- Amendment 3 changes the following items. A fully conformed copy of the solicitation is issued to incorporate these changes. This amendment also addresses questions asked. 1. Section Price Schedule Base Period line item numbers 0002a, 0002b, and 0002c and Option Years 1 through 4 corresponding option period line item numbers, Correct the quantities from 1 to 2 . 2. Section Price Schedule Base Period line item 0003 is added with its description System Upgrade to Bring System to Latest Version, and with its quantity and unit of measure 1 Each . 3. Section Statement of Work, para. (b) Scope of Work insert Initial upgrade of system at the St. Louis VA (both John Cochran Division and Jefferson Barracks Division) to bring the VA-owned RightFax system to the latest version. After Scope of Work: . 4. Section II Table 1 0003 is added. 5. Current RFQ Response Date, as extended as changed in amendment 2, is further extended to May 6, 2020, 2:30 P.M. Central Time. 6. Questions and Responses: Q1: What is method of payment, how should services be invoiced? Response: 1) Invoices and Payments will be in accordance with FAR 52.212-4 (g), Invoice, and are addressed in FAR 52.232-33 and VAAR 852.232-72 per the solicitation. 2) Invoices shall be submitted monthly in arrears. 3) Payments will be in accordance with FAR 52.212-4 (i), Payment. Q2: Will there be a line item for the initial upgrade to bring the system to the latest version? Response: This amendment addresses adding a line item for initial upgrade for the RightFax at the 2 locations. Q3: Is there currently a contracted provider for these services? Response: There is no current contracted services for this requirement for these 2 RightFax SUID numbers/systems. Q4: Are there any requirements regarding using subcontractors? Response: This acquisition is set-aside for Service-Disabled Veteran Owned Small Business. FAR 52.219-27, VAAR 852.219-10 and VAAR 852.219-74 apply.
