Description
This notice is to all prime contractors and their subcontractors with contracts that were awarded and/or are administered by Pearl Harbor Naval Shipyard and Intermediate Maintenance Facility (PHNSY & IMF).� Effective immediately, contractor employees meeting any of the five criteria below are restricted from entering Joint Base Pearl Harbor Hickam:����� Contractor employees traveling from outside of the island of Oahu are restricted from entering Joint Base Pearl Harbor Hickam for 14 days after arrival. � Contractor employees with symptoms of fever, cough, and/or shortness of breath are restricted from entering Joint Base Pearl Harbor Hickam.� � Contractor employees who have others living in their house that traveled to, from or through a Travel Health Notice (THN) Level 3 or Level 2 country are restricted from entering Joint Base Pearl Harbor Hickam for 14 days.� The 14-day period applies and starts from the day the traveler returned to the United States.� � Contractor employees who have others living in their house that identified as having COVID-19 are restricted from entering Joint Base Pearl Harbor Hickam for 14 days.� The 14-day period applies and starts from the day�when one of the others was identified as having COVID-19.� � � Contractor employees who are infected with COVID-19, or at higher risk for serious illness from, COVID-19 are restricted from entering Joint Base Pearl Harbor Hickam, in accordance with Centers for Disease Control and Prevention (CDC) guidelines, as well as the Privacy Act of 1974, and other level requirements.� Refer to CDC guidelines at https://www.cdc.gov/coronavirus/2019-ncov/specific-groups/high-risk-complications.html. � Contractors shall notify the PHNSY & IMF contracting officer if they become aware that any of their employees including subcontractor employees who would otherwise seek to access Joint Base Pearl Harbor Hickam as part of performing contract requirements meet one or more of the five criteria above.� The notice will provide the name of the contractor employee, identify the applicable criterion or criteria, and explain (for criteria (1), (2), (3), or (4)) when the 14 day period concludes.�� Contractors expected to have personnel requiring access to U.S. Navy ships All ships (i.e. Surface Ships, Submarines, and Barges) at PHNSY & IMF will begin screening for COVID-19 at the brow prior to boarding by asking questions, and asking for permission to test the individual for fever based on the answers.� The following questions will be asked, but not limited to: Have you been outside the United States in the past 14 days?� Have you been to any U.S. State that is at Health Protection Condition (HPCON) Level Bravo � Moderate � Increased Community Transmission in the past 14 days.� Reference COVID-19 US Public Health Risk Levels for Naval Forces.� See website below for reference.� https://www.med.navy.mil/sites/nmcphc/program-and-policy-support/pages/novel-coronavirus.aspx Are you currently sick?� Do you have any symptoms � Fever, chills, cough, sore throat, shortness of breath, body aches, abdominal pain?� In past 14 days, have you had close personal contact, as defined below, with anyone known to be infected with COVID-19?� Within 6 feet In a confined space (cab, small room, shared stateroom, berthing proximity, office, etc.) Had direct contact with secretions (been coughed on, sneezed on, etc) Have you visited any medical facility in the past 14 days for flu, cold or acute respiratory symptoms?� This notice may be amended, updated, and cancelled at any time and without prior notice to conform with the evolving national, state and local requirements.�
