Description

This Sources Sought Notice is to conduct market research and is not a solicitation for proposals or quotations. The purpose of this notice is to obtain information regarding (1) the availability and capability of qualified sources, and (2) the size classification relative to the North American Industry Classification System (NAICS) code. The Navajo Area Indian Health Service (NAIHS), Office of Environmental Health and Engineering (OEHE), is seeking sources who can provide Installation and Internet services for the OEHE sub-office located in Fort Defiance, Arizona. Statement of Work (SOW) and Request for Proposal (RFP) are not available at this time. Please do not request for copies. This sources sought notice is strictly for the purpose of identifying potential sources for market research. Instructions: Your response should include the following: Submit a capability statement that demonstrates how your company can provide the requested services that are required. The total page limit including the capability statement shall be no more than 6 pages in length, single-spaced. The document shall be in Adobe Acrobat or Microsoft Word format.� DO NOT submit marketing material.� DO NOT submit resumes. Include past performance in providing the requested services to the Indian Health Service, other Government (Federal or State) agency, or for a private facility, within the past three (3) years. Include contract number, point of contact, telephone number or e-mail address. Include the address of your corporate office. Please indicate whether service is available under contract with GSA Federal Supply Schedule and if so, provide the contract number or indicate if service is �open market.� Provide BUSINESS SIZE AND SOCIO-ECONOMIC STATUS; (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantages business (c) If disadvantages, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) Include the DUNS number of your firm. State whether your firm is registered with the System for Award Management (SAM) at http://www.sam.gov and/or the VetBiz Registry at http://vip.vetbiz.gov. Responses to this Sources Sought Notice shall be submitted via email to michelle.james@ihs.gov. no later than 3:00 p.m. MDT, June 17, 2020. Please place ""Attention: Installation and Internet services"" in the subject line of your email. This notice is to assist the NAIHS/GSU in determining sources only. NOTE: Responses to this notice will be used by the Government to make appropriate acquisition decisions. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to particular acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government in response to this sources sought.
