Description
STATEMENT OF WORK Orthodontics General Description:� The South Texas Veterans Health Care System (STVHCS) needs a company to: Make aligners to move patients teeth into more ideal orientation to decrease trauma from occlusion, improve phonetics and esthetics and for pre-prosthetic work. Maintain an open account with The South Texas Veterans Health Care System (STVHCS) and make available computer software to view, edit and approve Orthodontic tooth movements. The Company will accept patient analogue impressions and digital scans from STVHCS. If the requires special trays then the Company will provide impression trays via mail for the making of analogue impressions. The Company will follow the STVHCS doctor�s prescription and treatment plan the tooth movements and propose the tooth movements. The Company will accept and incorporate the STVHCS doctor�s changes requests and after final approval will manufacture the aligners for the case. The Company will accept Mid-Course Corrections and refinements as defined by the different case choices. The Company will ship the aligners after fabrication. The Company will provide annual training and support to the dentist and residents at STVHCS. Contract will be Indefinite Delivery Indefinite Quantity�of about�21 cases per year.� 2. Clinical Duties:� The contractor will design and fabricate aligners to move the patients teeth as prescribed by the treating STVHCS Dentist. 3.�� Administration: �Contractor will bill on a monthly basis. �All services rendered will be itemized by date of service �4.� �Security and Confidentiality: Responsible for maintaining Automated Data Processing (ADP) security. Responsible for maintaining patient information in the strictest confidence in accordance with the Privacy Act.� 5.��Work Hours: The services covered by this contract shall be furnished by the contractor as defined herein.� The contractor shall not be required, except in case of�emergency, to furnish such services on a national holiday or during non-working hours as described below. �Work hours:� Monday through Friday, 7:00 am 4:30 pm.� 6.� Delivery�may vary based on fabrication and delivery method.�� Due to the demands of the VA Medical Center care requirements, there shall be no restriction to the number of orders which may be placed by VA, nor the number of deliveries required. In the event that the requirements may be determined in advance, a longer delivery time will be scheduled accordingly. The delivery order shall specify: contract item (s) and quantity, time (s) of delivery and applicable VA Purchase Order number. Orders can be placed via the internet. 7. Delivery Location: South Texas Veterans Health Care System Date Point Dental Clinic 8410 Datapoint Drive San Antonio, Texas 78229 Deliveries will be made Monday through Friday (excluding holidays which are listed below) between the hours of 7:00am and 4:30pm, except in the case of emergency. Federal Holidays are as follows and include any other days specifically declared by the President Of the United States to be deemed a holiday. New Year�s Day Martin Luther King s Birthday Presidents Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas Deliveries will be supervised by the COR or their designee before beginning delivery operations. Contractor will contact the COR prior to delivery to arrange for delivery supervision. 8. Delivery Commitment: Time of delivery specified or mutually agreed to at the time of receipt of telephone order shall become mandatory upon the Contractor. Failure to perform in accordance with the delivery commitment may be grounds for termination of contract in accordance with the provisions for default. 9. Person Authorized to Place Orders: In addition to the Contracting Officer, the COR is authorized to place orders under this contract, as well as any AEGD Program Director employed by the VA Hospital. 10. The services specified herein may be changed by written modification. The modification will be prepared by the VA Contracting Officer.
