Description
This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government. This notice is intended strictly for Market Research. This is a Request for Information ONLY. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The Department of Veterans Affairs North Texas Healthcare System in Dallas, Texas, intends to award a firm-fixed price contract, for the purchase of Dental Aligners (See below Statement of Work for more information). The Government is conducting a market survey to help determine the availability and technical capability of qualified service-disabled veteran-owned small businesses, veteran-owned small businesses, small businesses, HUBZone small businesses and/or other large businesses capable of serving the needs identified below. This notice of intent is for open market as well as Federal Supply Schedule items. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women-owned small business, HUB Zone, 8(a), small business or large business, relative to NAICS 339114. Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov or GSA E-Buy at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. ***Brand-name ONLY *** Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. If you are an authorized distributor or reseller, please provide a letter of authorization from the original equipment manufacturer. Please advise if you carry the products for this requirement. Please submit all information to Myrtis Clemons at Myrtis Clemons @va.gov no later than Friday, April 17, 2020, 5:00 PM ET (4:00 PM CT). STATEMENT OF WORK Orthodontics 1. General Description:� The South Texas Veterans Health Care System (STVHCS) needs a company to: Make aligners to move patients teeth into more ideal orientation to decrease trauma from occlusion, improve phonetics and esthetics and for pre-prosthetic work. Maintain an open account with The South Texas Veterans Health Care System (STVHCS) and make available computer software to view, edit and approve Orthodontic tooth movements. The Company will accept patient analogue impressions and digital scans from STVHCS. The Company will provide impression trays via mail for the making of analogue impressions. The Company will follow the STVHCS doctor s prescription and treatment plan the tooth movements and propose the tooth movements. The Company will accept and incorporate the STVHCS doctor s changes requests and after final approval will manufacture the aligners for the case. The Company will accept Mid-Course Corrections and refinements as defined by the different case choices. The Company will ship the aligners after fabrication. The Company will provide annual training and support to the dentist and residents at STVHCS. Contract will be Indefinite Delivery Indefinite Quantity with Base Year (qty 6 per yr.) and four Option Years (qty 21 per yr). 2. Clinical Duties:� The contractor will design and fabricate aligners to move the patient s teeth as prescribed by the treating STVHCS Dentist. 3.�� Administration: �Contractor will bill on a monthly basis. �All services rendered will be itemized by date of service ������������ 4.� �Security and Confidentiality: Responsible for maintaining Automated Data Processing (ADP) security. Responsible for maintaining patient information in the strictest confidence in accordance with the Privacy Act. ������������ �������� 5.�� �Work Hours: The services covered by this contract shall be furnished by the contractor as defined herein.� The contractor shall not be required, except in case of�emergency, to furnish such services on a national holiday or during non-working hours as described below. �Work hours:� Monday through Friday, 7:00 am 4:30 pm. 6. Delivery Orders: Orders normally will be placed by 5 p.m. delivery shall be required by 6:30 am the following workday via secure webpage and/or phone. Due to the demands of the VA Medical Center care requirements, there shall be no restriction to the number of orders which may be placed by VA, nor the number of deliveries required per day. In the event that the requirements may be determined in advance, a longer delivery time will be scheduled accordingly. The delivery order shall specify: contract item (s) and quantity, time (s) of delivery and applicable VA Purchase Order number. Orders can be placed via the internet. 7. Delivery Location: South Texas Veterans Health Care System Date Point Dental Clinic 8410 Datapoint Drive San Antonio, Texas 78229 Deliveries will be made Monday through Friday (excluding holidays which are listed below) between the hours of 7:00am and 4:30pm, except in the case of emergency. Federal Holidays are as follows and include any other days specifically declared by the President Of the United States to be deemed a holiday. New Year s Day Martin Luther King s Birthday Presidents Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas Deliveries will be supervised by the COR or their designee before beginning delivery operations. Contractor will contact the COR prior to delivery to arrange for delivery supervision. 8. Delivery Commitment: Time of delivery specified or mutually agreed to at the time of receipt of telephone order shall become mandatory upon the Contractor. Failure to perform in accordance with the delivery commitment may be grounds for termination of contract in accordance with the provisions for default. 9. Person Authorized to Place Orders: In addition to the Contracting Officer, the COR is authorized to place orders under this contract, as well as any AEGD Program Director employed by the VA Hospital. 10. The services specified herein may be changed by written modification. The modification will be prepared by the VA Contracting Officer.
