THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). SEE ATTACHMENT FOR DETAILS: Must be Brand Name or Equal This is a requirement for: U.S. Department of Veterans Affairs Martinsburg VA Medical Center (VAMC). DESCRIPTION: The Martinsburg VAMC requires the contractor to deliver within 30 calendar days from contract award to the VA medical facility warehouse below during normal business hours from 8:00 a.m. to 3:30 p.m. (EST), Monday through Friday: Delivery to: U.S. Department of Veterans Affairs Martinsburg VA Medical Center Warehouse/Atten: Bobbi Corbin 510 Butler Avenue Martinsburg, WV 25405 DISCLAIMER: This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. IAW FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Note: The contractor shall deliver required commodities (Please see attachments) no later than 30 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). Commodities shall not be delivered to the Government site on Federal holidays or weekends unless directed by the CO. Delivery shall be made between the hours of 8:00 am and 3:30 pm of the delivery location, during normal workday business hours. The U.S. Government is Performing Market Research to identify responsible sources who have the skills, experience, and knowledge required to successfully meet the requirements. Information received from Market Research will determine the best acquisition strategy and if responsible sources exist for competition, and/or a total Small Business Set-Aside. This is only for Market Research but may result in an invitation to an open discussion with the U.S. Government. This notice is not to be construed as a commitment by the U.S. Government. All information is to be submitted at no cost or obligation to the U.S. Government. Any information submitted by respondents to this notice is strictly voluntary. Propriety information or trade secrets should be clearly marked. Information received will not be returned. The U.S. Government reserves the right to reject, in whole or in part, any private sector input as a result of this Sources Sought notice/market survey. LINE ITEM DESCRIPTION: FRUITS AND VEGETABLES FOR 35 VETERANS FOR 12 WEEKS. The farmers will provide 8-Quarts flat worth of fresh fruits & vegetables each week. The basket will vary depending on what produce is in a season Quantity: 420 Unit: EA Total Cost: Statement of Work Fruit and Veggie for Veterans Program TITLE OF PROJECT: The Martinsburg VA Medical Center at 510 Butler Ave Martinsburg, WV 25405 intends to secure fresh fruits and vegetables over the 2020 growing season to provide for 35 Veterans that are in our Weight Management and Diabetes programs. BACKGROUND: We serve a patient population with where many are overweight/obese and have diabetes. We educate and want to ensure that these Veterans learn to consume healthier foods to assist with better managing their health. SCOPE: The overall scope of services is for the Veterans in our weight management and diabetes programs at the Martinsburg medical center to be educated and consume healthier foods to better manage health. 1. Scope of Contract 1.1 The purpose of this solicitation is to establish a contract for procurement of fresh fruits and vegetables to be utilized be Veterans at the Martinsburg VA Medical Center. Specifications are listed in the part (five) section of this Statement of Work. 1.2 The contract awarded will be established for one (1) period. Base Period: June 3, 2020 August 19, 2020 1. Description: Delivery of an 8-quart flat of fruits and vegetables will be weekly on Wednesday s beginning 6/3/20 through 8/19/20. Each produce item delivered in the Fruits and Veggies for Vets project will be harvested at the peak of its freshness, properly stored for the item type, and delivered to the Martinsburg VAMC designated pick up site. The VAMC will not be responsible for problems that may occur in transport and will not accept under ripe or over ripe items. Upon delivery, the Health Promotion Disease Prevention Program Manager or designee may refuse items that are not at the required freshness The Government representative, the Health Promotion Disease Prevention Program Manager and/or designee, will require a site visit to inquire about Good Agricultural Practices at the local farm. 4. Justification: Increasing the availability and consumption of fresh fruits and vegetables to Veterans who are overweight/obese and/or have diabetes has been found to assist with weight reduction and better glucose control. 5. Specifications An 8-quart flat of fresh fruits and vegetables will be supplied weekly for 12 weeks to encourage healthier eating to assist with chronic disease management. 6. Confidentiality and nondisclosure: PRIVACY AND SECURITY: The Veterans will pick up the fruits and vegetables from a Martinsburg VAMC staff member who has completed all privacy and HIPPA standards training.
