- This a Firm Fixed Price, Request for Quote (RFQ) �for one (1) Non-personal Service Contract for Cardiologist Physician on ""As Needed Basis"" to provide echocardiogram read services to be performed via Life Image software. Services shall be performed in accordance with the attached Performance Work Statement (PWS). Period of Performance: Date of Award through December 31, 2020. The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from contract assignment. The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours are only estimated. Tour of Duty: Based upon the amount of echocardiograms APPLICANT DUTY REQUIREMENTS: See attached Performance Work Statement. -eQIP: Required Background Investigation Information Executive Orders 1045, 12968, and HSPD-12 require a background investigation as a condition of employment. This investigation will be processed utilizing the US Office of Personnel Management (OPM), Electronic Questionnaire for Investigation Processing (Eqip) system. Please provide the information required so that you can be initiated into eQIP. After you are initiated you will receive an email that will include the eQIP website link, instructions and additional forms required for the investigat ion. The eQIP form shall be completed prior to beginning of assignment. Reference attached form and complete Employee Data section only. Background checks: As directed by the Contracting Officer, the Contractor shall provide all requested information necessary to perform Level I and Level II background checks. The Contractor shall comply with the requirement to obtain security investigations. The Contractor shall work with the IHS to ensure that the pre-employment screening process includes the appropriate investigation questionnaires and forms to be completed. All completed forms will be reviewed by the Contractor and forwarded to the Government Personnel. The Contractor will be immediately removed from the position if at any time the investigation receives unfavorable adjudication, or if unfavorable information that would affect the investigation becomes known. ****FINGERPRI NT CARD - FBI FD-258 Contract assignment(s) are conditional on a cleared background check QUALIFICATION & EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Evaluation Factors will be considered: Past Performance, Meets Qualifications and Availability. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. As part of the evaluation process, candidates may be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. Your offer shall include the following documentations: 1. Quotation in the line items above. The Government will consider competitive rates upon negotiation. The rate shall be an all-inclusive rate of travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). 2. Complete Candidate Profile: Curriculum vitae, resumes, licensures, and all relative documents for proposed candidates. Experience - Doctor of medicine or Doctor of Osteopathy degree, must be either Board Eligible as per ABIM regulations or ABIM certified. License - Must have a permanent, full and unrestricted license to practice medicine in a State, District of Columbia, the Commonwealth of Puerto Rico or a territory of the United States. Must maintain medical staff clinical privileges, including any licensure requirements throughout the term of the contract. Required to have a Federal DEA number. Certifications - Must �possess a current Basic �Life �Support (BLS) and Advanced Cardiac-Life Support (ACLS). Language Requirement and Cultural Awareness - Shall read, understand, effectively communicate to patients by speaking and writing English, and other health care workers, and shall be respectful of the local, American Indian and Alaska Native culture. Information Technology Skills - Shall possess basic knowledge, skills and abilities to use a computer. Provide evidence of Past Performance and Experience for candidate of comparable scope and complexity in providing services within the past three (3) years. Provide Three (3) current references with active telephone numbers for candidate. The references MUST be noted in a separate Word or PDF document and submitted as an attachment. e-QIP: Required Background Investigation Information (MUST BE LEGIBLE & COMPLETED. DO NOT RE� FORMAT) PLEASE NOTE: UPON COMPLETION OF eQIP, THE SIGNATURE PAGES MUST BE SUBMITIED TO HUMAN RESOURCE DEPARTMENT ON THE FIRST DAY OF ASSIGNMENT INCLUDING TWO FORMS OF IDENTIFICATION. Health Requirements/Conditions of Employment: (Medical Evaluation I Immunization) Medical Evaluation: The contractor shall possess a health status allowing physical and mental ability to carry out required functions. This is to be verified on an annual basis by the employing agency or a copy of a physical examination provided by the contractor/practitioner. OTHER: -Retain US Citizenship -Contractor shall furnish health status allowing physical and mental ability to carry out required functions. To be verified on an annual basis by the employing agency or a copy of a physical