Johnson VA Medical Center 109 Bee Street Charleston SC 29403-5799 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-03, December 23, 2019. The associated North American Industrial Classification System (NAICS) code for this procurement is 238210, with a small business size standard of $16.5 Million dollars. All interested companies shall provide a quotation for the following: STATEMENT OF WORK (SOW) GENERAL INFORMATION 534-20-2-5034-0395 Rev Original TITLE OF PROJECT: Exterior Lighting Repair and Maintenance SCOPE OF WORK: The Contractor shall provide personnel, materials, lifts or other equipment, and services to the Myrtle Beach CBOC (Community Based Outpatient Clinic) located at 3381 Phillis Blvd., Myrtle Beach. BACKGROUND: The exterior lighting fixtures were inspected, and numerous deficiencies were found. Troubleshooting, repair, maintenance, and testing is required to ensure all the exterior lighting fixtures are 100% operational as designed, to include control systems, work as designed. PERFORMANCE PERIOD: The contractor shall provide the requirements under this Statement of Work (SOW) as described below and as directed. Work is expected to begin within 90 days of award, defined as the date the contract is signed by the Contractor, and finished within 90 days of start of work. TYPE OF CONTRACT: Firm Fixed Price GENERAL REQUIREMENTS These following specifications are a statement of the minimum level of work and services that are to be provided under this contract. They are not intended to be, nor shall they be construed as, limiting specifications or requirements. At a minimum, the contractor shall be required to take all steps and measures which would be taken by a prudent building owner to ensure that the materials supplied, and work performed will provide satisfactory performance. Contractor will provide a crew with appropriate supervision to perform troubleshooting, repair, and testing as required, to include materials, labor, lifts, test equipment, or other equipment, and documentation proving full and complete operation of all exterior lighting. Upon completion of repairs, operational tests and inspection shall occur to insure completely operational as designed, prior to acceptance. The lighting repairs shall include at a minimum replacing the following fixtures with LED type fixtures (entire fixture, not just the lamps) (NOTE: equivalent to XXX watts below means equivalent to the stated wattage incandescent, halogen or mercury vapor lighting commonly used on commercial exterior areas). Replace quantity of six (6), 30-foot pole light fixture heads with LED fixtures rated at lumens equivalent to 450 watts each, retaining the poles. Replace quantity of twelve wall mounted wall pack light fixtures with LED fixtures rated at lumens equivalent to 200 watts each. Replace quantity of one (1) ceiling mounted light fixture with LED fixture rated at lumens equivalent to 200 watts located at exterior door. Replace quantity of two (2) flagpole flood type light fixtures on the roof with LED fixture rated at lumens equivalent to 400 watts. Replace quantity of six (6) ceiling mounted canopy/carport lights with LED fixtures rated at lumens equivalent to 100 watts each. Troubleshoot and repair lighting controls. Ensure timers, photocells and lighting contactor/relays are completely operational. Replace all photocells associated with the exterior lighting system with new, proper type and rating for LED fixtures with new photocells. Submit cut sheets and information for all fixtures to be used in Contractor bid for approval. The Government (RH Johnson Veteran s Administration Medical Center, RHJVAMC or VA) retains the right to refuse any fixture the VA deems unacceptable and require re-submission. CHANGES TO STATEMENT OF WORK Any changes to this Statement of Work (SOW) shall be authorized and approved only through written correspondence from the Contracting Officer (CO). A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the Contractor through the actions of the parties other than the CO shall be borne by the Contractor. REPORTING REQUIREMENTS The Contractor shall obtain all necessary licenses and/or permits required to perform the work. In addition, the Contractor shall provide the CO with a copy of any licenses and/or permits obtained. GOVERNMENT RESPONSIBILITIES The VA shall grant the Contractor access permission to all areas of the Myrtle Beach CBOC exterior areas located at 3381 Phillis Blvd., Myrtle Beach as necessary to provide the services required under this contract. The VA will make all necessary areas available, 24 hours/day, 7 days/week exclusive of Federal holidays, the Contractor should inform contacts listed above at least two (2) working days prior notice to facilitate access to required work areas. CONTRACTOR REQUIREMENTS The Contractor shall provide personnel, materials and services to the RHJVAMC as detailed in this SOW. Contractor will coordinate with RHJVAMC supervision at least two (2) working day in advance in order to facilitate access to required work areas, at least one (1) working days notice if work will be performed outside of normal working hours defined as 07:00 to 17:30 hours local time, Monday through Friday exclusive of Federal holidays. Any licenses or permits necessary are the responsibility of the contractor and copies will be provided to the RHJVAMC within 30 days of completion of work. CONFIDENTIALITY AND NON-DISCLOSURE It is agreed that: The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the work scoped by this document. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS The C&A requirements do not apply, and that a Security Accreditation Package is not required. The government will evaluate information based on the following evaluation criteria: (i) technical capability to meet the Government requirement; (ii) price; and (iii) past performance. Technical: The vendor s quote shall be evaluated to determine if offer has the capabilities to install exterior lighting fixtures as detailed in the Statement of Work. To be considered technically acceptable for award each vendor must provide with their submitted quote/offer: The vendor must provide documentation of all state and federal licenses and/or certifications necessary to install exterior lighting fixtures as identified in the statement of work. The vendor must provide a narrative on the overall technical approach methods and procedures used to meet the requirements identified in the Statement of Work. This narrative should provide evidence that the vendor understands the objectives that the contract intends to meet, the nature of the required work and the level of effort necessary to accomplish all the work. Past Performance: Vendor must provide three (3) or more references of work for installing exterior lighting fixtures as detailed in the Statement of Work. References must include contact information; brief description of the work completed, and contract # (if relevant). References may be checked by the Contracting Officer to ensure offer can perform the required services as outlined in the Statement of Work. The Government also reserves the right to obtain information for use in the evaluation of past performance from any and all sources. Price: Vendor shall complete the Price/Cost Schedule in Section (vii), with vendors proposed contract line item number prices inserted in appropriate spaces. Price shall be evaluated for fair and reasonableness in accordance with FAR 12.209. The price stated in the offer s Price/Cost Schedule shall be inclusive of all necessary and appropriate labor, administrative functions, insurance coverage, and all other costs to install exterior lighting fixtures in accordance with the terms, conditions, specifications, and requirements as specified in the Statement of Work.
