Special Notice � Sole Source Dentist and Dental Assistant Stools Location: Four Corners Regional Health Center Jct US Hwy 160 & Navajo Rt. 35 Red Mesa, AZ 86514 The Navajo Area Indian Health Service (NAIHS), Four Corners Regional Health Center intends to make a sole source award to A-Dec, Inc. for doctor�s stools and assistant�s stools for the Dental Department. This is a one-time purchase. This award will be made in accordance to authority FAR 6.302-1 One and Only Responsible Source as stated in the attached Justification for sole source document titled Limited-Sources Justification (LSJ). This notice of intent is NOT a request for competitive offers; however, all responsible sources must submit a capability statement AND quote with required specifications listed below, on LSJ and against specifications attached within four (4) calendar days after the publication of this notice which may be considered by the requesting agency. Responses received after the response date or without the required information may not be considered.
