- 640-20-2-522-0036 Page 2 of 2 Statement of Work Department of Veteran Affairs VA Palo Alto Health Care System 3801 Miranda Ave Palo Alto, CA 94304 Project: Palo Alto Division- Research and VMU Building Project location: VA Palo Alto Health Care System, Palo Alto Division 3801 Miranda Avenue, Building 710, Palo Alto, CA 94304 Transaction Number: 640-20-2-522-0036 1. Overview The new Research and VMU Building, planned for the Eastern edge of the VA Palo Alto campus, is a 101,000 square foot facility comprised of two floors of laboratories, and below-grade interstitial and vivarium (VMU) floors. 2. Objective To purchase outdoor furniture for Research and VMU Building on the VA Palo Alto Campus. Furniture layout will be provided to contract awardee. Key criteria: High-performance green materials ready to stand up to real-world use in any environment, low maintenance and remain in serviceable condition. Parts to be available for field replacement by VA maintenance shops. 3. Contract Tasks/Requirements Contractor shall be familiar with VA Palo Alto campus, (ID regulations, security requirements), loading dock locations, and hours of operations. Contractor shall follow all VA Palo Alto safety requirements, to include COVID-19 safety requirements, when on the VA Palo Alto campus. Contractor to contact the assigned Contracting Officer s Representative (COR) for security badging. Any contractor personnel, including permanent and temporary staff installers, shall be cleared ahead of installation start date through VA Palo Alto Police Department. Personnel shall wear contractor-provided identification and VA visitor s badge at all times while on VA property. Contractor shall provide Proof of Insurance to VA prior to installation. Contractor Storage Facility shall meet all building requirement including but not limited to sprinkler, secure, safe, environmental controls not to void furniture warranties. Contractor must store all products prior to installation and must present a staging plan for delivery. Contractor shall have manufacturer-certified personnel for handling and installation of various systems and freestanding furniture assembly. Contractor shall provide Project Manager/Lead Supervisor, Client Representative, Installers, and Service Technicians as needed onsite when providing service to the VA. Contractor shall be responsible for all field measurements. Contractor shall not rely on AutoCAD drawings for final measurements. Contractor shall provide all necessary materials, equipment, labor, supervision, and Management to: (A) coordinate delivery: (B) load and unload; (C) place/install in designated rooms/locations; (D) assemble items as required; (E) secure as designated; and (F) properly dispose of all associated packing/crating materials off-site of VA premises. Contractor shall verify project schedule with VA COR, prior to order submittal. Contractor shall coordinate installation date with VA COR to occur no later than 45 days after contract award (notice to proceed) and notification of delivery. Contractor shall obtain VA approval for any changes or corrections to the furniture plan. 4. Project Management and Design Services (PM) The Project Manager shall represent the Contractor with whom the Government has awarded the purchase order. Contractor shall complete a pre-installation meeting with VA COR, at a minimum of (10) business days, prior to installation. Contractor shall be available and on site to coordinate and oversee the delivery of product to project site. Prior to ordering of product, Contractor shall produce and provide shop AutoCAD drawings with specified products incorporated on drawings. Site verification of existing conditions is required to provide accurate measurements for product installation. Contractor shall have VA COR s approval on submittals, drawings and furnishings prior to placing order with the manufacturer. VA COR must approve any product substitutions. 5. Protection of Property The Contractor shall perform an inspection of the buildings and grounds with the COR prior to commencing work. Contractor shall protect all items from damage. The Contractor shall take precaution against damage to the buildings, grounds and furnishings. The Contractor shall repair or replace any items related to buildings or grounds damaged accidentally, or on purpose due to actions by the Contractor, utilizing materials of the same quality, size, grade and color, to match existing work. To ensure that the contractor shall be able to repair or replace any items, components, on the buildings or grounds damaged due to negligence and/or actions taken by the Contractor. The Contractor shall communicate with the COR to resolve all repairs beyond simple surface cleaning.