- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart� 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-04. FEMA Region 1 have identified a requirement for 200,000 Non-Cellulose �N-95 Size Large�Respirators Brand Name or Equal�for the�COVID-19 response efforts. N-95 Masks must be manufactured by a company that is approved by the Center for Disease Control and Prevention (CDC), and must be National Institute of Occupational Safety and Health (NIOSH) approved. The manufacturer and mask product number must be listed on the CDC list. CDC List: https://www.cdc.gov/niosh/npptl/topics/respirators/disp_part/n95list1sect3.html If quoting KN-95 Masks,� A non-NIOSH approved or KN95 mask must be manufactured by a company that is listed on the Food and Drug Administration (FDA) Emergency Use Authorization, Appendix A under the Personal Protective Equipment (PPE) Section. Emergency Use Authroization (EUA) information list: https://www.fda.gov/emergency-preparedness-and-response/mcm-legal-regulatory-and-policy-framework/emergency-use-authorization Evaluation Critera: 1) Respirators must meet the certifications from approved manufacturers above, and size requested. 2) Meet or exceed the�20 day delivery requirement from time of award. 3)� Price (If two or more offerors meet the above referenced certifications and have identical lead time price will be considered in the determination of the winning offer). 4) Provide past performance history of similiar scope or magnitude. In accordance with FAR 52.212-1 (b) (10) � regarding past performance, references are requested.�� Each Contractor must submit references which are either current or recent customers, and provide the name, address, phone number, fax and email for each reference.The Contractor shall list all current or prior federal contracts, total dollar amounts, contract name, agency, address, telephone number, email, fax, and a brief description of the project performed.� If the offeror has no federal contracting experience, then he/shall send aforementioned references of contracts held in the following order:� state, local, and private.�� All quotes must include a valid, and current�certification from the list of approved CDC or EUA lists above, a product spec sheet referencing the manufacturer name, Country of Origin,�model # of respirators being quoted,and an�accurate and valid delivery timeframe for respirators. Partial deliveries will be considered only if able to meet the 20 day delivery requirement from time of award. Vendors must also provide�any relevant past performance pertaining to this RFP.���These respirators�are an urgent need for the COVID-19 response efforts. At this time, advanced payments are not authorized. Please submit your offer as one single attachment to include all of the requested information, company information, and contact information. Late submissions will not be considered. Delivery Location: 12 Forge Parkway Franklin, MA 02038 USA All contractors have to be registered in the SAM (System for Award Management) Database https://www.sam.gov prior to contract award pursuant to FAR provision 5.207. Therefore prospective offerors are encouraged to register prior to the submittal of quotations/proposals.Once the solicitation is posted, it is incumbent upon the interested parties to review this site frequently for any updates/amendments to all documents. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition, 52.212-2- Evaluation- Commercial Items is applicable. The provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, applies to this solicitation.
