Description
STATEMENT OF WORK 1. SCOPE Furnish all necessary labor, equipment, tools materials, repair service, software updates, parts, etc., for complete On-Site Maintenance and Service as specified below in accordance with the terms, conditions and schedule of this agreement. Equipment to be serviced /maintained includes: FC500 Flow Cytometer with UPS 2. SPECIFIC TASKS AND DELIVERABLES: A. ANNUAL PREVENTIVE MAINTENANCE: Contractor shall furnish all necessary services, including parts and labor (remote and/or on site as needed), to maintain the system in good working condition. This agreement shall include but not be limited to one (1) Preventive Maintenance visit per year. B. TECHNICAL SUPPORT: Unlimited Priority Technical Phone Support (800# telephone support is provided Monday through Friday, 8:00 am to 5:00 pm, CST). C. EMERGENCY REPAIRS: Contractor shall perform unscheduled emergency repairs as necessary to maintain the equipment in operational status. D. EMERGENCY REPAIR PARTS: All repair parts associated with emergency repairs (not including parts repaired or replaced during annual preventive maintenance) shall be paid for outside of this contract. 3. HOURS OF OPERATION: A. Normal hours of operations at the VA are Monday through Friday from 8:00 am to 4:30 pm, excluding holidays. All work will be performed during normal hours of coverage unless requested or approved by COR or her designee. Special Notes: The VA will have access to a technical support hotline at 800-742-2345 during the hours of 8am 5pm. Cost for same is included in the contract price. If however, a problem or malfunction of the equipment cannot be resolved with technical support via telephone, then the Contractor shall be responsible repair the equipment in a timely fashion. Hardware/software update/upgrade installations (if applicable) may be scheduled and performed outside normal hours of coverage at no additional charge to the Government. B. Federal Holidays observed by the VA are: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day And any other day specifically designated as a national holiday by the President of the United States. 4. PLACE OF PERFORMANCE: Jesse Brown VAMC Research and Development 2030 W. Taylor Chicago, IL 60612-4223 5. PERIOD OF PERFORMANCE: Base Year: August 1, 2020 through July 31, 2021 Option Year One: August 1, 2021 through July 31, 2022 Option Year Two: August 1, 2022 through July 31, 2023 Option Year Three: August 1, 2023 through July 31, 2024 Option Year Four: August 1, 2024 through July 31, 2025 6. CONFORMANCE STANDARDS: All work shall be performed and equipment shall function in conformance with manufacturer's/industry standards, the latest published edition of NFPA 99, FDA, OSHA, JCAHO, ISO 9000, UL and other applicable state, local, national and industry standards. Equipment shall meet EMS (Electromagnetic Compatibility) for EIC 601-1-2 CE, CSA, ETL Approved to U.L. 2601-1, second edition. 7. CONTRACTING OFFICER TECHNICAL REPRESENTATIVE (COR): VA Contact Person(s): The VA shall appoint a Contracting Officer s Representative to oversee contract execution. The COR shall be notified on dates and times when all service, warranty repair and/or on-site visits are accomplished. Written service report will be provided to the COR upon completion of each visit, including but not limited to warranty repairs. 8. REPORTS/DOCUMENTATION: For any services performed on site, upon arrival at the Medical Center, the Contractor representative shall check in with the COR before performing any warranty, repairs and/or maintenance services. This check in is mandatory. When the service is completed, the contractor s representative shall document services rendered on a legible ESR(s). A legible signed service report shall be provided directly to the COR following completion of each call, and shall include equipment identification (i.e. serial number), date and time of service call, list of parts replaced, results of inspections and/or calibrations, and a description of work accomplished. The report shall be signed or initialed by the COR or authorized representative. Payment under this contract may be affected if a service report is not provided at the conclusion of each site visit. 9. IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS: The Contractor's FSEs shall wear visible identification at all times while on the premises of the VA. It is the responsibility of the contractor to obtain and pay for parking. The VA does not provide any parking. The VA will not invalidate or make reimbursement for parking violations or charges for the Contractor under any conditions. Smoking is prohibited inside any buildings at the VA. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. 10. DEFINITIONS/ACRONYMS: A.
