- Page 5 of 5 This request for information (RFI) is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. This notice is published to conduct market research to determine if there is a sufficient number of certified Service-Disabled Veteran Owned Small Businesses (SDVOSB) and/or Veteran Owned Small Businesses (VOSB), capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. Registration may be accomplished at http://www.vip.vetbiz.gov/ and www.sba.gov/, respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance (3) intentions of subcontracting program to include set-asides if any; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) If you are a distributor a statement from the manufacturer that your company is an authorized distributor; (6) a statement to inform the government if you are a manufacturer or distributor; (7) any information regarding FSS Schedule availability; (8) SAM Registration (9) any other pertinent company documentation. Responses are due by April 17, 2020 at 4:30 pm MST. It is anticipated that a solicitation may be issued in May 2020. If issued, award of a subsequent contract is contemplated in June/July 2020. Electronic submissions are acceptable via Eric.Carroll@va.gov, and are highly preferred. NO ADDITIONAL INFORMATION IS AVAILABLE OTHER THAN WHAT IS NOTED HEREIN. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION. The CSP CRPCC requires annual maintenance services for its material handling equipment including two forklift trucks, five pallet jacks, and one scissor lift located in the CSP CRPCC warehouse. Maintenance services will be provided as three quarterly Service plans and one Annual service plan. The required services are to include fluids, unlimited labor, travel, telephone service, lodging, and any other collateral expenses required to service and perform Preventive Maintenance (PM) and emergency repairs. Any required parts will be procured separately from this contract as needed. It is requested that responses include a rough estimate of pricing for completion of the requirements identified below. Purpose: The Cooperative Studies Program Clinical Research Pharmacy Coordinating Center (CSP CRPCC) requires annual maintenance services for its material handling equipment including two Toyota Forklift Trucks, five Toyota Electric Pallet Jacks, and one Genie Scissor Lift located in the CSP CRPCC warehouse. Maintenance services shall be provided as three quarterly Service plans and one Annual service plan. The required services are to include fluids, unlimited labor, travel, telephone service, lodging, and any other collateral expenses required to service and perform Preventive Maintenance (PM) and emergency repairs. Any required parts will be procured separately from this contract as needed. Required Services: Contract service shall ensure that the equipment functions in conformance with the latest published edition of the applicable industry standards and the Toyota or Genie Manufacturer's Performance standards/ specifications as used when the equipment was originally procured and that any upgrades/updates must meet the stated standards/specifications. GASOLINE POWERED EQUIPMENT Blow out radiator core and clean machine with compressed air. Clean grease fittings and lubricant. Fill clutch throwout bearing grease cap. Lubricate distributor, generator. Change engine oil and filter element. Check transmission and differential oil lever; fill if necessary. Clean air cleaner; inspect hose and connections. Check upright assembly for proper adjustment and lubricant. Check battery; add water and clean terminals. Check hydraulic sump filter cap; clean or replace if necessary. Check hydraulic oil level; fill if necessary. Check and lubricate all connecting shafts and levels. Check steering gear box level; fill if necessary. Check power steering oil level; fill if necessary. Check power steering for proper adjustment. Clean protectoseal gas filler cap; inspect for proper installation. Check fuel, oil, water systems for leaks. Check radiator for proper coolant level. Inspect clutch pedal and linkage for proper adjustment. Inspect brake pedal linkage for proper adjustment. Check master cylinder; fill if necessary. Check engine vent pipe. Check all accessory drive belts for proper tension. Check carburetor and accelerator linkage, including choke control. Check distributor points for adjustment and condition. Inspect spark plugs and ignition wiring. Inspect lift and tile cylinders for leaks. Check tilt cylinder rod ends and anchors; adjust and tighten as necessary. Operational check and physical inspection of machine. ELECTRIC POWERED EQUIPMENT Clean machine (compressed air). Clean grease fittings and lubricate. Check upright assembly for proper adjustment and lubricant. Check hydraulic sump filter cap; clean or replace if necessary. Check hydraulic oil level; fill if necessary. Check and lubricate all connecting shafts and levers. Check steering gear box level; fill if necessary. Check steering gear box for proper operation and adjustment. Check differential oil lever; fill if necessary. Inspect lift and tilt cylinders for leaks. Inspect brake pedal linkage for proper adjustment. Inspect battery; check water level. Check battery cables and receptacles. Check contactor assembly and all switches for proper operation. Check tilt cylinder rod ends and anchors; adjust and tighten as necessary. Operational check and physical inspection of machine. Conformance Standards: Contract service shall ensure that the equipment functions in conformance with the latest published edition of the applicable industry standards and the Toyota or Genie Manufacturer's Performance standards/ specifications as used when the equipment was originally procured and that any upgrades/updates will meet the stated standards/specifications. Deliverables: Filed Service Reports shall be provided for all service activities and shall contain the following information: Date(s) and time period of service. Complete description of equipment serviced including model number and serial number, and equipment services performed including upgrades and updates. Complete list of all materials, parts, or assemblies installed or replaced including serial numbers of parts where applicable. A statement of the complaint that initiated the service call. Place of Performance: Delivery of services shall be coordinated through the Biomedical Engineering located at: NMVAHCS CSP CRPCC 2401 Centre Ave SE Albuquerque, NM 87106
