Pecos Road, Building 6 North Las Vegas, NV 89086 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, and Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-04, dated 01-15-2020. The associated North American Industrial Classification System (NAICS) code for this procurement is 811219, with a small business size standard of $22.0M. The Department of Veteran Affairs Southern Nevada Health Care System (VASNHCS) is intending to negotiate a sole source award to Draeger Inc. for preventative maintenance service Total Care contract for 10 Draeger Infinity V500 ventilators located at the VA Medical Center, Las Vegas, Nevada for a one year base period, plus four one-year option periods. Evaluation of Options: Except when it is determined in accordance with FAR 17.206(b) not to be in the Government s best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by using the prices offered for the last option period to determine the price for a 6-month option period, which will be added to the base and other option years to arrive at the total price. Evaluation of options will not obligate the Government to exercise the option(s). (Option Year 4) 1 Year $ $ Total Price Quote $ STATEMENT OF WORK Requirement: Draeger Total Care product support services from Draeger Inc. on the ten (10) Draeger Medical Evita V500/VN500 Infinity Ventilator machines. Background. The VA Southern Nevada Health Care System (VASNHCS) is required to keep and maintain the Draeger Evita Infinity V500 ventilators at the VA Medical Center, N. Las Vegas Nevada in operational condition for Respiratory Therapy. The equipment shall be serviced by factory trained personnel who are certified by Draeger Inc. in programming, repair, and system upgrade functions. Draeger Inc is the Original Equipment Manufacturer (OEM) that has been maintaining this equipment and is the only certified service provider for maintenance and repair of the Draeger ventilators at the VASNHCS. Scope: The contractor shall provide all necessary trained experienced personnel, labor, tools, diagnostic equipment and software, material, supplies, transportation, parts and equipment, excluding expendable items to perform repair support of the Draeger Medical Ventilator machines. Service shall consist of scheduled preventive maintenances (PM) (for V/VN500) and scheduled preventive maintenances (PM) and unlimited 24/7 repair calls for all devices listed under contract. Normal Duty Hours: Monday through Friday 7:30 am. to 4:30 p.m., excluding national holidays and weekends. Contractor shall report or telephonically notify the Biomedical Eng. Service at (702) 791-9000 Ext. 14022 [Scott Peltier, Technical Point of Contact (TPOC) of Respiratory Therapy] or (702) 791-9000 Ext. 19079 qw Performance Location: VA Southern Nevada Health Care System Medical Center, Biomed Dept., 5th Floor 6900 N. Pecos Rd. N. Las Vegas, Nevada 89086. After Duty Hours: Contractor shall report to the Administrative Officer (AOD) at the front desk of the hospital to be escorted to the appropriate work area If on-site support is required, Service dispatch will coordinate with other personnel of Draeger Medical to provide this support. This shall be a Turn Key service in that the vendor shall supply all needed services listed above. MFG will provide parts/software necessary in order assure effective repair. Performance Monitoring The VASNHS (TPOC) will monitor each task in this SOW in the following manner: * Installation of the equipment and software will be checked by the VASNHS TPOC to ensure it meets the contract SOW and predetermined capabilities. * Equipment/software lists will be checked by the VASNHS TPOC to ensure all items purchased have been supplied. * Installation activities will be monitored and checked for VASNHS compliances and capabilities. Performance checks will be completed on all installed equipment/software to ensure it meets the performance requirements before acceptance will be given. Security Requirements Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security. A prohibition on unauthorized disclosure: ""Information made available to the contractor or subcontractor by VA for the performance or administration of this contract or information developed by the contractor in performance or administration of the contract shall be used only for those purposes and shall not be used in any other way without the prior written agreement of the VA."" A requirement for data breach notification: Upon discovery of any known or suspected security/privacy incidents, or any unauthorized disclosure of sensitive information, including that contained in system(s) to which the contractor/subcontractor has access, the contractor/subcontractor shall immediately and simultaneously notify the TPOC, the designated ISO, and Privacy Officer for the contract. The term 'security incident' means an event that has, or could have, resulted in unauthorized access to, loss or damage to VA assets, or sensitive information, or an action that breaches VA security procedures. A requirement to pay liquidated damages in the event of a data breach: ""In the event of a data breach or privacy incident involving any SPI the contractor processes or maintains under this contract, the contractor shall be liable to VA for liquidated damages for a specified amount per affected individual to cover the cost of providing credit protection services to those individuals."" A requirement for annual security/privacy awareness training: ""Before being granted access to VA information or information systems, all contractor employees and subcontractor employees requiring such access shall complete on an annual basis either: (i) the VA security/privacy awareness training (contains VA's security/privacy requirements) within 1 week of the initiation of the contract, or (ii) security awareness training provided or arranged by the contractor that conforms to VA's security/privacy requirements as delineated in the hard copy of the VA security awareness training provided to the contractor. If the contractor provides their own training that conforms to VA's requirements, they will provide the TPOC or CO, a yearly report (due annually on the date of the contract initiation) stating that all applicable employees involved in VA's contract have received their annual security/privacy training that meets VA's requirements and the total number of employees trained. A requirement to sign VA's Rules of Behavior: ""Before being granted access to VA information or information systems, all contractor employees and subcontractor employees requiring such access shall sign on an annual basis an acknowledgment that they have read, understand, and agree to abide by VA's Contractor Rules of Behavior which is attached to this contract."" Note: If a medical device vendor anticipates that the services under the contract will be performed by 10 or more individuals, the Contractor Rules of Behavior may be signed by the vendor's designated representative. The contract must reflect by signing the Rules of Behavior on behalf of the vendor that the designated representative agrees to ensure that all such individuals review and understand the Contractor Rules of Behavior when accessing VA's information and information systems. Bio-Medical devices and other equipment or systems containing media (hard drives, optical disks, etc.) with VA sensitive information must not be returned to the vendor at the end of lease, for trade-in, or other purposes."" Contractor Considerations: Risk Control The VASNHS is an active hospital and therefore will require consideration to those areas with patients and staff. It may be necessary for the contractor to seal, wrap, and clean any medical equipment below the ceiling, cable trays, and hallways. Draeger support technicians will return equipment as found.
