- This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources (small and other than small) to perform a potential requirement. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 238220 and SBA Sized Standard is $16.5M. PSC J041 NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry https://www.vip.vetbiz.va.gov/. Statement of Work: Period of Performance: Base Year: 06/012020-08/01/2020 REPLACE ROOF LEADER, APPROX 80 PROJECT OVERVIEW, OBJECTIVES, AND DESCRIPTION OF WORK Objectives and Description of Work Contractor to provide supervision, labor, material, to replace approx. 80 of 4 drainpipe, PVC schedule 40 piping. 1.0.2 Work Location Work to be performed is in the Chiller Plant at the Bowling Campus, 1101 Veterans Drive, Lexington Kentucky 40517 Project Work Tasks/Description or Work 1. Mobilize to site during business hours Monday through Friday 2. Remove the decayed roof leader piping, approx. 80 3. Contractor is responsible to discard all removed piping after demolition is complete. 4. All work is below grade and some means of lift/ lowering device will be necessary to get a scaffold/ lift down to perform the work. 5. If a scaffold is used then the contractor is responsible for having the scaffolding inspected by a 3rd party before use. 6. Install new 4 x 80 of schedule 40 DWV, PVC piping with the necessary elbows and fittings to complete the task. 7. All piping must be insulated with a 1 thick fiberglass pipe insulation. 8. Upon completion of the project all debris must be removed from the premises. 9. After the completion of the project it will be inspected by COR personnel for acceptance. 10. Demobilize from site. REFERENCES: None Manufacturer recommendations: None REGULATORY Contractor shall comply with all hospital regulations including: 1.2.1.1 No smoking or use of tobacco products within any V.A. building, including construction area. 1.2.1.2 Parking and traffic regulations. 1.2.1.3 Accident reports provided to Engineering Service. 1.2.1.4 Environmental Policy including submitting for approval the Material Safety Data Sheets (MSDS) for all chemicals that s/he intends to bring on site, prior to the chemicals arriving on station. In all cases, non-hazardous chemical products will be utilized, if available, for the specific application involved. 1.2.2 Workmen are subject to rules of the Medical Center applicable to their conduct. 1.2.3 Contractor will clean up area and remove all trash and debris associated with the work as stated above. 2.0 SECURITY 2.0.1 Security Clearance must be coordinated with the COTR. 3.0 CONTRACTOR FIRM/PERSONNEL QUALIFICATIONS 3.0.1 Experience: Contractor shall have a minimum one-year experience with performing this type work. 3.1 QUALITY 3.1.2 Provide one (1) year standard warranty on all material and workmanship from date of completion of the above referenced project. Provide three (3) year manufacturer s warranty on transformer materials from date of completion 4.0 PROJECT COORDINATION 4.0.1 Site Visit Site visit is not required. 4.0.2 Hours of Work All work to be performed Monday through Friday, 8am to 4:30 pm unless requested then approved by the VA. 4.0.3 Utility Outages N/A 4.1 SCHEDULES 4.1.1 Execute work so as to interfere as little as possible with normal functioning of the Medical Center as a whole, including operations of utility services, fire protection systems and any existing equipment and with work being done by others. 4.1.2 Completion time: Contractor will have 30 days after the Notice to Proceed is given. Enter number of Days to complete project. 4.2 GOVERNMENT SUPPORT 4.2.1 Temporary storage will be located on job site The Medical Center Loading Dock may be used only to load or unload equipment and supplies. 4.2.2 Government Furnished Equipment N/A 5.0 SUBMITTALS 5.0.1 Final Inspection/Service Report Submission Format All work will be inspected by VA personnel upon completion and paperwork submitted in MS Word Information Sought Response to this announcement shall not exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide the service as described above (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code; (3) Information should be provided electronically in a Microsoft Word or Adobe PDF format.
