Market research was conducted and determined that Biofire is the only vendor capable of providing supplies & service necessary that meet the requirements. STATEMENT OF WORK FOR BIOFIRE TORCH SYSTEM Introduction/Background VA Hudson Valley Health Care Systems provides quality health care to veterans using the abilities of all employees supported by our commitment to education and research. We are recognized locally, regionally, and nationally as a leader in quality patient care, positive customer service, medical/allied health education, and health-related research and employment opportunities. Intended use Pathology and Laboratory Medicine Service, Laboratory is requesting the Biofire Torch System in support of the COVID-19 pandemic. Biofire Torch System will be used to perform COVID-19 testing for VA-Hudson Valley Healthcare System (VA HVHCS). In addition to the detection of SARS-CoV-2, the Biofire Torch System detects 21 additional respiratory pathogens to help clinicians quickly rule in and rule out other common causes of respiratory illness in about 45 minutes. Scope of Work The Biofire Torch System will be used to perform COVID-19 testing for VA Hudson Valley HCS. Item/Quantity Description Items for Castle Point Campus Reference# Item Description Technical Description QTY HTFA-ASY-0001 Biofire Torch 6 Film Array (polymerase chain reaction -PCR/molecular diagnostic) modules that can perform moderate volume testing for respiratory pathogens including COVID-19 as well as distinguish between other causes of acute respiratory infections such as influenza and other viruses and bacteria. -Torch is comprised of 6 film array modules See IGCE for additional items 1 Installation Vendor will be responsible for the installation, calibration, validation, and training of all equipment. Place of Performance / Delivery VA Hudson Valley Health Care Systems Castle Point Campus Warehouse: 41 Castle Point Road Wappingers Falls, NY 12590 Travel/Parking The contractor will be responsible for their travel. As stated earlier, due to other construction projects and limited parking available at the site, contractor must coordinate any parking needs with the assigned COR and the Police Service. Details of any vehicles entering the site must be provided in advance. (Driver s Full Name, Driver s License Number, Vehicle Description, Vehicle License Plate Number) Government-Furnished Property Contractor will not need any government furnished equipment or property. Education and Training Requirements The Vendor shall be responsible for ensuring that their employees providing work on the required equipment are fully trained and completely competent to perform the required work. Specify what training and education is required for the clinical staff to be competent in using/operating the equipment. Any contractors that will come on site for install will not need access to VA network. Reference the attached Handbook 6500.6 Appendix A - Checklist for Information Security in the initiation phase of acquisitions. A VA Directive 6550 Pre-Procurement Assessment form is not applicable. Recalls The Vendor shall immediately notify all affected VA Medical Centers and designated POC in the event of a device recall, removal, required adjustment, or required modification that is suggested or mandated by the Vendor, distributor, manufacturer, or any regulatory or government agency. The following information shall be included in this notification: A complete item description and identification (to include lot and serial numbers where applicable) Administrative identification data including order numbers and order date Reasons for recall, removal, adjustment, or modification; and Instructions for appropriate corrective action. Defective Items/Warranty All device will come with at least 1-year of warranty.
