Description
The Federal Bureau of Prisons (Bureau), Community Treatment Services Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health services in Phoenix, Arizona. The services provided should be within a 5 mile radius of the �Phoenix City Hall, 200 W. Washington St., Phoenix, AZ 85003 and within a half mile walk to a bus stop. Performance periods under this contract will be for a one-year base period expected to begin on September 01, 2020 with four (4) one-year option renewal periods with service ending on August 31, 2025 (if all options are renewed). The following required services may be sub-contracted to other licensed contractors. Base Year: Substance Abuse Treatment Services: 60 Inmates Substance Use Intake: (one per authorize inmate) for 60 inmates = 60 intakes. Substance Use Assessments & Report: (one per authorized inmate) for 60 inmates = 60 assessments Individual Counseling Sessions: �48 units (30 minutes = 1 unit) x 60 (per inmate) = 2880units Group Counseling Sessions: 72 units (30 minutes = 1 unit) x 60 (per inmate) = 4320 units Family Counseling: 8 units (30 minutes = 1 unit) x 5 (per inmate) = 40 units Mental Health Treatment Services: 30 Inmates� Mental Health Intake: (one per inmate) for 30 inmates = 30 Intakes Mental Health Intake Assessment & Report: (one per inmate) for 30 inmates = 30 units Psychiatric Evaluation & Report: 1 report per authorized inmate x 8 inmates = 8 reports Crisis Intervention: 1 intervention for 5 inmates = 5 units Mental Health Individual Counseling: 36 units (30 minutes =1 unit) per inmate for 30 inmates = 1080 units Mental Health Medication Monitoring: (5 visits per inmate) for 8 inmates = 40 monitoring visits Mental Health Group Counseling: 9 units (30 minutes = 1 unit) per inmate for 5 inmates = 45 units Sex Offender Treatment Services:�10 Inmates Sex Offender Intake: (one intake per inmate) = 10 intakes Sex Offender-Specific Evaluation and Report: (One Evaluation and Rpt per inmate) = 10 Evaluations Sex Offender Individual Counseling: 36 units (30 minutes = 1 unit) per inmate for 10 inmates = 360 units Sex Offender Group Counseling: 30 units (30 minutes = one unit) per inmate for 5 inmates = 150 units Sex Offender Family Counseling: 12 units per inmate for 3 inmates = 36 units Transportation: 48 trips (1 trip = 1 unit) for 48 inmates = 1920 Units Option Year One:� Substance Abuse Treatment Services: 60 Inmates Substance Use Intake: (one per authorize inmate) for 60 inmates = 60 intakes.
