- N95 or KN95 Masks Department of Veterans Affairs (VA) Veterans Health Administration (VHA) Veterans Integrated Service Network (VISN) 4 Lebanon VAMC, Lebanon, PA 17042 Scope VISN 4 has determined the need to purchase N95 or KN95 masks needed for potential COVID-19 scenarios. This is a time sensitive requirement in response to the COVID-19 pandemic and is vital in the protection of the Veterans Affairs healthcare staff. Justification VISN 4 is requesting N95 or KN95 masks to protect VA Employees while administering care during the COVID 19 Pandemic. Product Requirements and Certifications N95 Masks QTY: 100,000 The N95 mask must be manufactured by a company that is approved by Center for Disease Control and Prevention (CDC) National Institute for Occupational Safety and Health�(NIOSH) approved. The manufacturer and mask product number must be listed on the CDC list. The link for the CDC list is: https://www.cdc.gov/niosh/npptl/topics/respirators/disp_part/N95list1sect3-t.html KN95 Masks QTY: 100,000 A non-NIOSH approved or KN95 mask must be manufactured by a company that is listed on the Food and Drug Administration (FDA) Emergency Use Authorization, Appendix A under the Personal Protective Equipment (PPE) Section. The most recent list is found in Attachment A. The link for the FDA Emergency Use Authorization page is: https://www.fda.gov/emergency-preparedness-and-response/mcm-legal-regulatory-and-policy-framework/emergency-use-authorization NOTE: Simply being registered on the FDA site does not qualify. Award will be made for CLIN 0001 OR CLIN 0002, but not both. Quotes provided will be considered based on the following: (1) Meets the certifications described above (2) Shortest delivery time frames (3) Price (If two or more offerors meet the above referenced certifications and have identical lead time price will be considered in the determination of the winning offer). All bids must include the certifications referenced above. A product/specifications sheet must be included with the bid. The bid must also include the exact manufacturer s name and mask product number and easily found on either the FDA Emergency Use Authorization list or the CDC NIOSH list depending on the mask being offered (N95 OR KN95) as stated above. Submission of Quotes: This is a solicitation for one or the other. Selection and award will be made based upon the Certifications described above, Shortest delivery time frames and Price if two or more offerors meet the above referenced certifications and have identical lead time price will be considered in the determination of the winning offer). Combinations of these two types of masks will not be considered. The request is for 100,000 of one or the other. All questions are to be submitted to Victoria.Roberts4@va.gov. Quotes shall be submitted no later than 12:00 PM EST on Tuesday. April 14th, 2020 to Victoria.Roberts4@va.gov. Late quotes will not be accepted or considered. Quotes shall indicate whether N95 masks OR KN95 masks are being offered. All quotes MUST include certifications referenced in Part 3 of this statement of need. Offers must also supply exact manufacturers name and mask product number. Offers not including the stated above will not be further considered. Delivery: All items are to be delivered to the following address NLT 5/8/2020. Lebanon VA Medical Center Attn: Howard C. Schappell 1700 South Lincoln Avenue Lebanon, PA 17042 These items are required in preparation of COVID-19 and are being requested to be delivered within the next 30 days. Timeliness of delivery is paramount. Soonest delivery of these products is imperative.
