1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. 1.A. Project Title: Sepulveda Electrical System Maintenance 1.B. Description: VA Greater Los Angeles Healthcare System requires a contractor to conduct a 36-month preventive maintenance service of transformers at the Sepulveda Ambulatory Care Center. 1.C. Project Location: 16111 Plummer St, North Hills, CA 91343-2036 1.D. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26220Q0759. 1.E. Set Aside: This solicitation is 100% set aside for Service-Disabled Veteran Owned Small Businesses IAW Public Law 109-461, 38 USC Section 8127 1.F. Applicable NAICS code: 238210, Electrical Contractors and Other Wiring Installation Contractors 1.G. Small Business Size Standard: $16.5 Million 1.H. Type of Contract: Firm Fixed Price 1.I. Period of Performance: 90 days. All work to be done outside of normal duty hours. 1.J. Wage Determination: SCA Wage Determination No. WD 2015-5614, revision 15 1K. Key solicitation milestones are: 1.K.1. A site survey will be conducted at 1000 PST Wednesday, May 13, 2020. Participants will meet in conference room B125 in building 1. FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items 2. Statement of Work for Sepulveda Ambulatory Care Center Electrical System Preventative Maintenance 2.A. Background: The Joint Commission on Accreditation of Healthcare Organization (JCAHO) requires preventive maintenance checks and service of transformers every 3 years for failures which would interrupt patient care. 2.B. Scope: Contractor shall provide all personnel, equipment, tools, materials, technical expertise and supervision to conduct a 36-month preventive maintenance service of transformers at the Sepulveda Ambulatory Care Center 2.B.1. Specific Tasks: Contractor will inspect and preform preventative maintenance on all transformers and switches on the enclosed list in accordance with the following procedures: 2.B.1.A. The contractor shall completely shut down equipment prior to testing and preventative maintenance. Any shut-down that will affect building operations shall be done on off hours and coordinated with the VA facility manager. 2.B.1.A.1. The contractor shall visually and mechanically inspect and electrically test 53 listed transformers throughout facility. 2.B.1.A.2. The contractor shall visually and mechanically inspect and electrically test 13 listed main circuit breakers throughout the facility. 2.B.1.A.3. The contractor shall visually and mechanically inspect and electrically test 32 listed medium voltage disconnects throughout the facility. 2.B.1.B. The contractor shall perform the following visual and mechanical inspections on liquid filled transformers Inspect physical and mechanical condition. Inspect anchorage, alignment, and grounding. Verify the presence of PCB labeling, if applicable. Prior to cleaning the unit, perform as-found tests, if required. Clean bushings and control cabinets. Verify that cooling fans and/or pumps operate correctly. Inspect bolted electrical connections for high resistance using by verifying tightness of accessible bolted electrical connections by calibrated torque-wrench method in accordance with manufacturer s published data. Verify correct liquid level in tanks and bushings. Verify that positive pressure is maintained on gas-blanketed transformers. Perform inspections and mechanical tests as recommended by the manufacturer. Verify the presence of transformer surge arresters. Perform as-left tests. Verify de-energized tap-changer position is left as specified. 2.B.1.B.1. The contractor shall perform the following electrical tests on liquid filled transformers Perform insulation-resistance tests, winding-to-winding and each winding-to-ground. Apply voltage in accordance with manufacturer s published data. Perform turns-ratio tests at the designated tap position. 2.B.1.C. The contractor shall perform the following visual and mechanical inspections on the dry type, air cooled, low voltage, small transformers Compare equipment nameplate data with drawings and specifications. Inspect physical and mechanical condition. Inspect anchorage, alignment, and grounding. Verify that resilient mounts are free and that any shipping brackets have been removed. Verify the unit is clean. Use of a low-resistance ohmmeter Verify tightness of accessible bolted electrical connections by calibrated torque-wrench method 2.B.1.C.1. The contractor shall perform the following electrical tests on dry type, air cooled, low voltage, small transformers Perform insulation-resistance tests winding-to-winding and each winding-to-ground. Perform turns-ratio tests at all tap positions Remove a sample of insulating liquid in accordance with ASTM D 923. The sample shall be tested for the following: Dielectric breakdown voltage: ASTM D 877 and/or ASTM D 1816 Acid neutralization number: ANSI/ASTM D 974 Interfacial tension: ANSI/ASTM D 971 or ANSI/ASTM D 2285 Color: ANSI/ASTM D 1500 Visual Condition: ASTM D 1524 2.B.1.D. The contractor shall perform the following visual and mechanical inspections on Circuit Breakers, Air, Low-Voltage Power Inspect physical and mechanical condition. Inspect anchorage, alignment, and grounding. Verify that all maintenance devices are available for servicing and operating the breaker. Prior to cleaning the unit, perform as-found tests, if required. Clean the unit. Inspect arc chutes. Inspect moving and stationary contacts for condition, wear, and alignment. Verify that primary and secondary contact wipe and other dimensions vital to satisfactory operation of the breaker is correct. Perform all mechanical operator and contact alignment tests on both the breaker and its operating mechanism in accordance with manufacturer s published data. Verify cell fit and element alignment. Verify racking mechanism operation. Use appropriate lubrication on moving current-carrying parts and on moving and sliding surfaces. Perform as-left tests. Perform adjustments for final protective device settings in accordance with coordination study provided by end user. Record as-found and as-left operation counter readings, if applicable. 2.B.1.D.1. The contractor shall perform the following electrical tests on Circuit Breakers, Air, Low-Voltage Power Perform insulation-resistance tests for one minute on each pole, phase-to-phase and phase-to ground with the circuit breaker closed, and across each open pole. Apply voltage in accordance with manufacturer s published data. In the absence of manufacturer s published data, use Table 100.1. Perform a contact/pole-resistance test. Determine long-time pickup and delay by primary current injection. Determine short-time pickup and delay by primary current injection. Determine ground-fault pickup and delay by primary current injection. Determine instantaneous pickup current by primary current injection. Perform minimum pickup voltage test on shunt trip and close coils in accordance with Table 100.20. Verify correct operation of auxiliary features such as trip and pickup indicators, zone interlocking, electrical close and trip operation, trip-free, anti-pump function, and trip unit battery condition. Reset all trip logs and indicators. Verify operation of charging mechanism. 