- REQUEST FOR INFORMATION (RFI) Face Shields�� 1.0 Introduction 1.1 General. The purpose of this request is to obtain information from industry to support the government�s intent to establish a Qualified Supplier List (QSL) for the standardization of Face Shields. This RFI is an invitation to submit to the Defense Health Agency (DHA) Medical Logistics (MEDLOG) Division Medical Materiel Enterprise Standardization Office (MMESO), technical information and all related supporting documentation for Face Shields. This request is a market research effort undertaken by the government which will be used for information gathering purposes. The information received may be used for acquisition planning purposes and may help define future requirements for the subject items. This is not a request for quote or proposal. 1.2 RFI Submission Package. Vendors are requested to provide responses to this RFI via the RFI Vendor Response Worksheet. Complete answers and provide supporting documentation with response. Responses for each salient characteristic should be in the form of yes, no, or descriptive explanation. Additional comments may be provided for all responses. �2.0 Scope This Face Shields product line could include but is not limited to: face shields designed as Personal Protective Equipment (PPE) for eye and face protection within the healthcare setting, for single use, and available in multiple shield sizes. These products are not made with natural rubber latex. 3.0 Characteristics/Specifications Feedback Requested: Vendors are requested to provide information about their Face Shields (See Vendor Response Worksheet) 3.1 Provide the Country of Origin for your Face Shield products. 3.2 How do you indicate if Face Shield products are not made with natural rubber latex? In the form of a statement or the ISO BS EN 15223-1:2016 symbol [reference number 5.4.5] symbol? Is this information on the product, on the box label, case, or in product information? 3.3 How do you indicate that Face Shield products are for single use? In the form of a statement or the ISO BS EN 15223-1: 2016 symbol [reference number 5.4.2]? Is this information on the product, box label, case label, or in product information? 3.4 Provide Face Shield product part numbers that have a foam pad that is vented. 3.5 Provide Face Shield product part numbers that have a foam pad that is solid/not vented. 3.6 List all of the foam pad widths and thicknesses you currently offer for your Face Shield products. 3.7 Provide Face Shield product part numbers that have a clear shield. 3.8 Provide Face Shield product part numbers that have a tinted shield. 3.9 Provide Face Shield product part numbers that have anti-glare properties. Which industry anti-glare standard test do your face shield products meet? 3.10 Provide Face Shield product part numbers that have anti-fog properties. Which industry anti-fog standard test do your face shield products meet? 3.11 Provide all of the widths and lengths of the Face Shield products you currently offer. � 3.12 List all of the headband attachment styles you currently offer for your Face Shield products. 3.13 List all of the headband circumferences you currently offer for your Face Shield products. 3.14 List all of the headband widths and thickness for the Face Shield products you currently offer. 4.0 Additional Feedback Please complete the attached RFI Vendor Response Worksheet with indicated information about your Face Shields. 4.1 Have your Face Shield products been subject to any recalls? If so, what corrective actions and/or product modifications have been made in response to the recalls? 4.2 Do you have any emerging technology for Face Shield products that you would like to share? 4.3 Please provide any additional information including relative topics outside of the specific information requested, which you consider relevant. Any information provided in response to this RFI will be considered and treated as unrestricted information, which can be used by the Government in any future planning or acquisitions.