examination. -The contractor must be credentialed and clinical privileges be granted by the Northern Navajo Medical Center clinical director or designee prior to providing services. -The contractor must retain certification and licensure status throughout the term of the contract. -Contractor shall furnish a current immunization for MMR (Measles, Mumps, and Rubella) Diphtheria and Tetanus and DT Booster, PPD status, or as required by NA-IHS immunization policy. -Contractor shall provide documents to complete security procedures to include finger print cards prior to the start date. -Contractor is subject to character investigation as required by Public Law 101-630 - The Indian Health Child Protection and Family Violence Prevention Act prior to performance of a contract. ADDITIONALINFORMATION: -Indian Health Service or Veterans Administration experience preferred. -Candidate availability of shifts critical. -Adequate/satisfactory computer skills required to enter patient data into an electronic records system. Immunization: Contractor shall provide evidence of current immunization documentation for MMR (measles, Mumps, and Rubella) Diphtheria and Tetanus and DT Booster, OOD status,or as required by the Navajo Area IHS immunization policy. A copy of the document shall be submitted to the Employee Nurse upon arrival at the hospital before providing service. NOTE: Incomplete Submission(s) shall not be reviewed nor considered if submitted directly to the hospital unit managers. The Contractor shall account for all forms of Government-provided identification issued to the Contractor employees in connection with performance under this contract. FAR 52.237-7 Indemnification and Medical Liability Insurance As prescribed in 37.403, insert the following clause: INDEMNIFICATION AND MEDICAL LIABILITY INSURANCE (JAN 1997) (a) It is expressly agreed and understood that this is a nonpersonal services contract, as defined in Federal Acquisition Regulation (FAR) 37.101,under which the professional services rendered by the Contractor are rendered in its capacity as an independent contractor .The Government may evaluate the quality of professional and administrative services provided, but retains no control over professional aspects of the services rendered, including by example, the Contractor's professional medical judgment, diagnosis, or specific medical treatments .The Contractor shall be solely liable for and expressly agrees to indemnify the Government with respect to any liability producing acts or omissions by it or by its employees or agents. The Contractor shall maintain during the term of this contract liability insurance issued by a responsible insurance carrier of not less than the following amount(s) per specia lty per occurrence :* $100,000.00������������������������������������������������������������ _ {b) An apparently successful offeror, upon request by the Contracting Officer, shall furnish prior to contract award evidence of its insurability concerning the medical liability insurance required by paragraph (a) of this clause. Liability insurance may be on either an occurrences basis or on a claims-made basis. If the policy is on a claims-made basis, an extended reporting endorsement (tail) for a period of not less than 3 years after the end of the contract term must also be provided. Evidence of insurance documenting the required coverage for each health care provider who will perform under this contract shall be provided to the Contracting Officer prior to the commencement of services under this contract. If the insurance is on a claims-made basis and evidence of an extended reporting endorsement is not provided prior to the commencement of services, evidence of such endorsement shall be provided to the Contracting Officer prior to the expirat ion of this contract . Final payment under this contract shall be withheld until evidence of the extended reporting endorsement is provided to the Contracting Officer. The policies evidencing required insurance shall also contain an endorsement to the effect that any cancellation or material change adversely affecting the Government's interest shall not be effective until 30 days after the insurer or the Contractor gives written notice to the Contracting Officer. If,during the performance period of the contract the Contractor changes insurance providers, the Contractor must provide evidence that the Government will be indemnified to the limits specified in paragraph (a) of this clause, for the entire period of the contract, either under the new policy,or a combination of old and new policies. The Contractor shall insert the substance of this clause, including this paragraph (f), in all subcontracts under this contract for health care services and shall require such subcontractors to provide evidence of and maintain insurance in accordance with paragraph (a) of this clause. At least 5 days before the commencement of work by any subcontractor, the Contractor shall furnish to the Contracting Officer evidence of such insurance.
