REQUEST FOR INFORMATION RFI� W81K04-20-Q-0004 Firefighters Physicals Exams and Medical Case Review Services �The U.S. Army Health Contracting Activity (USAHCA), Health Readiness Contracting Office (HRCO), U.S. Army Medical Command (MEDCOM) on behalf of the Unites States Army Reserve Command (USARC), has developed this Request for Information (RFI) for W81K04-20-Q-0004 to solicit market research information from industry to assist the Government with the analysis of a Government requirement and provide commercial insight on performance-based solutions, best practices, possible contractual arrangements, and potential issues. THIS IS A REQUEST FOR INFORMATION ONLY. Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise reimburse your company for the information provided in response here to. �In accordance with FAR 52.215-3 Request for Information or Solicitation for Planning Purposes: ���� a. The Government does not intend to award a contract on the basis of this solicitation or to otherwise pay for the information solicited except as an allowable cost under other contracts as provided in subsection 31.205-18, Bid and proposal costs, of the Federal Acquisition Regulation. ���� b. Although �proposal� and �offeror� are used in this Request for Information, your response will be treated as information only.� It shall not be used as a proposal. ���� c. This RFI is issued for the purpose of: determining the scope of industry capabilities and interest in Firefighters Physicals Exams and Medical Case Review Services. This is not a Request for Proposal, a Request for Quotation, an Invitation for Bids, a Solicitation, or an indication that HRCO will contract for the items contained in the RFI. �This RFI is part of a Government market research effort to determine the scope of industry capabilities and interest and will be treated as information only.� In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this RFI are strictly voluntary and the Government will not pay respondents for information provided in response to this RFI. Responses to this RFI will not be returned and respondents will not be notified of the result of the review. If a solicitation is issued, it will be announced on the Federal Business Opportunities website http://www.fbo.gov� at a later date, and all interested parties must respond to that Solicitation announcement separately from any response to this announcement.� This RFI does not restrict the Government�s acquisition approach on a future Solicitation. Description on Requirement: �The Health Readiness Contracting Office has a nonpersonal service requirement for a contractor to provide physical examinations and medical case reviews for 165 firefighters and 211 Police/Guards within a month of receipt of award. The services shall provide be performed at the contractor�s facility. The examinations and medical case reviews shall be within the Continental United States and Puerto Rico. The contractor shall send the full physical examination reports to the U.S. Army Reserve Safety Directorate.� The Government intends to award a contract within 30 days.� A draft Performance Work Statement (PWS) is attached to this announcement for reference.� RFI (Capability) Response: Interested parties should provide feedback/input to this RFI by submitting the following information via email to the HRCO POCs no later than 1700hrs (5pm) on��� 10 June 2020 Central Time: 1. Brief company response statement addressing the following items: ���� a. Does your firm have clinics, healthcare facilities, for each of the locations listed in the PWS paragraph 5.1.4. Task 1 � Physical Exams to DoD Firefighters and 5.2.4 Task 2 � Physical Exams to DoD Police and Guards? ���� b. Does your firm have the capability of providing physical exams to 165 firefighters within a month after receipt of award?� Paragraph 5.1.4. Task 1 Physical Exams to DoD Firefighters provides the breakout number of exams by location.� ���� c. The DOD Firefighter�s Occupational Medical Examinations is based on National Fire Protection Association�(NFPA) 1582, Standard on Comprehensive Occupational Medical Program for Fire Department and DOD 6055.05-M, Occupational Medical Examinations and Surveillance Manual. Does your firm have the capability of providing the firefighter�s occupational exam in accordance NFPA 1582 Standards? �Listed in PWS paragraph 6.1.1. ���� d. Does your firm have the capability of providing physical exams to a total of 211 DOD Police and Guards within a month after receipt of award?� Paragraph 5.2.4 Task 2 � Physical Exams to DOD Police and Guards provides the breakout number of exams by location.� ���� e. The DoD Police and Guards Occupational Medical Examination is based on �the DoD 6055.05-M, Occupational Medical Examinations and Surveillance Manual, May 2, 2007 (Incorporating Change 3, August 31, 2018), Chapter 3, Exams for Specific Groups, Paragraph C3.4. �Police Officers and Security Guards�. �Does your firm have the capability of providing a DoD Police and Guard�s occupational medical examination in accordance with the DoD 6055.05-M, Occupational Medical Examinations and Surveillance Manual.� Listed in PWS paragraph 6.1.2. ���� f. Does your firm have past performance performing occupational medical examinations for the DOD? �If so, please provide the following information for each relevant contract: (1) name of your customer; (2) contract number; (3) contract start date; (4) contract length; (5) total dollar value for all periods; (6) place of performance; and (7) a brief description of services? ���� g. Provide any concerns, comments or recommendations you may have with the attached draft PWS no later than 1700hrs (5pm) on 10 June 2020, Central Time via email to the HRCO POCs. Give specific PWS paragraph numbers for reference. ��������h. Provide size/description, based upon Federal Acquisition Regulation (FAR) clause 52.212-3(a)-(c), �Offeror Representations and Certifications.� Please predicate your response on North American Industry Classification System (NAICS) code 621111 �Office of Physicians except Mental Health Specialist� and Product Service Code (PSC) Q403 �Evaluation and Screening.� Please limit your documentation to the following page size, font type and size, spacing and page numbering.� Responses shall be submitted using a Microsoft Word document with page size 8.5 by 11 inches. Font type shall be Times New Roman, size 12 using standard tabs and indent software features with no less than single spacing between lines.� Responses are limited to no more than eight (8) pages for the entire RFI response statement. All transmitted information marked proprietary shall be treated as such.� Therefore, business firms should identify any proprietary information in their responses. �Proprietary materials will neither be distributed, nor discussed with, any other firms.� Information submitted in response to this RFI will be used at the discretion of the Government.� Further, the information submitted will remain confidential insofar as permitted by law, including the Freedom of Information and Privacy Acts.
