- This is a SOURCES SOUGHT NOTICE ONLY. This SOURCE SOUGHT NOTICE is for the purpose of Market Research. Response to this notice will be used for information and planning purposes. No proposals or quotes are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS or QUOTES and NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall NOT be construed as solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Responses to this notice is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The Network Contracting Office (NCO) 16 is seeking preferably Service Disabled Veteran Owned Small Businesses, or Small Businesses (but will accept other than small business for market research purposes), capable of providing entrance mats services for the Gulf Coast Veterans Health Care System, Biloxi, MS. See DRAFT Statement of Work for detailed description of services. The NAICS code is 812332�Industrial Launderers and the Size Standard is $41.5M. GENERAL: The contractor shall furnish all plant facilities, labor, material and equipment to provide mat services, to include pickup/delivery of items in such quantities as stated in the contract. 2. The Chief, Environmental Management Service (EMS) (or a representative) is to ensure compliance with this Statement of Work (to include inspection and acceptance of deliveries). Delivery personnel will report to the Chief, EMS (or representative) upon arrival and prior to leaving the Gulf Coast Veterans Health Care System (GCVHCS). Facilities are located at: Gulf Coast Veterans Health Care System (GCVHCS) 400 Veterans Ave. Biloxi, MS 39531 Mobile Outpatient Clinic (MOPC) 4444 Demetropolis Road Mobile, AL. 36619 3. Mat Requirements: A. All mats shall be free of all stains, tears, and holes. Mats with minor rips, tears, open seams or holes due to normal wear and tear are unacceptable and shall not be used. Only mats of the highest quality will be placed in service. Mats will be burgundy, steel gray or black in color. All mats will be manufactured using dyes of steadfast colors. Mat colors will be color fast and shall not bleed nor fade. Outside mats shall be rubber drainage mats designed with large holes to allow liquids and debris to fall through and not collect on the surface. B. Contractor furnished mats are to be returned clean, dry, and free of lint, odor, spots, stains, and wrinkles. No soapy, greasy or other objectionable odor shall be left in the cleaned articles. C. All mats supplied for this requirement must be heavy duty. All edges of each mat will be beveled. All mats must stay in place and remain flat and wrinkle/bunch free during normal use in a medical care environment, including wheelchair (motorized or manual); hospital bed/gurney traffic. D. Articles that do not meet the conditions of paragraph 3 A, B, and C, above, shall be rejected by the Government. Within 24 hours of delivery the Chief, EMS or representative will record any unacceptable items delivered to the GCVHCS, Biloxi division. The contractor shall replace all rejected articles within 24 hours of the notice of rejection. 4. LOCATION/SIZE: Mat size must be appropriate for specific location as outlined in Attachment A of this document. Mat location and size may be changed only with prior approval of the Chief, EMS. 5. PICK UP AND DELIVERY SCHEDULE: The contractor shall deliver and pick-up weekly on the day and time coordinated with the Chief, EMS. Contractor personnel will comply with all sign in/ sign out procedures required by the GCVHCS, Biloxi division. Any changes in the established delivery schedule must be coordinated with the Chief, EMS. 6. REPLACEMENT COSTS FOR LOST OR DAMAGED ITEMS: The replacement costs for items, lost or damaged by Veterans Administration personnel, patients and or visitors will be funded by the GCVHCS. The replacement fee will be itemized on the contract. Replacement cost for items lost or damaged by the contractor will solely be the responsibility of the contractor. The contractor will not issue an invoice for individual lost or damaged items at any time during the performance or at the conclusion of this contract. 7. WEEKLY RENTAL FEE: The weekly rental fee is determined by multiplying the weekly quantity of each item by the contractor s unit price for the item. Weekly rental fee applies only to the weekly quantity as stated on the Mat Listing and does not apply to the total inventory necessary to support the weekly quantity. The contractor will make a 100% exchange of all mats each week. Replacement mats will be of the exact size, color, and style indicated in attachment A . Deviations from this must have prior approval from the Chief, EMS. 8. VARIATION IN QUANTITY: Quantities of rental items may occasionally increase or decrease on a temporary basis during the term of the contract due to circumstances such as (for example): The number of mats required may increase or decrease due to inclement weather patterns. A 15% variation in quantity will be funded at time of award. The total amount will be computed by the contracting office. The contractor will not charge for an increase in quantity unless the increase in quantity is delivered and accepted by the Chief, EMS or representative. Prices for any temporary increases or decreases in quantity shall be in accordance with the contractor s proposed prices on the mat listing for each location. 9. ADDITIONS/DELETIONS: In the event that the GCVHCS, Biloxi division requirement exceeds the 15% variation in quantity described in paragraph 7 above, or the GCVHCS, Biloxi division requires additional items other than those originally shown on the MAT LISTING (provided upon award), or the GCVHCS, Biloxi division requests to delete items originally shown on the GCVHCS, Biloxi division MAT LISTING (provided upon award). The Government will issue a bilateral modification to increase or decrease the quantity, to incorporate the additional items, or to delete unwanted items. Prices for all increases, decreases, additions, or deletions shall be in accordance with contractor s proposed prices on the MAT LISTING for each location. 10. IDENTIFICATION BADGES: Within five working days of contract award, the contractor will provide the names of personnel (including subcontractors if applicable) that will be working under this contract by completing a Contractor s ID Badge Worksheet, which will be provided by the COR. The COR will forward to PIV office. Prior to commencement of work, contractor personnel shall have a photo identification (ID) badge including company name produced by PIV office (ext. 35421) at Bldg. 53, Room 2A129, Biloxi division or from the other clinics as applicable. Badges will be worn at all times while on the premises of the Gulf Coast Health Care System. Badges will be placed in a visible location, above the waist, clipped to the front of wearing apparel with photo and name clearly visible. Identification badges will be turned in to the COR when the contract has expired. The COR will then forward the badges to PIV office within five working days.
