- STATEMENT OF WORK DESCRIPTION OF SERVICES: Maintenance of Service Agreement, including annual validation schedule, for the Rees Scientific Centron Environmental Monitoring System VAL/SERV B System currently in place in the Blood Donor Processing Division at the Capt. James A Lovell Federal Health Care Center in North Chicago, IL. Contractor s pricing shall be all inclusive, including parts, labor, travel, and all other costs associated with completing the work. SPECIFIC TASKS & DELIVERABLES. Contractor shall provide maintenance of 29 temperature probes to include the following: Contractor shall provide annual preventive maintenance visit including validation and comprehensive IQ/OQ test. Contractor shall provide full documentation to satisfy all applicable regulatory authorities. Comprehensive system test that verifies, calibrates, and documents all system functions including readings, databases, alarms, dial-outs, outputs, reports, and user programming. Government representative shall review and approve all validation documentation prior to departure of contractor s technician. New software updates (as available). Vendor will apply and all software updated within 30 days of release. Good Manufacturing Practices (GMP) validation report and software validation test report. Calibration of temperature humidity with National Institute of Standards Technology (NIST) traceable equipment. On-site refresher training during Preventative Maintenance (PM) visit. Contractor shall maintain a telephone answering system that is operational 24 hours a day, 7 days a week. Messages left for the contractor during off hours shall be responded to on the next business day to provide off-site assistance. Live telephone support shall be available Monday though Friday, 8:30am to 5:00pm CST. A qualified technician shall be available via telephone within two hours of any call to provide off-site assistance. Service calls when needed. All parts and labor to restore system to normal operation upon system failure. Parts available overnight from factory. LIST OF EQUIPMENT: Centron Presidio Serial Number 03484 PLACE OF PERFORMANCE: James A. Lovell Federal Health Care Center Building 133CA 3001 Green Bay Road North Chicago, IL 60064-3048 PERIOD OF PERFORMANCE: Base Year: 10/1/2020 9/30/2021 Option Year One: 10/1/2021 9/30/2022 Option Year Two: 10/1/2022 9/30/2023 Option Year Three: 10/1/2023 9/30/2024 Option Year Four: 10/1/2024 9/30/2025 HOURS OF OPERATION Normal hours of coverage are Monday through Friday from 7:00 am to 3:30 pm CST, excluding holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by COR or designee. Holidays include: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day And any other day specifically designated as a national holiday by the President of the United States CONTRACTOR RESPONSIBILITIES In the event that on-site repairs are requested, contractor shall respond no later than 2:00pm CST on the third business day after the request. Contractor shall warrant that all completed repairs have been completed per OEM guidelines and accepted industry standards. Contractor to maintain all equipment per the Original Equipment Manufactures (OEM) guidelines. All replacement parts must be new. The use of rebuilt parts will only be allowed if certified by the OEM and new parts are not available. Contractor shall be certified by Rees Scientific to perform all work on all Rees Scientific equipment. The contractor shall obtain all necessary licenses and/or permits required to perform this work. They shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. They shall be responsible for any injury to contractor employees, as well as any damage to government property that occurs during performance of the contract. SERVICE MANUALS/TOOLS/EQUIPMENT: The VA shall not provide tools, (test) equipment , service manuals or service diagnostic software to the contractor. The contractor shall obtain, have on file, and make available to its Field Service Engineers (PSE) all operational and technical documentation (such as: operational and service manuals, schematics, and parts list), which are necessary to meet the performance requirements of this contract. The location and listing of the service data manuals, by name and/or the manuals themselves shall be provided to the Point of Contact upon request. REPORTING REQUIREMENTS: The contactor shall be required to report to the Contracting Officer Representative at the Blood Donor Processing Division. This check is mandatory. When the service is completed, the FSE shall document all services as required by the POC. The FSE shall be required to log out with same. COMPETENCY OF PERSONNEL SERVICING EQUIPMENT Each respondent must have an established business, with an office and full time staff. The staff includes a fully qualified FSE and a fully qualified FSE who will serve as a backup. Fully Qualified is based upon training and on experience in the field. For training, the FSE(s) has successfully completed a formalized training program, for the equipment identified in the schedule. For field experience, the FSE(s) has a minimum of two years of experience (except for equipment newly on the market) with respect to scheduled and unscheduled preventive and remedial maintenance, or emergency generators. The FSE(s) shall be authorized by the contactor to perform the maintenance services. All work shall be performed by Fully Qualified competent FSE(s). The contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSE(s) for each make and model the contractor services at the VA. The CO may authenticate the training requirements, request training certificates or credentials from the contactor at any time for any personnel who are servicing or installing any VA equipment. The CO and/or POC specifically reserve the right to reject any of the contactor s personnel and refuse them permission to work on the VA equipment. If subcontractor(s) are used, the y must be approved by the CO; the contractor shall submit any proposed change in the subcontractor(s) to the CO for approval/disapproval. CONFORMANCE STANDARDS: The contractor shall perform the testing Food and Drug Administration (FDA), American Association of Blood Banks (AABB), and Code of Federal Regulations title 21 standards, as well as all other applicable local, state, JCAHO, and federal regulations. TEST EQUIMENT: Prior to commencement of work on this contract, the contractor shall provide the VA with a copy of the current calibration certification of all test equipment, which is to be used by the contractor on VA s equipment. This certification shall also be provided on a periodic basis when requested by the VA. Test equipment calibration shall be traceable to a national standard. IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS: The contractor s FSE(s) shall wear visible identification at all times while on the premises of the VA. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. The VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions. Smoking is prohibited inside any building at the VA. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, of municipal court. ORIENTATION FOR CONTRACTOR EMPLOYEES Contractor will attend an orientation meeting as arranged by the Point of Contact. The VA will schedule this meeting and it will include discussions of the following topics: (VA will provide information to the contractor regarding these topics and will document the meeting) Fire and Safety Infection Control Disaster procedures Other as deemed necessary The contractor will be responsible to ensure that contractor employees coming to the work site will receive the information required above. The contractor shall be responsible to ensure contractor employees providing work on this contract are fully trained and completely competent to perform the required work. ESR Vendor Engineering Service Report. A documentation of the services rendered for each incidence of work performance under the terms and conditions of the contract. Acceptance Signature VA employee who is authorized to sign-off on the ESR which indicates that the PM has been concluded or is still pending completion, or that the Emergency Repair has been accomplished or is still in a pending status.