� Concurrence of the COR is required before the Contractor may perform any significant repair work. In all cases, repairs shall utilize materials of the same quality, size, texture, grade and color to match adjacent existing work. 6. Delivery, Receive and Unload Contractor shall contact the COR, when complete order has been received, to schedule installation. Contractor shall confirm the scheduled installation date no less than 15 working days prior to start of installation to ensure that all parts have been received and in good condition. VA COR will confirm that space will be made available at appointed installation start date. Installation shall not be scheduled until Contractor has confirmed the receipt of full order. In the event there is a delay in delivery, Contractor shall notify the VA COR immediately. Contractor shall notify the VA COR a minimum of 24 hours advance notice of the scheduled date and time of delivery. Contractor shall ensure that the driver of the delivery vehicle has copies of the delivery order at time of delivery. Contractor shall not attempt to deliver any items before the scheduled delivery date. The Contractor shall verify and inspect all items both at time of receipt at off-site storage warehouse and upon arrival to the VA site. Any and all deficiencies (damage/overage/shortage) shall be brought to the attention of the VA COR. If the Contractor is unable to complete the assembly and placement of all unloaded items before the end of the workday, the Contractor shall be responsible for moving these items to a secure location, until the next scheduled workday. Contractor shall be responsible for moving the items from the overnight storage site to its designated position in the building. Contractor shall store damaged items in the location designated by the VA COR. Contractor shall maintain a complete file of all documents relating to each discrepancy and copies of all Discrepancy Reports shall be forwarded to the VA COR on a daily basis. Contractor shall repair or coordinate for replacement of damaged, defective, or missing items. Contractor shall uncrate all items received and perform all required assembly in accordance with the manufacturers instructions. 7. Assembly, Installation, and Removal Contractor shall uncrate all items received and perform all required assembly in accordance with the manufacturers instructions. Contractor shall place items in building(s) as identified in the contract and spaces in accordance with the spreadsheets and/or design drawings or specifications by the COR. Contractor shall inspect components to ensure that they are clean, dust free, free of defects and that installation is complete and all items are ready for use. Ground shall be swept/vacuumed upon completion. Contractor shall adhere to manufacturer s specifications and not use other manufacturer s product to interlock with product. Contractor shall place items as identified in the contract and rooms in accordance with the spreadsheets and/or design drawings or specifications approved by the VA COR. All furniture, equipment and accessories shall be level, plumb, square, and in proper alignment with adjoining furniture. Furnishings shall be attached to the building where applicable or for seismic safety. Contractor shall do a final walkthrough with VA COR and provide a punch list before releasing crew. Contractor Service Technician shall be available for any service call backs received within 30 days of installation completion. Contractor shall deliver all excess supplies, parts and pieces at the conclusion of the project to a VA Palo Alto-designated location. Coordinate with VA COR prior to delivery. Contractor shall be responsible for all safety and security guidelines within the area in which work is being performed, to include securing any contractor-owned tools or equipment, and any on-site storage being utilized. Contractor shall prevent its personnel from entering any area other than the designated work area. The Contractor shall maintain a means of egress within all designated work areas to comply with fire codes. Installation of furniture is at: VA Palo Alto, 3801 Miranda Avenue, Building 710, Palo Alto, CA 94304. 8. Quality Control Contractor shall be responsible for the removal and disposal of all trash/debris connected with uncrating and assembling and other items installed under this contract. Final acceptance from the VA will not occur until all debris connected with the installation is removed from the VA site. Recyclable products shall be disposed of in accordance with applicable statutes, at respective off-site locations. Packing materials will not be stored in the buildings for any period exceeding 24 hours. Contractor shall remove all personal trash (food wrappers, drink containers, etc.,) from the work site daily. Conduct of Contractor installation crew shall be courteous and respectful at all times. Be particularly mindful of our Veteran s and their needs. The Contractor shall ensure its personnel eat, drink, or smoke only in designated areas. No food or beverages other than water allowed in installation area. 9. Contract Hours The normal work hours will be business hours (7:30am-4:00pm) Monday through Friday. The Government has the option to modify the normal work week, days and hours, as necessary to meet the mission of the VAPAHCS. Contractor shall be flexible if a change in work hours is necessary to complete the project in order to accomplish the mission of VAPAHCS.
