It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract.� Requests for a solicitation will not receive a response.� Responses to this sources sought must be in writing.� The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below.� Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition.� Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and If respondent is VOSB or SDVOSB, respondent is to provide proof of www.vetbiz.gov certification. In addition, SDVOSB/VOSB Contractors are asked to acknowledge that they understand the limitations on sub-contracting pursuant to FAR 52.219-14, Limitations on Sub-Contracting, which will be included in the impending solicitation. Furthermore SDVOSB/VOSB contractors are asked to respond that they are capable of providing the requested services keeping within the parameters of this clause. If an Offeror proposes to use any subcontractors, joint ventures, or teaming arrangements the Offeror shall include the percentage and type of work the subcontractor will accomplish to include their certifications. (Any subcontractors, joint ventures, or teaming partners shall have the required certifications as described in the sources sought notice) (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below and The Network Contract Office (NCO) 22 Gilbert is seeking a potential qualified contractor for: Southern AZ VA Health Care System (SAVAHCS) located at 3601 S. 6th Avenue, Tucson, Arizona 85723 REQUIREMENT: SERVICE: The Southern AZ VA Health Care System hereafter (SAVAHCS) is requesting services for vendor to provide: The contractor will perform on-site preventive maintenance (PM) inspection on equipment/systems as provided for in the contract agreement. PMs will be accomplished in accordance with the Original Equipment Manufacturer (OEM) recommendations. Must use OEM certified parts and service engineer/technicians must be OEM trained on all equipment included in agreement. Toll free support. Unlimited service calls with on-site response within 24 hours All parts covered (wearables such as needles, syringes, etc. are not included). Travel and labor included. Designated Response time 48 hours unless otherwise agreed upon by COR or designee Response: All requests for repair service will be considered requests for emergency service unless a statement to the contrary is made by the COR, POC or designee. A competent service representative adequately equipped with tools, test instruments, service manuals, and repair parts will arrive at the SAVAHCS within the designated response time. The service representative may contact VA staff by phone before arrival at the SAVAHCS. Such contacts, however, will not serve to alter the requirements for arrival on-site except when the request for service is withdrawn by Biomedical Engineering. EQUIPMENT: Boston Scientific Labsystem Pro Recording System and Micropace Stimcore Equipment and Accessories MODEL: LS PRO EE#87104 SN:1D417183 MODEL: LS9900 EE#92687 SN:1969022 MODEL: MICROPACE EE#80840 SN:3516 Notification and Response Time: The Contractor shall provide an emergency telephone number for purposes of accepting service calls, available 24 hours a day, 7 days a week. Privacy & Information Security Training: Contractor and their sub-contractors assigned work under the contract are required to receive annual training on patient privacy as established by HIPAA statues. Training must meet VHA s and/or the Department of Health and Human Services Standards for Privacy of Individually-identifiable health information. Privacy and HIPPA training required to be equivalent to VA Privacy and Information Security Awareness and Rules of Behavior (TMS 10176), which COTR must monitor and report to least yearly. Contractor shall provide certification to the VA upon request that all employees and sub-contractor employees assigned work and/or having access to Protected Health Information have received annual training.� 6500.6/Appendix A/Item 6 Service does not require connection to a VA Network. Therefore, Certification and Accreditation requirements do not apply and a Security Accreditation Package is not required. http://www.va.gov/vapubs/viewPublication.asp?Pub_ID=471&FType=2 Important information:� The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this Sources Sought Synopsis request.� This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach.� The Government will in no way be bound to this information if any solicitation is issued.� The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Business set aside. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined.� No sub-contracting opportunity is anticipated.� The North American Classification System (NAICS) code for this acquisition is 811219 ($20.5 million). Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA).� Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at www.vip.vetbiz.gov.� All interested Offerors should submit information by e-mail to Kim.Leone@va.gov include the solicitation in number in the subject line of email.