2.B.1.E. The contractor shall perform the following visual and mechanical inspections on Switches, Air, Medium-Voltage, Metal-Enclosed Inspect physical and mechanical condition. Inspect anchorage, alignment, grounding, and required clearances. Prior to cleaning the unit, perform as-found tests, if required. Clean the unit. Verify correct blade alignment, blade penetration, travel stops, arc interrupter operation, and mechanical operation. Verify that fuse sizes and types are in accordance with drawings, short-circuit studies, and coordination study. Verify that expulsion-limiting devices are in place on all fuses having expulsion-type elements. Verify that each fuse holder has adequate mechanical support and contact integrity. Verify operation and sequencing of interlocking systems. Verify that phase-barrier mounting is intact. Verify correct operation of all indicating and control devices. Use appropriate lubrication on moving current-carrying parts and on moving and sliding surfaces. Perform as-left tests. 2.B.1.E.1. The contractor shall perform the following electrical tests on Switches, Air, Medium-Voltage, Metal-Enclosed Measure contact resistance across each switchblade assembly and fuse holder. Perform insulation-resistance tests for one minute on each pole, phase-to-phase and phase-to ground with switch closed and across each open pole. Apply voltage in accordance with manufacturer s published data. In the absence of manufacturer s published data, use Table 100.1. Measure fuse resistance. Verify heater operation. 2.B.2. The contractor shall exercise extreme caution during testing and shall be responsible for any utilities damaged during this project. 2.B.3. All inspection and test procedures shall adhere to the Standard for Maintenance Testing Specifications for Electrical Power Distribution Equipment and Systems set forth by the International Electrical Testing Association (NETA) and approved by the American National Standards Institute (ANSI). 2.B.4. The contractor shall use the required PPE cited by OSHA regulations for the specific equipment being tested. 2.B.5. Extreme care shall be taken when restoring electrical power after testing is complete. 2.B.6. The contractor shall maintain and follow all safety regulations and traffic control. 2.B.7. The contractor shall have a job foreman present at all times during this repair/ replacement. 2.B.8. A complete report of all equipment shall be submitted upon completion of testing. The report will specify findings, actions taken and recommendations for any follow on actions. 2.B.9. The contractor shall remove all debris off the facility and clean area prior to completion. 2.B.10. The contractor shall have and be able to demonstrate a minimum of 5 years experience in testing medium voltage equipment. 2.C. General Requirements: 2.C.1. Changes. The awarded Contractor is advised that only the Contracting Officer, Peter Kim, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. 2.C.2. Parking. It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor s personnel under any circumstances. 2.C.3. Identification Badges: ll Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. 2.C.4. Smoking is strictly prohibited on the grounds of any VHA facility. Per VHA Directive 1085 dated March 5, 2019, it is VHA policy that all VHA health care facilities, including hospitals, community clinics, administrative offices and Vet Centers, will be smoke-free for patients, visitors, contractors, volunteers, and vendors effective October 1, 2019. There are no designated smoking areas. 2.C.5. All work shall be done outside of normal duty hours which are 8am-4pm Monday through Friday. 2.C.5.A. The Contractor is not required to provide service on the following U.S. Government holidays nor shall the Contractor be paid for these days: 2.C.5.B. National Holidays: New Year s Day January 01 Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Independence Day July 04 Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11 Thanksgiving Day Fourth Thursday in November Christmas Day December 25 2.C.5.C. If the holiday falls on a Sunday, the following Monday will be observed as a National holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National holiday by U.S. Government Agencies, and any day specifically declared by the President of the United States of America. 2.C.6. Safety 2.C.6.A. All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering on VA property shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. 2.C.6.B. Contractor shall perform work in strict accordance with existing relevant, accredited standards and codes to minimize the possibility of injury to personnel, or damage to Government equipment. Every effort shall be made to safeguard human life and property. 2.C.6.C. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, NFPA 70E, Article 130.7 29 CFR 1910.132(d)(1)(iii), 29CFR1910.137 2.C.6.D. Contractor shall provide all necessary tools, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. 2.C.6.E. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employee s fault or negligence. Contractor shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government. 2.C.7. Insurance Coverage. 2.C.7.A. The Contractor agrees to procure and maintain, while the contract is in effect, Workers Compensation and Employee s Public Liability Insurance in accordance with Federal and State of California and/or Nevada laws. The Contractor shall be responsible for all damage to property, which may be done by him, or any employee engaged in the performance of this contract. 2.C.7.B. The Government shall be held harmless against any or all loss, cost, damage, claim expense or liability whatsoever, because of accident or injury to persons or property of others occurring in the performance of this contract. 2.C.7.C. Before commencing work under this contract, the Contracting Officer shall require the Contractor to furnish certification from his/her insurance company indicating that the coverage specified by FAR 52.228-5 and per FAR Subpart 28.307-2 has been obtained and that it may not be changed or canceled without guaranteed thirty (30) day notice to the Contracting Officer. 2.C.7.D. Contractor is required to provide copies of proof of Workers Compensation and Employee Public Liability Insurance within fifteen (15) calendar days after notification of contract award. 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-05, dated 03/30/2020.
