- Page 4 of 4 Sources Sought Notice- National Contracts Standardization Program The purpose of this Sources Sought Notice is for information and planning purposes only and does not constitute a solicitation. Offers are not being solicited at this time. The Government does not intend to award a contract based on this request for information. The Department of Veterans Affairs (VA) National Acquisition Center is conducting market research for the procurement of Group A) non-adherent moist dressings and Group B) transparent film dressings of various types and sizes for the Pharmacy Personnel Department of Veterans Affairs (VA), Bureau of Prisons (BOP), Indian Health Service (IHS), and Federal Health Care Center (FHCC). This procurement will result in a national mandatory requirement contract for the above agencies (for outpatient pharmacy use only). Other companies with the same/similar product will be blocked out for ordering; therefore, it is our goal to award lower than FSS/Open Market prices on the resultant national contract based on purchases of a committed volume. These products will be awarded per group in the aggregate for the base year and all option years included in the solicitation. Please provide a response to the following: 1. Is your Company able to meet the following minimum requirements: __________________ a. National Product Code: Offerors shall provide a separate and distinct twelve-digit Global Trade Item Number (GTIN) or Universal Product Code number (UPC) number unique to the offeror or manufacturer for the product proposed. Does your company/the manufacturer have a unique GTIN or UPC? _____________ b. If the offeror is not the manufacturer of the offered items, the offeror shall submit a letter of commitment from the manufacturer to the offeror which will assure the offeror a source of supply sufficient to satisfy the Government's requirements for the contract period. Group B may be awarded separately. The Government may award two separate contracts for Groups A and B, or one contract will be awarded for both groups. To be considered for award, offerors must submit a price for the base year and all four option years for the groups proposed. 2. Is your Company interested in submitting a response to a solicitation? ________ 3. As this is a national standardized contract, our preference is for a base plus four option years to provide our customers continuity and consistency of product by allowing long term committed purchases of your product.
