2020. Statement of Work Brand Name or Equal LNK048 Transport Stretchers Lincoln Community Based Outpatient Clinic (CBOC) Lincoln, NE PURPOSE The overall purpose is to provide and deliver Transport Stretchers at the Lincoln CBOC located at 600 South 70th Street, Lincoln, NE 68510. This is a Brand Name or Equal solicitation. SCOPE The Contractor shall deliver all items listed in section 2.4. All items must meet all salient characteristics defined in section 2.4. The Contractor shall furnish all personnel, supplies, and equipment required for delivery. SALIENT CHARACTERISTICS Stretcher, Mobile, Transport (JSN M4655) Basis of Design: Stryker 748 or Equal Qty 5 Electrical: N/A Dimensions within 1 inch: Length: 83 Width: 34 Pt Area: Length: 75.5 Width: 30 Adjustable Height Range between 21 and 37 Positioning Requirements Knee Gatch Approx Zero to 30 degrees Backrest Approx Zero to 90 degrees Trendelenberg and Reverse Trendelenberg plus or minus 18 degrees Siderails should be at least 13 high and 75 inches long Casters Approx 8 Diameter Constructed of material that will not damage flooring. Options: (1) Removable IV Pole per stretcher (1) Combination instrument/serving tray per stretcher (1) Oxygen Holder, upright per stretcher Four-sided steering and brake control (1) Knee Crank per stretcher DELIVERY Delivery Timeframe: No Later Than December 1, 2020 Acquisition of the above items are part of the initial outfitting and activation of the newly constructed Lincoln CBOC. The Contractor will be contacted by the Onsite POC to schedule a pre-delivery meeting for verification of delivery dates. The Contractor may be required to adjust the delivery date from the date specified in the contract award document. Delivery Location: Contractor shall deliver all equipment to the Lincoln CBOC, Loading Dock, 600 South 70th Street, Lincoln, NE 68510. Delivery Times: The loading dock accepts deliveries between 8am and 3pm Central Time. The loading dock is a federal owned/operated location and will not accept deliveries on federal holidays. Delivery will be coordinated through the Onsite POC. POCs for delivery are: Bill Brown, Ph: 910-750-2162 INSPECTION AND ACCEPTANCE The Contractor shall conduct a joint inspection with the Onsite POC upon completion of delivery. In the event deficiencies are identified, the Contractor shall provide date when the identified deficiencies will be addressed if not addressed on the date of delivery. The Contractor shall conduct a joint inspection with the Onsite POC and Government POC after addressing all deficiencies. All deficiencies identified during joint inspections shall be fixed by the Contractor prior to government acceptance of the item. Disputes shall be resolved by the Contracting Officer. Operation and Maintenance Manuals Binders - Quantity (2) each for items identified in section 2.4. Digital Copies - Quantity (1) each for items identified in section 2.4. Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packaged in binder(s) to Onsite POC upon completion of delivery. PROTECTION OF PROPERTY Contractor shall protect all items from damage during delivery. The Contractor shall take precaution against damage to the building(s), grounds, and furnishings. The Contractor shall repair or replace any items related to building(s) or grounds damaged accidentally or on purpose due to actions by the Contractor. WARRANTY The contractor shall provide all manufacturer(s) warranties on all parts and labor at time of delivery. The warranties shall include all travel and shipping costs associated with any warranty repair.
