- Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71320Q00023 has been issued as a Request For Quotation for a firm fixed price commercial items services contract for brand-name supply contract for laboratory supplies, located at Salem, OR at one lump sum price for items on the attached spreadsheet. The performance period includes a one-year Base and 4 one-year options. For supplies, the required delivery date is required as needed�. The delivery location and place of acceptance is Western Oregon Service Unit, Chemawa Indian Health, 3750 Chemawa Road NE, Salem Oregon, 97305, F.O.B. Destination. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years. This requirement is set-aside for SMALL BUSINESS. ATTACHMENT 2 � Statement of Work QUOTATION INSTRUCTIONS: The quoter shall remit a completed price/cost schedule (Attachment 1). The quoter shall enter the unit price and extended totals for each item listed on attachment 1. The price shall include necessary supervision, management, labor, transportation, equipment, materials, any other direct incidentals costs, overheard and profit, unless the cost is identified as separate priced contract line item. Award will be made to the responsible respondent pursuant to FAR subpart 9.1. A firm fixed price Single purchase order award will be made.
